Epicardial Lipomatosis as a Cause of Constrictive Pericarditis.

Epicardial lipomatosis is a rare cause of constrictive pericarditis. There is a paucity of literature on the condition, and it is possible that it may be missed in contemporary clinical practice. Herein, we report two patients with epicardial lipomatosis who presented with traditional cardiovascular risk factors. After an initial suspicion of pericardial effusion, both patients underwent multimodality imaging with transthoracic echocardiography, computed tomography, and cardiac magnetic resonance imaging, which demonstrated diffuse epicardial fat. Conservative management was pursued in both ca

Contemporary Assessment and Management of Effusive-Constrictive Pericarditis.

PURPOSE OF REVIEW: Effusive-constrictive pericarditis (ECP) is a complex clinical condition that combines features of pericardial effusion/tamponade and constrictive pericarditis. The classic hemodynamic definition is persistent elevation of right atrial pressure despite drainage of a pericardial effusion. This review summarizes recent data on its epidemiology, pathophysiology, diagnosis, and management. RECENT FINDINGS: Prevalence varies from 2.4% to 14.8% depending on diagnostic criteria and etiology, reaching up to 50% in tuberculous pericarditis in endemic regions. Common causes include id

Constrictive Pericarditis as a Post-Cardiac Surgery Complication.

BACKGROUND Constrictive pericarditis is a chronic inflammatory process characterized by fibrosis and impaired ventricular filling. The diagnosis is challenging because of overlapping clinical features with other pathologies such as cardiac tamponade, and restrictive cardiomyopathy. We report a case of effusive-constrictive pericarditis with cardiac tamponade in a patient with a history of multiple myeloma, bone marrow transplantation, and colchicine treatment. CASE REPORT A 62-year-old woman was admitted to a hospital in Germany due to cardiac tamponade secondary to bacterial pericardial effus

Effusive-Constrictive Pericarditis Associated With Parvovirus B19 Infection.

Parvovirus B19, pathogen of children's fifths disease, is among the causes of viral/idiopathic pericarditis. This paper presents 3 adult cases of complicated effusive-constrictive parvovirus B19 pericarditis presenting with overt diastolic heart failure during an outbreak in September to October 2023. Comprehensive transthoracic echocardiography and cardiac magnetic resonance played a key role in noninvasive assessment of constrictive physiology and pericardial inflammation, guiding successful treatment with low-dose prednisolone, having failed standard colchicine and nonsteroidal anti-inflamm

An eye-opening case report of constrictive pericarditis.

BACKGROUND: Constrictive pericarditis is characterized by the encasement of the heart by a stiff pericardium leading to impaired diastolic function, which ultimately leads to congestive heart failure. CASE SUMMARY: We report a case of a young woman, who first presented to the ophthalmologist with the sudden appearance of floaters and vision reduction. Eventually, invasive haemodynamic assessment led to the diagnosis of constrictive pericarditis leading to venous congestion. CONCLUSION: Understanding the pathophysiology and integrating the results of invasive and non-invasive diagnostic work

Idiopathic Effusive-Constrictive Pericarditis Presented by Variant Angina.

An 80-year-old man presented to our hospital complaining of loss of appetite. During the medical examination, he developed variant angina accompanied with heart failure. Oral calcium channel blocker therapy controlled his variant angina, but medical management of heart failure became increasingly difficult due to gradually increasing pericardial effusion, and pericardiocentesis leading to the diagnosis of effusive-constrictive pericarditis (ECP). Here, we report a rare case of idiopathic pericarditis caused variant angina with already having endothelial dysfunction and eventually developed ECP

Coronavirus disease 2019 (COVID-19)-associated constrictive pericarditis.

This report documents a rare case of COVID-19-associated constrictive pericarditis (CP) in the setting of a recent COVID-19 infection. A 55-year-old man with a history of hypertension and gout presented with acute hypoxic respiratory failure and was diagnosed with COVID-19 pneumonia with progression to acute respiratory distress syndrome. His hospital course was complicated by a large pericardial effusion; an emergent bedside transthoracic echocardiography was concerning for cardiac tamponade, so pericardiocentesis was performed. A workup with cardiac magnetic resonance imaging showed changes

Constrictive pericarditis after heart transplantation: a case report.

