Tezepelumab Efficacy in Patients with Severe, Uncontrolled Asthma with Comorbid Nasal Polyps in NAVIGATOR.

PURPOSE: Tezepelumab, a human monoclonal antibody, blocks thymic stromal lymphopoietin. In the phase 3 NAVIGATOR study (NCT03347279), tezepelumab reduced annualized asthma exacerbation rates (AAERs) versus placebo, irrespective of baseline disease characteristics, and improved lung function and symptom control versus placebo in adults and adolescents with severe, uncontrolled asthma. We assessed the efficacy of tezepelumab in patients with severe asthma with or without nasal polyps (NPs) in the 2 years before randomization in NAVIGATOR. METHODS: Patients with severe asthma (N=1059) were rando

Navigating Parotid Lymphoepithelial Cysts in HIV Patients: A Tale of Two Distinct Scenarios.

A rare occurrence in HIV-infected individuals is the development of diffuse infiltrative lymphocytic syndrome (DILS), which is marked by a widespread infiltration of CD8+ lymphocytes in body tissues, persistent elevation of CD8+ lymphocyte levels, as well as bilateral parotid swellings and cervical lymphadenopathy. It is distinct due to its suspected autoimmune origin and is found in about 5-10% of people living with HIV. This retrospective analysis involves two patients admitted to our tertiary care rural hospital with complaints of bilateral parotid swellings, a provisional diagnosis of DILS

Gout Flare Burden, Diagnosis, and Management: Navigating Care in Older Patients with Comorbidity.

Gout is the most common form of inflammatory arthritis, and its incidence is highest in middle-aged and older patients. Adding to the diagnostic complexity, up to 50% of patients aged > 65 years present atypically, with subacute oligo- or polyarticular flares. Comorbidity and polypharmacy, common in older populations, affect real-world treatment decisions in gout management, and no specific guidelines are available to address these issues in these at-risk groups. Despite the growing public health burden posed by gout, suboptimal management has led to increased morbidity and substantial healthc

Advocating for Patients With Inflammatory Bowel Disease: How to Navigate the Prior Authorization Process.

In an effort to manage health care costs and avoid improper medication use, prior authorizations (PAs) have become a standard stipulation required by payers in the determination of medication coverage. For gastroenterologists managing patients, especially those with inflammatory bowel disease (IBD), the PA process is time-consuming and further complicated by 2 additional factors: step therapy requirements and failure of payers to recognize updated IBD treatment pathways. These factors often lead to PA denials and cause treatment delays, which in turn can lead to disease progression, ongoing pa

Navigating patient death and grief in German neurosurgical residency: a mixed-methods study.

OBJECTIVE: Neurosurgical residents encounter patient death during their training, yet the literature lacks understanding of their experiences and responses. The aim of this study was to explore how residents navigate patient death, communicate with families, and engage with coping and support mechanisms during their neurosurgical training in Germany. METHODS: The authors conducted a qualitative-dominant mixed-methods study among neurosurgical residents in Germany. The qualitative component included semistructured in-depth interviews and focus group discussions with neurosurgical residents. Th

Discretionary Compromises: How Clinicians Navigate Competing Values in Cardiovascular Patient Prioritisation.

Formal criteria and evidence-based guidelines commonly frame healthcare prioritisation, yet everyday prioritisation often occurs in organisational 'grey zones' where competing values coexist. Drawing on interviews with clinicians in two Norwegian cardiology wards, we examine how prioritisation is justified and enacted under institutional pluralism and moral ambiguity. Using Boltanski and Thévenot's framework of 'orders of worth', we show how clinicians develop discretionary compromises: situated adjustments that combine competing evaluative principles into hybrid justifications, making contest

"Medical Cannabis": Advising Patients While Navigating Medical Ethics and Limited Evidence.

