Brucella suis bacteremia misidentified as Ochrobactrum anthropi by the VITEK 2 system.

Ochrobactrum and Brucella are genetically related genera of the family Brucellaceae, sharing 98.8% rRNA similarity. Because of their phenotypic similarity, Ochrobactrum can be miscoded as Brucella by automated identification systems. The misidentification on blood cultures (BCs) of B. suis as O. anthropi by the VITEK 2 system is herein described. A 67-year-old male with a prosthetic mitral valve and fever was admitted with bacteremia due to a Gram-negative coccobacillus identified as O. anthropi by VITEK 2. The patient's fever persisted along with positive blood cultures despite specific antim

Design of a pragmatic randomized implementation effectiveness trial testing a health system wide hypertension program for older adults.

Hypertension control remains poor. Multiple barriers at the level of patients, providers, and health systems interfere with implementation of hypertension guidelines and effective lowering of BP. Some strategies such as self-measured blood pressure (SMBP) and remote management by pharmacists are safe and effectively lower BP but have not been effectively implemented. In this study, we combine such evidence-based strategies to build a remote hypertension program and test its effectiveness and implementation in large health systems. This randomized, controlled, pragmatic type I hybrid implementa

Dissociative disturbance in Hangul-Hanja reading after a left posterior occipital lesion.

Since the Korean language has two distinct writing systems, phonogram (Hangul) and ideogram (Hanja: Chinese characters), alexia can present with dissociative disturbances in reading between the two systems. A 74-year-old right-handed man presented with a prominent reading impairment in Hangul with agraphia of both Hangul and Hanja after a left posterior occipital- parietal lesion. He could not recognize single syllable words and nonwords in Hangul, and visual errors were predominant in both Hanja reading and the Korean Boston Naming Test. In addition, he had difficulties in visuoperceptual tes

Community-acquired pulmonary infection caused by Bordetella hinzii: An under-recognized pathogen.

INTRODUCTION: Bordetella hinzii is an emerging Gram-negative pathogen increasingly recognized in humans, especially in patients with chronic lung disease. PATIENTS AND METHODS: A 70-year-old woman with a single lung and recurrent fungal infections presented with progressive respiratory symptoms. Sputum samples were analyzed, and identification was attempted using routine automated systems and confirmed with MALDI-TOF mass spectrometry. Antimicrobial susceptibility was assessed based on MIC determination, current EUCAST guidance for organisms lacking species-specific breakpoints, and compariso

Designing a Cardiac Arrest System That Includes Extracorporeal Cardiopulmonary Resuscitation: A Contemporary Review.

Extracorporeal cardiopulmonary resuscitation for refractory out-of-hospital cardiac arrest is increasingly used, with survival rates up to ≈30% reported in select highly organized systems, although outcomes vary widely across programs. However, outcomes are driven largely by system performance rather than the procedure itself. To date, most literature has focused on efficacy, patient selection, and cannulation techniques, with limited emphasis on system-level design. This narrative review synthesizes the key components required to implement an effective extracorporeal cardiopulmonary resuscita

A case report of an M protein-negative patient with POEMS syndrome associated with renal involvement.

BACKGROUND: POEMS syndrome with polyneuropathy, organomegaly, endocrinopathy, M protein, and skin changes is an uncommon plasma cell paraneoplastic syndrome involving multiple system. It is relatively rare in clinical practice, and renal involvement is a usual yet easily overlooked symptom. CASE PRESENTATION: We successfully treated a patient with M protein-negative POEMS syndrome with membranoproliferative glomerulonephritis (MPGN) findings and thrombotic microangiopathic changes by comparing the level of Vascular endothelial growth factor (VEGF) in the serum and the changes in polyserositis

Disparities in the Use of Internet and Telephone Medication Refills among Linguistically Diverse Patients.

BACKGROUND: Health systems are increasingly implementing remote telephone and Internet refill systems to enhance patient access to medication refills. Remote refill systems may provide an effective approach for improving medication non-adherence, but more research is needed among patients with limited English proficiency with poor access to remote refill systems. OBJECTIVE: To compare the use of remote medication refill systems among limited-English-proficiency (LEP) and English-proficient (EP) patients with chronic conditions. METHODS: Cross-sectional survey in six languages/dialects (Engli

Systematic identification of rare disease patients in electronic health records enables evaluation of clinical outcomes.

Identifying rare disease (RD) patients in electronic health records (EHRs) is difficult, as most of the over 10,000 RDs are not adequately captured by standard coding systems. To address this, we developed a semi-automated workflow to map RDs to SNOMED-CT and ICD-10 codes, enabling improved RD identification across EHR systems. The optimized workflow yielded 88.4% true RD codes in a subset of 1,715 manually curated diseases. Using this workflow and starting with 12,003 GARD IDs mapped to ORPHANET, we obtained 12,081 SNOMED-CT and 357 ICD-10 codes representing 6,342 RDs, organized into 30 ORPHA

From Amyloid Exposure to Genetic Causality: Converging hiPSC-Based Models of Alzheimer's Disease.

Alzheimer's disease (AD) is a neurodegenerative disorder in which genetic susceptibility, aging, protein aggregation, and neuroglial dysfunction converge to drive progressive neuronal loss. Although animal models and conventional cell systems have provided fundamental insights into pathomechanisms, they only partially reproduce the human-specific and multifactorial nature of AD. Human induced pluripotent stem cell (hiPSC)-based models have emerged as a powerful platform to investigate disease-relevant processes within a patient-specific genetic background while enabling the generation of multi

Optimising ICU supply chains: Evidence-based strategies for efficiency, resilience and patient safety-a structured narrative review.

