Complete Pathologic Response to Neoadjuvant Chemoimmunotherapy and Oxaliplatin-Induced Fever Associated With IL-6 Release in a Patient With Locally Advanced Colon Cancer.

Neoadjuvant systemic therapy is a preferred treatment approach for a number of tumor types due to many potential advantages over upfront surgery, including tumor downstaging, early treatment of micrometastatic disease, and providing an in vivo test of tumor biology. For colon cancer, current standard of care is upfront surgery followed by adjuvant systemic therapy in high-risk patients. Concerns about inaccurate radiological staging and tumor progression during preoperative treatment, as well the lack of randomized data demonstrating benefit, are among the reasons for the limited use of neoadj

Implications of Pathologic Complete Response Beyond Mediastinal Nodal Clearance With High-Dose Neoadjuvant Chemoradiation Therapy in Locally Advanced, Non-Small Cell Lung Cancer.

PURPOSE: To determine, in a retrospective analysis of a large cohort of stage III non-small cell lung cancer patients treated with curative intent at our institution, whether having a pathologic complete response (pCR) influenced overall survival (OS) or freedom from recurrence (FFR) in patients who underwent definitive (≥60 Gy) neoadjuvant doses of chemoradiation (CRT). METHODS AND MATERIALS: At our institution, 355 patients with locally advanced non-small cell lung cancer were treated with curative intent with definitive CRT (January 2000-December 2013), of whom 111 underwent mediastinal re

[Discussion on treatment strategies for locally recurrent rectal cancer aiming at R0 resection and pathological complete response].

With the implementation of surgical techniques, such as total pelvic exenteration, and improvements in perioperative management, an increasing number of patients with locally recurrent rectal cancer (LRRC) are potential targets for curative surgery. Negative pathological margins (R0 resection) and favorable pathological complete response (pCR) have been identified as independent predictors of better oncological outcomes in LRRC. However, achieving R0 resection and optimal tumor regression remain primary concerns and significant challenges for colorectal surgeons. Based on the latest research b

Pathological complete response with neoadjuvant pembrolizumab and chemotherapy in non-metastatic triple-negative inflammatory breast cancer.

BACKGROUND: The efficacy of neoadjuvant chemoimmunotherapy in patients with non-metastatic triple-negative inflammatory breast cancer (TN-IBC) remains unclear, as patients with IBC have been poorly represented in pivotal clinical trials. PATIENTS AND METHODS: We conducted an observational, retrospective/prospective, multicenter cohort study from 2023 to 2024 of patients with non-metastatic TN-IBC who received at least one cycle of neoadjuvant pembrolizumab plus multi-agent chemotherapy from September 2018 to April 2023. The median follow-up was 1.4 years [95% confidence interval (CI) 1.2-1.7

Is postoperative adjuvant therapy necessary for esophageal cancer patients with pathological complete response after neoadjuvant treatment?

BACKGROUND: Current National Comprehensive Cancer Network (NCCN) guidelines recommend omission of adjuvant therapy in esophageal cancer patients achieving pathological complete response (pCR) following neoadjuvant treatment. However, 20-40% of these patients experience disease recurrence, potentially indicative of residual micrometastatic disease. This retrospective study evaluates survival outcomes between adjuvant therapy and observation in pCR populations. METHODS: We analyzed 184 esophageal cancer patients with confirmed pCR after completed neoadjuvant therapy at Hebei Medical University

MRI radiomics for predicting pathological complete response in breast cancer: a systematic review of current evidence and limitations.

BACKGROUND: Machine learning models based on MRI radiomics show promise for predicting pathological complete response (pCR) to neoadjuvant therapy in breast cancer, with increasing interest in clinical translation. However, uncertainties remain regarding methodological quality, generalizability, and performance across patient subgroups. This systematic review provides a comprehensive evaluation of MRI radiomics models for pCR prediction, focusing on methodological robustness, validation practices, and factors affecting equitable performance. METHODS: The review protocol was registered with PR

Circulating and Tissue MicroRNA Profiles Associated with Pathological Complete Response in HER2+ Breast Cancer: A Systematic Review.

