Experiences of COVID-19 survivors in Gauteng province, South Africa: socioecological implications for South Africa's preparedness for future pandemics.

Gauteng province was at the epicentre of the COVID-19 pandemic in South Africa. An understanding of the experiences of survivors in this setting has the potential to inform South Africa's preparedness and response in future pandemics. We aimed to explore the experiences of survivors via a qualitative study conducted in Tshwane and Sedibeng districts, Gauteng. One-on-one interviews were conducted on 20 purposefully sampled survivors and carers. Analysis was guided by the framework methods. An explanatory model was developed using the social ecological model. As a result, eight major themes emer

A state wise analysis of the socioeconomic and health impacts of the COVID-19 pandemic in India: lessons for future health system preparedness.

BACKGROUND: The COVID-19 pandemic significantly affected individuals, society, and the national economy. However, there is limited information on the socioeconomic dimension of COVID-19-related health impacts from nationally representative large datasets using an integrated approach. Such information is crucial for tailored policy responses and national preparedness planning. Our study aimed to assess the state-level health and economic impacts of COVID-19 in India. METHODS: The COVID-19 data on age and gender distribution were collated from the Registrar General of India, state dashboards, a

An investment too good to be true?: Reassessing the World Health Organization and World Bank return-on-investment estimates for pandemic preparedness.

The World Health Organization, World Bank, G20 and related health agencies have requested annual investments of US$31.1 billion in pandemic prevention, preparedness and response. To justify these unprecedented costs, they rely on a return-on-investment case developed by WHO and World Bank. Any investment in one area of public health will have knock-on effects on others, through diversion of funds and human resources. Intended outcomes must outweigh alternate investments and be likely to achieve the results on which such comparisons are predicated. This perspective examines the assumptions unde

Financing fragility and pandemic preparedness in Central Asia: a policy review of aid volatility, donor dynamics, and health system resilience.

BACKGROUND: The COVID-19 pandemic exposed the structural fragility of health systems built on volatile external financing. Central Asia-Kazakhstan, Kyrgyzstan, Tajikistan, Turkmenistan, and Uzbekistan-receives health financing from three divergent donor streams: OECD bilateral and multilateral channels, Chinese Belt and Road Initiative mechanisms, and Eurasian Economic Union frameworks. The 2025 suspension of USAID operations, which eliminated 69-78% of program funding in Kyrgyzstan and Tajikistan, transformed chronic financing instability into an acute system-level shock. The relationship bet

Integrating Oral Health Into Pandemic Preparedness and Response: Missed Opportunities and Strategic Imperatives for Global Health System Resilience and Security.

BACKGROUND: Despite evidence linking oral health to severe COVID-19 outcomes and its potential role in surveillance and service delivery, integration remained absent. This study enquired about: (1) representation of oral health in national COVID-19 taskforces and funding; (2) policy-level barriers to inclusion; (3) lessons from maintaining essential services; and (4) strategies for mobilizing oral health professionals in future emergencies. METHODS: We conducted a scoping review guided by the PRISMA-ScR framework. Systematic searches were performed in PubMed/MEDLINE and Scopus (2000-2025) usi

The emergence of highly pathogenic avian influenza H5N1 in dairy cattle: implications for public health, animal health, and pandemic preparedness.

BACKGROUND: The 2024 outbreak of Highly Pathogenic Avian Influenza (HPAI) H5N1 in U.S. dairy cattle represents a significant change in the behavior of zoonotic influenza viruses. Previously, H5N1 was primarily an avian pathogen with limited infection in mammals. The emergence of clade 2.3.4.4b H5N1 in dairy herds across multiple states reveals the virus's adaptation to mammalian hosts and highlights its potential for zoonotic transmission, raising important concerns for public health, veterinary medicine, and agriculture. RESULTS: The virus demonstrated unique tropism for mammary tissue, with

Evolution of WHO Influenza Antiviral Stockpile: Rapid Access and Use in Low- and Middle-Income Countries for Pandemic Preparedness and Response.

The SARS outbreak and influenza A(H5N1) infections (2003-2004) prompted WHO to establish a global influenza antiviral stockpile, enabling rapid distribution to 72 countries during the 2009 A(H1N1) pandemic. To improve access in low- and middle-income countries, WHO added antivirals to the WHO Model List of Essential Medicines and included them in the Prequalification Programme. The 2011 Pandemic Influenza Preparedness Framework refined strategies for equitable access and rapid response. Lessons from COVID-19 led to a new WHO-led mechanism-the Interim Medical Countermeasures Network (i-MCM-net)

Who would take part in a pandemic preparedness cohort study? The role of vaccine-related affective polarisation: Cross-sectional survey.

Cohort studies are an important tool for public health research about emerging infectious diseases. Willingness to participate in research is associated with multiple factors. Little is known about the role of people's feelings, especially in the aftermath of the COVID-19 pandemic. Affective polarisation, an affinity for people with similar attitudes to oneself and hostility toward those with opposing views, is a measure of feelings about issues, including COVID-19 vaccination. The aim of this study was to investigate factors associated with willingness to participate in a future household-bas

Structural gradients in excess mortality during and after the COVID-19 pandemic: economic development, preparedness, and cross-national differences, 2020-2024.

BACKGROUND: Excess mortality has emerged as a comprehensive indicator of overall mortality burden of the COVID-19 pandemic, reflecting both direct and indirect deaths beyond officially reported COVID-19 fatalities. However, the substantial cross-national variation in excess mortality remains incompletely explained, particularly in relation to structural heterogeneity across countries. This study examines whether pandemic mortality differs according to economic development and national preparedness. METHODS: We analysed 59 countries from 2020 to 2024. Excess mortality ratios were estimated bas