BACKGROUND: Constrictive pericarditis (CP) is a disease characterized by inflammation, progressive fibrosis, and thickening of the pericardium. Constrictive pericarditis after heart transplantation (HT) is a rare phenomenon, with a reported incidence of 1.4-3.9%. It is an important clinical problem which shares similar clinical features with entities such as restrictive cardiomyopathy. Therefore, it poses diagnostic challenges and therapeutic dilemmas even for experienced clinicians. CASE SUMMARY: A 53-year-old patient developed a zoster infection with pericardial effusion 9 months after HT f

Surgery for Patients with Radiation-Induced Constrictive Pericarditis.

Radiation-induced heart disease (RIHD) is a major side effect of chest radiation therapy (RT). Most changes of pericardium will occur within a few weeks after receiving chest RT, while most of them will take decades or more to become constrictive pericarditis. Pericardiectomy is an effective treatment method. Here, we report 2 cases of radiation pericarditis after chest RT at our center.

Hourglass-shaped right ventricle and localized constrictive pericarditis.

Constrictive pericarditis is characterized by thick pericardial fibrosis and frequent calcification that progressively impairs diastolic filling of the heart. The diagnosis of constrictive pericarditis has been challenging even though multiple diagnostic modalities have been developed. The diagnosis of constructive pericarditis is especially difficult in localized constrictive pericarditis which is extremely rare. We report a case of localized constrictive pericarditis with a 3D multislice cardiac computed tomography (CT) finding of the constrictive band causing strangulation and hourglass sha

[Effectiveness of the Waffle Procedure for Constrictive Pericarditis].

A 66-year-old woman was referred to our hospital with dyspnea. Right-sided congestive pleural effusion of an unknown etiology was detected and she was diagnosed with constrictive pericarditis. Pericardiectomy was performed via median sternotomy under extracorporeal circulation because severe adhesion was observed. However, hemodynamics did not improve after pericardial resection because of thickend epicardium in front of the right ventricle. Hence, the waffle procedure was additionally performed on the anterior surface of the right ventricle after coming-off the extracorporeal circulation. Alt

A 72-year-old man presented with fever, dyspnea, and weight loss. He was referred to our hospital for further examination of the cause of the pleural effusions. Chest computed tomography showed pleural effusions, a pericardial effusion, and enlarged lymph nodes in the carina tracheae. We administered treatment for heart failure and conducted analyses for a malignant tumor. The pericardial effusion improved, but the pericardium was thickened. Positron emission tomography-computed tomography (PET-CT) showed fluorine-18 deoxyglucose accumulation at the superior fovea of the right clavicle, carina

Constrictive pericarditis: an underdiagnosed cause of chest pain.

Constrictive pericarditis is an underdiagnosed condition characterized by a rigid pericardium leading to restriction in cardiac filling with consequent fluid overload and decreased cardiac output. The authors provide classic echocardiographic and cardiac catheterization images which, in addition to good clinical acumen, assist in making the correct diagnosis.

[Constrictive pericarditis as complication of viral respiratory infection].

A 24 year-old man with 3-months medical history of recurrent respiratory infections and pericardial effusion, despite treatment with nonsteroid anti-inflammatory drugs, was admitted to the hospital with dyspnea on exertion. On admission he presented the symptoms of right heart insufficiency. Computed tomography of the chest demonstrated a thickened pericardium. Echocardiographic examination and right heart catheterisation established the diagnosis of constrictive pericarditis. Serologic tests suggested viral aetiology. The patient was referred to cardiothoracic surgery, partial pericardiectomy

Effusive-constrictive pericarditis: the role of noninvasive imaging.

Effusive-constrictive pericarditis is a rare entity in which clinical suspicion is of paramount importance, and although cardiac catheterization remains the gold standard for diagnosis, noninvasive imaging techniques are very useful in consolidating the diagnosis prior to confirmation by invasive means. The authors describe the case of a 52-year-old man, who had undergone heart surgery in the past, with a long history of heart failure refractory to medical therapy and chronic recurrent pericardial effusion, in whom noninvasive imaging techniques were decisive in arriving at the correct diagnos

Localized constrictive pericarditis after Gore-Tex pericardial substitution.

A 65-year-old patient presented with recurrent cardiac decompensation 12 years after aortic prosthesis replacement and expanded polytetrafluoroethylene (ePTFE) membrane pericardial substitution. Diagnosis of pericardial constriction was difficult. Only one cardiac imaging method, radionuclide ventriculography, was helpful. Upon re-operation, an epicardial fibrous strap which restricted right ventricle (RV) diastolic expansion was found between the anterior free wall and diaphragmatic portion of the RV. Clinical status dramatically improved after surgical removal of this bridle, as did ventricu