Inventory management in intensive care units (ICUs) plays a critical role in ensuring uninterrupted patient care, yet it remains an underprioritised component of healthcare operations. This narrative review explores current challenges and evidence-based strategies for optimising ICU supply chains. Issues such as unpredictable demand, manual stock tracking, procurement inefficiencies and limited staff training often contribute to stockouts, expired supplies and delayed interventions. The COVID-19 pandemic further exposed systemic vulnerabilities, emphasising the need for resilient, technology-d

Impact of an electronic health record transition on chemotherapy error reporting.

BACKGROUND: Incident reporting systems allow for frontline employees to report errors and are a critical component of healthcare patient safety programs. Although incident reporting systems cannot quantify total errors, organizations can utilize incident reporting systems to help identify risks and trends to act upon. The objective of this article is to utilize incident reporting systems to evaluate trends in medication error reporting before and after implementation of a new electronic health record system. METHODS: A five-month pre- and post-analysis was completed in a cancer hospital follo

[Risk assessment].

The aim of preoperative risk analysis is to reduce postoperative morbidity and mortality by identification of compromised organ function which can be improved by targeted preoperative measures and problem-oriented postoperative therapy. Ideally, therefore, preoperative risk assessment, influences the timing of the surgical procedure, the choice of the surgical approach, and the postoperative management. Identification of preexisting relevant disorders that may influence the postoperative course is an essential prerequisite of risk analysis. While preoperative risk analysis today gains in impor

Coordinator-based systems for secondary prevention in fragility fracture patients.

The underlying causes of incident fractures--bone fragility and the tendency to fall--remain under-diagnosed and under-treated. This care gap in secondary prevention must be addressed to minimise both the debilitating consequences of subsequent fractures for patients and the associated economic burden to healthcare systems. Clinical systems aimed at ensuring appropriate management of patients following fracture have been developed around the world. A systematic review of the literature showed that 65% of systems reported include a dedicated coordinator who acts as the link between the orthopae

A case report of diagnosis of cat-scratch disease using metagenomic next-generation sequencing.

Cat-scratch disease (CSD) is an anthropozoonotic infection caused by Bartonella henselae, and it is one of the most common causes of lymph node infections in children and adolescents. B. henselae, belonging to the genus Bartonella, is a common human pathogen of human beings. CSD commonly develops as a result of cat scratches and bites or when injured skin comes into contact with cat saliva. The manifestation of CSD clinically differs for each patient based on their immune system. Individuals who have healthy immune systems generally manifest minimal clinical symptoms and do not necessitate any

In vivo bio-imaging using chlorotoxin-based conjugates.

Surgical resection remains the primary component of cancer therapy. The precision required to successfully separate cancer tissue from normal tissue relies heavily on the surgeon's ability to delineate the tumor margins. Despite recent advances in surgical guidance and monitoring systems, intra-operative identification of these margins remains imprecise and directly influences patient prognosis. If the surgeon had improved tools to distinguish these margins, tumor progression and unacceptable morbidity could be avoided. In this article, we review the history of chlorotoxin and its tumor specif

Chronic suppurative joint effusion due to burkholderia pseudomallei: a case report.

Burkholderia pseudomallei, a Gram-negative bacillus is the causative agent of Melioidosis, a glanders-like disease, primarily a disease of animals. Melioidosis has been only a rare and sporadic disease in humans outside its endemic region. Currently, diagnosis of B. pseudomallei in the clinical laboratory is very difficult, owing to low awareness of physicians to the nonspecific clinical manifestations, lack of responsiveness among microbiologists outside endemic areas, identification systems in the average sentinel laboratory, and the biosafety conditions necessary to process these organisms.

[Accurate diagnosis of Pseudomonas luteola in routine microbiology laboratory: on the occasion of two isolates].

Pseudomonas luteola which was previously known as Chryseomonas luteola; is a gram-negative, non-fermentative, aerobic, motile, non-spore-forming bacillus. It is frequently found as a saprophyte in soil, water and other damp environments and is an opportunistic pathogen in patients with underlying medical disorders or with indwelling catheters. It has been reported as an uncommon cause of bacteremia, sepsis, septic arthritis, meningitis, endocarditis, and peritonitis. Thus, early and accurate identification of this rare species is important for the treatment and also to provide information abou

CONORM-DEID: Robustness Evaluation of a Multilingual De-Identification System for Clinical Texts.

Clinical narratives contain rich, detailed information that is essential for medical research but often locked behind privacy constraints due to the presence of personally identifiable information. To enable secondary use while protecting patient confidentiality, these texts must undergo de-identification. Because even a few missed instances can lead to disclosure, systems must achieve high recall. In this work, we present CONORM-DEID, a multilingual model for clinical text de-identification, and investigate how large-scale synthetic pre-training and the availability of human-annotated data im

First case of human spondylodiscitis due to Shewanella algae.

We present the first case of human spondylodiscitis due to Shewanella algae. Our patient did not have any predisposing factors. The portal of entry was probably a cutaneous lesion on the leg, exposed to seawater. Bacteria were isolated in pure culture from a needle biopsy specimen of the vertebral disk. Automated identification systems identified the organism as Shewanella putrefaciens. However, molecular biology identified it as S. algae. Treatment with ceftriaxone and amikacin, then ciprofloxacin successfully addressed the infection. We also review four published cases of human osteoarticula