MicroRNAs (miRNAs) have emerged as promising biomarkers for treatment response prediction in breast cancer. However, the role of circulating and tumour-derived miRNAs in predicting pathological complete response (pCR) following neoadjuvant therapy in HER2-positive (HER2+) breast cancer remains incompletely defined. To systematically evaluate the association between miRNA expression profiles and pCR in HER2+ breast cancer undergoing neoadjuvant systemic therapy, including chemotherapy with HER2-targeted agents. [Pathological Complete Response after Combined Chemotherapy of Durvalumab, Gemcitabine and Cisplatin for Intrahepatic Cholangiocarcinoma-A Case Report].

The patient was a 66-year-old man. He came to our hospital because of abdominal discomfort. A CT scan revealed hypovascular tumors of about 70 mm in the S4/5 and 16 mm in the S6 of the liver. The S4/5 tumor was directly invasive into the greater omentum and abdominal wall, and a nodule suspected of peritoneal dissemination was observed around the tumor. Based on tumor biopsy results, a diagnosis of intrahepatic cholangiocarcinoma(ICC), cT3N0M1, Stage ⅣB was made. A total of 9 courses of GCD therapy(gemcitabine+cisplatin+durvalumab)were performed for ICC and the tumor was found to have shrunk,

Complete pathological response after neoadjuvant chemotherapy and the benefit of subsequent surgery in oligometastatic pancreatic cancer patient.

INTRODUCTION: Pancreatic cancer is one of the most aggressive tumors diagnosed in local-ly advanced or metastatic stage in more than half of the cases. The standard of care is a systemic chemotherapy but the prognosis of metastatic patients remains extremely poor with a median overall survival less than one year.  However, there is increasing evidence of surgery treatment benefit in a carefully selected oligometastatic cases.  -Because oligometastatic pancreatic cancer is rare, there is a lack of robust clinical trials defining strategy, efficacy and safety of this procedure. PATIENT CONCERNS

Evolving approaches in advanced gallbladder cancer with complete pathological response using chemo‑immunotherapy: A case report.

The combination of chemotherapy and immunotherapy for metastatic cholangiocarcinoma (CCA) offers promising improvements in survival and response rates beyond traditional treatments. TOPAZ-1 and KEYNOTE-966 have demonstrated the efficacy of combining immunotherapy (durvalumab and pembrolizumab) with chemotherapy, even in gallbladder cancer (GBC), with a complete response rate of 2.7% in the TOPAZ-1 trial. Advanced CCA treated with immunotherapy combinations has shown complete responses influenced by high programmed death-ligand 1 (PD-L1) or Epstein-Barr virus expression. These responses were en

Neoadjuvant Lorlatinib Induces Pathological Complete Response in Advanced ALK-Positive Lung Cancer: A Case Report.

BACKGROUND Chemotherapy has been the conventional treatment method for advanced non-small-cell lung cancer (NSCLC). Nevertheless, the identification and comprehension of oncogenic driver alterations have paved the way for targeted therapies, significantly enhancing patient outcomes. The management of locally advanced NSCLC that is positive for ALK presents a challenge due to the lack of reported randomized controlled trials. The efficacy of neoadjuvant and adjuvant targeted therapy in this context remains uncertain. CASE REPORT A 54-year-old man was diagnosed with stage IIIB (pT1N3M0) upper ri

Neoadjuvant durvalumab improves survival in early triple-negative breast cancer independent of pathological complete response.

BACKGROUND: Addition of immune checkpoint inhibitors to neoadjuvant chemotherapy (NACT) is a promising strategy in early breast cancer, but the optimal duration of therapy is currently unknown. In the GeparNuevo (NCT02685059) trial, addition of durvalumab to NACT as previously reported led to a moderate increase in pathological complete response (pCR) rate by an absolute 9% (P = 0.287). PATIENTS AND METHODS: Patients with cT1b-cT4a-d triple-negative breast cancer (TNBC) received durvalumab 1.5 g or placebo every 4 weeks added to nab-paclitaxel 125 mg/m2 weekly for 12 weeks, followed by durval