Cannabis is increasingly legalized in more areas of the USA and marketed under the misnomer "medical cannabis." Yet, there are no official dosing guidelines or approved medical uses for plant cannabis by the Food and Drug Administration. Due to significant federal barriers, research on efficacy, safety, and adverse effects of cannabis is limited. Patients are interested in cannabis as potential treatment of medical issues and/or symptoms and clinicians should include a discussion of the risks of cannabis consumption. However, many risks are not yet known, and clinicians might be unaware of the

Navigating the patient journey in migraine prevention: An American Migraine Foundation position paper.

OBJECTIVE: This study aimed to understand the factors limiting access to medications for the preventive treatment of migraine and to improve access to evidence-based preventive care. BACKGROUND: For decades, the effective use of medication for the preventive treatment of migraine was limited by slow onset, slow and complex dose titration schedules, modest benefits, drug interactions, frequent side effects, and very low long-term adherence. The calcitonin gene-related peptide (CGRP) targeted preventive medications mitigate some of these limitations and demonstrated substantial therapeutic bene

Navigating the clinical challenges of zolbetuximab in patients with claudin positive advanced gastric cancer.

Zolbetuximab, a first-in-class monoclonal antibody targeting Claudin 18.2 (CLDN 18.2), has demonstrated significant survival benefit when combined with chemotherapy in HER2-negative, CLDN 18.2-positive advanced gastric/gastroesophageal junction (G/GEJ) adenocarcinoma. However, its integration into clinical practice presents two major challenges: the best treatment selection between zolbetuximab or immune checkpoint inhibitor (ICI)-based regimens in patients co-expressing programmed death-ligand 1 (PD-L1), and the management of zolbetuximab-related nausea and vomiting (N/V). We critically revie

Advanced chronic kidney disease coexisting with heart failure: navigating patients' management.

Chronic kidney disease (CKD) and heart failure (HF) are interrelated, mutually exacerbating conditions. HF in patients with moderate to severe CKD poses unique clinical problems. Indeed, considerations related to specific concomitant derangements, such as vascular calcification, inflammation, and proteinuria, inform and demand personalized treatment strategies. Pharmacological interventions, including renin-angiotensin system antagonists, sodium-glucose cotransporter 2 (SGLT2) inhibitors and novel mineralocorticoid receptor blockers are valuable in managing these complex conditions, although f

Navigating Psoriatic Arthritis: Treatment Pathways and Patient-Specific Strategies for Improved Outcomes.

Psoriatic arthritis (PsA) is a multifaceted chronic immune-mediated disease characterized by joint, skin, nail and entheseal involvement, affecting approximately 0.3-1% of the global population. In recent years, the treatment options for PsA have expanded from traditional nonsteroidal anti-inflammatory drugs and conventional synthetic disease-modifying antirheumatic drugs (DMARDs) to include biologic DMARDs and targeted synthetic DMARDs. Owing to the heterogeneity of the disease and prevalence of comorbidities, the selection and sequence of treatment are often unclear. In this narrative review

Navigating Care Challenges in Elderly Patients Following Hypoglossal Nerve Stimulator Implantation.

INTRODUCTION: Hypoglossal nerve stimulation (HNS) "Inspire© therapy" has garnered popularity among obstructive sleep apnea (OSA) patients seeking an alternative to continuous positive airway pressure (CPAP) therapy. The growth in HNS has been particularly high in older adults living with OSA. Consistent and proper use of HNS in the geriatric population faces unique age-associated barriers: a high rate of multiple chronic conditions (MCC) and polypharmacy (being on five or more drugs). Early recognition and patient-centered management of these barriers will allow older patients to obtain maximu

Factors affecting the ability of patients with complex vascular anomalies to navigate the healthcare system.

BACKGROUND: Vascular anomalies (VAs) are rare congenital disorders that can cause pain, disfigurement, coagulopathy, asymmetric growth, and disability. Patients with complex VAs experience multiple barriers to accessing expert care. It is imperative to understand which factors support these patients' ability to navigate the healthcare system. RESULTS: We surveyed adult patients with VAs using previously validated measures, recruiting participants from five patient advocacy groups and multidisciplinary VA clinics. The primary outcome was self-reported ability to access needed medical care, usi
