Hot Chip Challenge: Acute Gastrointestinal Bleeding in a Pediatric Patient on Left Ventricular Assist Device.

Gastrointestinal bleeding (GIB) is the most common and potentially life-threatening bleeding complication in pediatric patients on left ventricular assist devices (LVAD), particularly under anticoagulation. We report a 13-year-old female with dilated cardiomyopathy on HeartMate 3 support who developed acute GIB after participating in a "Hot Chip Challenge," consuming excessive spicy chips. The bleeding required blood transfusion and intensive management. This case highlights the potential impact of lifestyle factors, especially dietary choices, on GIB risk in pediatric LVAD patients.

Supplemental Nutrition Assistance Program participation gaps within a pediatric leukemia clinical trial cohort.

Poverty-exposed children with cancer are more likely to experience adverse outcomes. Supplemental Nutrition Assistance Program (SNAP) benefits improve food insecurity and child health outcomes, and could be used to mitigate disparities. We conducted a secondary analysis of parent-reported data collected in a frontline pediatric leukemia trial (NCT03020030) to assess SNAP eligibility (proxied by other means-tested program participation) and participation. At diagnosis, 105/287 families (37%) were SNAP-eligible, of whom 53 (50%) were SNAP participants. At 6 months, 104/257 families (41%) were SN

Quality of life in pediatric patients on a paracorporeal ventricular assist device with a novel mobile driving system.

BACKGROUND: EXCOR ventricular assist device (VAD) is the gold standard circulatory support for children with end-stage heart failure. Until recently, the only available driving unit was the stationary Ikus. This study (NCT04634708) investigates the impact of the novel mobile EXCOR Active driving unit on patients' mobility and the quality of life (QoL) of their families. METHODS: Children on EXCOR VAD support with the Ikus who were mobilizable outside the hospital room were eligible for the prospective study. Patients remained on Ikus for 7 days, then switched to Excor Active, and were observe

Technology-assisted Pain Management in Pediatric and Adolescent Burn Care: A Systematic Review and Meta-analysis.

OBJECTIVE: Burn injuries are one of the leading causes of disability in the pediatric and adolescent population globally. Current practice in pediatric burn management uses opioid analgesics as acute pain management. Technology-assisted pain management, including the application of video games and virtual reality (VR), offers an attractive alternative that can be used to help alleviate pain and anxiety in pediatric and adolescent patients with burns. This systematic review and meta-analysis aim to update and collate the most recent evidence on the effectiveness of said technologies in reducing

Durable left ventricular assist devices in pediatrics: impact of body size on outcomes and size limitations.

Despite the range of body sizes in children, few ventricular assist devices (VAD) exist to support pediatric patients with end-stage heart failure. Large registry data identified weight < 20 kg to be associated with higher rates of VAD-related stroke, compared to > 40 kg. Moreover, patients < 1 years of age experience the highest post-implant mortality, with 1-year survival improving in an age-dependent manner. Within different VAD types, intracorporeal continuous (IC) devices confer the greatest clinical benefit and quality of life compared to paracorporeal alternatives. The major limitation

Surgical Correction of Ankyloglossia Using Diode Laser-Assisted Frenectomy in a Pediatric Patient: A Case Report.

Tongue-tie is a continuation of the lingual frenum that is attached to the tip of the tongue. It is a congenital oral anomaly that could restrict tongue movements, caused by a lingual frenum a membrane that originates from the floor of the mouth to the bottom of the tongue that is too thick and short, which limits the natural ability of the tongue to move and function. The tongue is an auxiliary organ that facilitates speaking, mastication, and deglutition. This condition may result in several difficulties including chewing, breastfeeding, speech, and pronunciation of particular words, as well

Effect of the bileaflet inlet valve angle on the flow of a pediatric ventricular assist device: Experimental analysis.

BACKGROUND: Mechanical heart valves (MHV) and its fluid dynamics inside a pulsatile pediatric ventricular assist device (PVAD) can be associated with blood degradation. In this article, flow structures are analyzed and compared by an experimental investigation on the effect of bileaflet MHV positioned at varying angles in the inlet port orifice of a PVAD. METHODS: Time-resolved particle image velocimetry was applied to characterize the internal flow of the device. St Jude Medical bileaftlet valves were used on the inlet orifice and positioned at 0°, 15°, 30°, 45°, 60°, and 90° in relation to

Fractional CO2 laser-assisted topical rifamycin drug delivery in the treatment of pediatric cutaneous leishmaniasis.

Cutaneous leishmaniasis is challenging to treat. Various drugs have been proposed to manage this condition, with variable results. In this case report, we describe laser-assisted delivery of rifamycin to treat this infection. Two sessions of fractional CO2 laser were performed one month apart. Each was followed by a topical application of rifamycin for three days. Resolution with minimal scarring was obtained, suggesting this technique might be safe and effective in treating cutaneous leishmaniasis.

Codetection of pulmonary tuberculosis and toxoplasmosis in a pediatric bronchoalveolar lavage specimen: A cytologist's assistance to clinical management.

BACKGROUND: Toxoplasmosis causes serious and sometimes life-threatening disease in immunocompromised patients like organ transplant recipients, immunodeficiency disorders or HIV-infected individuals. The co-occurrence of toxoplasmosis in a respiratory sample harboring tuberculosis (TB) may be missed especially in an area endemic for the latter infection. CASE REPORT: A 10-year-old child presented with complaints of fever with loss of appetite and weight. Based on radiological and clinical features, a presumptive diagnosis of pulmonary TB was made and bronchoalveolar lavage (BAL) performed for

Endoscopically assisted Crawford tube placement results in shorter general anesthesia times in pediatric patients.

BACKGROUND: Crawford tube placement is commonly used to achieve patency of nasolacrimal ducts for epiphora secondary to nasolacrimal duct obstruction. The nasal passages of pediatric patients are narrower than adults, and the result is a relatively higher risk of intranasal complications (e.g., synechiae, bleeding) with Crawford tube placement. There is evidence that general anesthesia may negatively affect the neurocognitive function and behavioral development of children, which prompts efforts to decrease operation times for potential health benefits and also potentially to reduce health car

Third Generation Ventricular Assist Device: Mid-Term Outcomes of the HeartWare HVAD in Pediatric Patients.

The HeartWare HVAD is a small, third generation continuous flow pump that is intracorporeally placed for support of a failing ventricle in adult patients. This device is small in size when compared to other left ventricular assist devices and can therefore be used in smaller sized pediatric patients. We present our initial experience using the HVAD as a bridge to heart transplantation in the pediatric population. We performed a retrospective, single center, nonrandomized review of 17 pediatric patients who underwent HVAD implantation between June 2013 and March 2016. The primary endpoints eval
