BACKGROUND: Patient and family engagement (PFE) in the intensive care unit (ICU) enhances patient safety through detection and reporting of safety incidents that may be missed by healthcare professionals, providing an important and additional safeguard against harm. However, empirical evidence to inform policy and practice in resource-limited critical care settings remains scarce.
OBJECTIVES: The aim of this study was to explore the perspectives and experiences of patients, families, and healthcare professionals on PFE in patient safety in Ethiopian ICUs.
METHODS: A pragmatic, exploratory qu

Comprehensive and accurate information about the medicines available to them is a key issue for patients to participate in the decision making process of their own healthcare. One aspect of this information is the patient information leaflet provided by the manufacturer, according to national and international guidelines. The comprehensibility and user-friendliness of these manufacturers' leaflets has been tested and criticised. In addition to this information under the laws of national and international authorities, consumer-oriented websites of self-help groups are becoming a more and more r

Given the high proportion of patients with asthma who remain uncontrolled despite controller treatment, there remains a need for the development of more effective treatment options with a proven safety and tolerability profile. Recently, asthma guidelines have evolved to incorporate new therapies, including long-acting muscarinic antagonists (LAMAs) and biologics. Here we focus on the safety profile of tiotropium, a LAMA, using data from the large-scale UniTinA-asthma® clinical trial program, which investigated the use of tiotropium in over 6000 patients with asthma who remained symptomatic de

Connexin 36 (Cx36), a neuronal gap junction protein, is essential for retinal signal transmission but may also contribute to neurodegeneration by facilitating the spread of excitotoxic stress signals. This study aimed to examine the degradation pathway of Cx36 under stress conditions relevant to retinal disease. Using immortalized 661W murine photoreceptor cells transfected with GFP-tagged Cx36, we evaluated the effects of lysosomal inhibition (Bafilomycin A1) and excitotoxic stimulation (NMDA). Immunofluorescence revealed intracellular Cx36 accumulation following Bafilomycin and NMDA exposure

BACKGROUND: Onychomycosis and dermatomycoses can result in serious complications in patients with underlying chronic diseases such as diabetes. To avoid these complications, these dermatological disorders need to be treated efficiently, for example with the triazole antifungal itraconazole. Itraconazole can inhibit the metabolism of drugs by CYP 3A4 and therefore might affect the efficacy of antidiabetic agents.
OBJECTIVE: To investigate this, we assessed the safety of itraconazole in diabetic patients with onychomycosis or dermatomycoses.
METHODS: We reviewed pharmacokinetic and safety data

Background/Objectives: Psychological safety is increasingly recognized as a key factor in promoting communication, learning, and teamwork in healthcare, with implications for patient safety culture. Although psychological safety and patient safety culture have been reviewed separately, evidence on how nurse managers contribute to both constructs within nursing teams remains fragmented. This scoping review aimed to map the available evidence on the role of nurse managers in promoting psychological safety and patient safety culture in nursing teams, identify leadership behaviors, strategies, and

BACKGROUND: National Dutch guidelines have been introduced to improve suboptimal perioperative care. A multifaceted implementation programme (IMPlementatie Richtlijnen Operatieve VEiligheid [IMPROVE]) has been developed to support hospitals in applying these guidelines. This study evaluated the effectiveness of IMPROVE on guideline adherence and the association between guideline adherence and patient safety.
METHODS: Nine hospitals participated in this unblinded, superiority, stepped-wedge, cluster RCT in patients with major noncardiac surgery (mortality risk ≥1%). IMPROVE consisted of educat

Background Patient safety culture is a critical determinant of healthcare quality, particularly in high-risk environments such as hemodialysis units, where complex procedures, advanced technology, and vulnerable patient populations increase the likelihood of adverse events. Despite global emphasis on patient safety, evidence regarding safety culture in hemodialysis settings within Saudi Arabia remains limited. This study aimed to assess patient safety culture and identify its work-related predictors among hemodialysis nurses in Jeddah, Saudi Arabia, using the Hospital Survey on Patient Safety

BACKGROUND: Patient safety incidents (PSIs) frequently occur in primary care and are often considered to be preventable. Better knowledge of factors contributing to PSIs is required to build safer care. The aim of this work was to describe the underlying factors, specifically the human factors, that are associated with PSIs in primary care using CADYA ("CAtégorisation des DYsfonctionnements en Ambulatoire" or "Categorization of Errors in Primary Care").
METHODS: We followed a mixed method with content analysis and coding in CADYA of PSIs reported in the ESPRIT study, a French cross-sectional

BACKGROUND: Dipyridamole imaging is widely used as an alternative to exercise testing to identify and risk stratify patients with coronary artery disease. Safety data on intravenous dipyridamole stress testing has been derived largely from individual institutional data.
METHODS AND RESULTS: Data were collected retrospectively by 85 coinvestigators from 73,806 patients who underwent intravenous dipyridamole stress imaging in 59 hospitals and 19 countries to determine the incidence of major adverse reactions during testing. The dose of dipyridamole infused was 0.56 mg/kg in 64,740 patients, 0.7

Artificial intelligence (AI) has progressed from technical research to routine clinical use, reaching an inflection point where technological capabilities may exceed current regulatory and oversight frameworks. These systems are becoming more complex, progressing from narrow, task-specific algorithms to foundation models and early agentic prototypes. This progression has redistributed risk, responsibility, and clinical judgment, requiring radiologists and health care leaders to understand how policy choices affect patient safety and clinical innovation advancement. This special report provides
