Reconstruction of the diabetic foot using the anterolateral thigh perforator flap.

BACKGROUND: The purpose of this study was to evaluate the outcome of the diabetic foot reconstructed with the anterolateral thigh perforator flap. METHODS: This study reviews 71 cases of salvaged diabetic foot over a 52-month period. Patients ranged in age from 33 to 72 years (average, 51 years), with an average follow-up of 11 months. RESULTS: Flaps survived in all but one reconstructed case, resulting in equivocal findings compared with microvascular free tissue transfer of nondiabetic patients. Early complications such as delayed healing with minor wound dehiscence were seen in three case

Superficial circumflex iliac artery perforator flap for reconstruction of limb defects.

The superficial circumflex iliac artery perforator (SCIP) flap differs from the established groin flap in that it is nourished by only a perforator of the superficial circumflex iliac system and has a short segment (3 to 4 cm in length) of this vascular system. Three cases in which free superficial circumflex iliac artery perforator flaps were successfully transferred for coverage of soft-tissue defects in the limb are described in this article. The advantages of this flap are as follows: no need for deeper and longer dissection for the pedicle vessel, a shorter flap elevation time, possible t

The perforator-based conjoint (chimeric) medial Sural(MEDIAL GASTROCNEMIUS) free flap.

The prototypical conjoint or so-called "chimeric" free flap heretofore has been composed of several large independent flaps, each supplied by a separate major branch, that ultimately arise from a common source vessel. The perforator-based type of chimeric flap is a relatively new concept, usually involving multiple muscle perforator flaps each based on a solitary musculocutaneous perforator, but still arising from the same "mother" vessel. This principle of split cutaneous perforator flaps has been now successfully adapted to the medial suralMEDIAL GASTROCNEMIUS perforator free flap on 2 separ

Using the proximal perforator-based anterolateral thigh flap from the same donor site to salvage the reconstruction after failure of the distal perforator-based anterolateral thigh flap.

We presented a patient who experienced the anterolateral thigh (ALT) flap failure but the reconstruction was successfully salvaged by harvesting a second ALT flap from the same donor site 2 days after the first reconstruction. A 47-year-old man received cancer ablation for right mouth floor squamous cell carcinoma. The resultant defect was planned to be reconstructed with the ALT flap. During the flap dissection, we identified three proximal cutaneous perforators originating from the transverse branch of the lateral circumflex femoral artery (t-LCFA) and two distal cutaneous perforators origin

Medial sural perforator plus island flap: a modification of the medial sural perforator island flap for the reconstruction of postburn knee flexion contractures using burned calf skin.

BACKGROUND: The medial sural perforator island flap may be suitable for the reconstruction of postburn knee flexion contractures. However, postburn knee flexion contractures are usually associated with burns of the calf, which is the donor site of the medial sural perforator flap. Thus, there are concerns regarding the safety of raising medial sural perforator flaps from burned calves. METHODS: Between 2005 and 2010, 12 patients (11 males and 1 female) with postburn knee flexion contractures associated with second-degree burns of the calf (that healed by secondary intention) underwent reconst

Does the Difference in Vascular Anatomy in the Multiple Perforator DIEP Flap Affect the Flap Dissection Time?

BACKGROUND: Multiple perforator deep inferior epigastric perforator (DIEP) flaps demonstrate superior perfusion. The selection of perforators is influenced by several factors, including the number of perforators, the running layer, the pedicle length, and the branching pattern. This study aimed to investigate the relationship between the basic and anatomical characteristics and flap dissection time in patients undergoing unilateral breast reconstruction using multiple perforator DIEP flaps. METHODS: This was a cohort study including 134 patients. Patients were divided into 2 groups based on t

Is the Medial Sural Artery Perforator Flap a New Workhorse Flap? A Systematic Review and Meta-Analysis.

BACKGROUND: The medial sural artery perforator flap offers thin, pliable tissue with a relatively long pedicle and low donor-site morbidity. This study explores the characteristics and uses of the medial sural artery perforator flap along with postoperative outcomes and complications. METHODS: A systematic literature review was performed using PubMed, Embase, and Cochrane Central Register of Controlled Trials to identify all cases of medial sural artery perforator flap reconstruction. Descriptive and meta-analyses were performed on pooled outcomes. Multivariate logistic regression identified

The Effects of Angiosome Morphology on Choke Vessels and Flap Necrosis in a Rat Multiterritory Perforator Flap.

BACKGROUND: Although the angiosome concept has been proposed for a long time, very few studies have been done on its morphology. Our study investigated the effects of angiosome morphology on choke vessels and flap necrosis in a rat multiterritory perforator flap. METHODS: Seventy-two male Sprague-Dawley rats were randomly divided into 3 groups (n = 24/group). The flap contained the right iliolumbar, posterior intercostal, and thoracodorsal angiosomes (TDAVs), termed angiosomes I, II, and III, respectively. Only the posterior intercostal artery and iliolumbar vein were preserved in group 1, wh

Primary and secondary perforator-based flap-in-flap reconstructions of postexcisional head and neck soft tissue defects.

INTRODUCTION: Perforator-based flap-in-flap (PBFIF) refers to the construct of one flap within another based on a perforator. Primary flap-in-flap is the simultaneous construct of two flaps, one within the other. It is particularly useful in cases where despite perfect planning, the flap does not fit congruently into recesses of the defect. It facilitates tension-free flap inset without the need for secondary movement from adjacent areas. Secondary flap-in-flap is the construction of a flap within a previously transferred settled flap. It is particularly useful in cases of wound dehiscence and

Sural perforator flap: assessment of the posterior calf region as donor site for a free fasciocutaneous flap.

Three kinds of free fasciocutaneous flap from the posterior calf region have been described in the literature: the medial sural perforator flap, the lateral sural perforator flap, and the traditional posterior calf fasciocutaneous flap that is supplied by superficial cutaneous vessels. Moreover, it has been reported that superficial cutaneous vessels are of a suitable size for microanastomosis when deep musculocutaneous perforators are absent or relatively tiny. To establish a safe technique for free fasciocutaneous flap elevation from the posterior calf region, we examined the number and loca

Peroneal artery perforator chimeric flap: changing the perspective in free fibula flap use in complex oromandibular reconstruction.

The fibula osteoseptocutaneous flap has undergone multiple refinements since its first description in oromandibular reconstruction. There is now a better understanding of the blood supply to the skin of the lateral aspect of the leg. Multiple free skin paddles can be harvested freestyle from the lateral aspect of the leg. The size of the flaps that can be harvested has not been clearly defined. A case report of a complex oromandibular reconstruction following a shotgun injury to the face demonstrates a way of maximizing the skin harvest. An osteoseptocutaneous fibula flap was used for the repl

The proximal radial artery perforator flap (PRAP-flap): an anatomical study for its use in elbow reconstruction.

We investigated the anatomical vascular basis of the proximal radial artery perforator flap (PRAP-flap) and we report here the first clinical application in a case of reconstructive surgery as an alternative to traditional regional fasciocutaneous pedicled flaps or microsurgery solutions. In 16 unembalmed cadaveric forearms, the radial artery was slowly injected with acrylic resin through the brachial artery and the superficial flexor compartment was dissected. The flap was raised from the lateral and medial sides of the forearm toward the axis represented by the radial artery, and the charact

A novel flap to repair medial and lateral malleolar defects: anterior tibial artery perforator-based adipofascial flap.

Soft tissue coverage of the ankle is a difficult challenge. The adipofascial flap based on perforators of the anterior tibial artery was harvested from the dorsum of the foot and used for repairing of soft tissue defects either on the lateral or medial malleolar region in 7 patients. Two medial and 5 lateral malleolar defects were covered using the flap described. The size of flaps varied from 3.5 x 10 cm to 4.5 x 12 cm. All flaps had good perfusion and survived completely. Donor-site morbidity was minimal in all cases. Both functional and esthetical results were satisfactory. Because of thin

The Microvascular Peroneal Artery Perforator Flap as a "Lifeboat" for Pedicled Flaps.

UNLABELLED: Pedicled perforator flaps have expanded reconstructive options in extremity reconstruction. Despite preoperative mapping, intraoperative findings may require microvascular tissue transfer when no adequate perforators can be found. The free peroneal artery perforator flap may serve as a reliable back-up plan in small defects. METHODS: In 16 patients with small soft tissue defects on the upper and lower extremities, perforator-based propeller flaps were planned. The handheld Doppler device was used to localize potential perforators for a propeller flap in close proximity to the defe

Perforator mapping and clinical experience with the superthin profunda artery perforator flap for reconstruction in the upper and lower extremities.

INTRODUCTION: Thin skin flaps have gained popularity for extremity reconstruction. However, the use of the profunda artery perforator (PAP) flap has not been explored as much. With its concealed donor site on the medial thigh and bulk, the PAP has become popular for reconstruction in the breast, head, and neck. The thickness of the subfascial PAP flap is decreased through elevation on the thin or superthin plane, making it more suitable for extremity reconstruction. METHODS: A consecutive series of 28 patients with 29 flaps who received a thin or superthin single perforator PAP flap for upper

Perforator flaps of the facial artery angiosome.

For small to moderate-sized defects in the head and neck region, local flaps have been the mainstay of reconstruction for years. However, in certain instances, additional flap translation is required be it advancement, transposition or rotation. In such cases, the local flap concept is combined with perforator flap know-how, allowing larger loco-regional flaps to be raised to reconstruct relatively larger defects, even in cosmetically-expensive areas. In our cohort of fifteen patients', we have utilised detailed microanatomy of the facial artery perforators to reconstruct such defects with goo
