Pericardiectomy in children <15 years of age.

Between January, 2002 and December, 2011, 27 patients (19 boys) underwent pericardiectomy. The mean age was 9.3 ± 4.96 years (range 0.4 to 15 years) and the mean duration of symptoms was 16.9 ± 22.15 months. In all, 25 patients had dyspnoea; eight were in New York Heart Association (NYHA) class IV; six had bacterial pericarditis; and 18 were on anti-tuberculosis treatment, although only nine had records suggesting tuberculosis. There were nine patients who underwent pre-operative pigtail catheter drainage of pericardial fluid. Surgical procedures were complete pericardiectomy (n = 20), partial

Clinical review: intrapericardial fibrinolysis in management of purulent pericarditis.

Purulent pericarditis (PP) is a potentially life-threatening disease. Reported mortality rates are between 20 and 30%. Constrictive pericarditis occurs over the course of PP in at least 3.5% of cases. The frequency of persistent PP (chronic or recurrent purulent pericardial effusion occurring despite drainage and adequate antibiotherapy) is unknown because this entity was not previously classified as a complication of PP. No consensus exists on the optimal management of PP. Nevertheless, the cornerstone of PP management is complete eradication of the focus of infection. In retrospective studie

The effects of HIV/AIDS on the clinical profile and outcomes post pericardiectomy of patients with constrictive pericarditis: a retrospective review.

OBJECTIVE: The clinical profile and surgical outcomes of patients with constrictive pericarditis were compared in HIV-positive and -negative individuals. METHODS: This study was a retrospective analysis of patients diagnosed with constrictive pericarditis at Inkosi Albert Luthuli Central Hospital, Durban, over a 10-year period (2004-2014). RESULTS: Of 83 patients with constrictive pericarditis, 32 (38.1%) were HIV positive. Except for pericardial calcification, which was more common in HIV-negative subjects (n = 15, 29.4% vs n = 2, 6.3%; p = 0.011), the clinical profile was similar in the tw

Pericardiectomy: a changing scene.

The indications for pericardiectomy as well as the causal factors have changed in recent years. Sixty-eight patients operated on at the UCLA Medical Center between 1955 and 1982 have been described. There were 37 male patients and 31 female patients. The indication for operation was acute pericarditis in 37 patients, of whom 31 had recurrent effusion, and chronic constrictive pericarditis in 31 patients, of whom 8 were calcific. The most common cause of pericardial disease was tumor (20 patients), followed by idiopathic (13 patients), uremic (7 patients), viral (7 patients), tuberculous (6 pat

Reoperation for calcific constrictive pericarditis 43 years after pericardiectomy.

The case of calcific pericarditis in a 63-year-old male with a 1-year history of progressive right heart failure 43 years after pericardiectomy in 1960 is reported. The recurrent constriction was caused by the calcification of the residual posteroinferior pericardium and the impressing sternum of a pectus excavatum encasing the heart like a skull. The patient underwent total pericardiectomy of his residual pericardium through median sternotomy using cardiopulmonary bypass resulting in complete relief of his symptoms.

"Acute" granulomatous pericarditis. clinical and hemodynamic correlate.

Ten cases of acute constrictive pericarditis from southern Iran are presented. They differed from chronic constrictive pericarditis by the absence of ascutes and edema of the lower extremities, the presence of cardiomegaly on posteroanterior chest x-ray film, the lack of calcification of the pericardium on fluoroscopic examination, and the presence of sinus rhythm in all cases. If tuberculosis is the etiologic agent, early pericardiectomy should be considered. This is especially important in areas where close follow-up of patients is difficult or impossible, since all patients in this series u

Results of subxiphoid pericardiostomy in pericardial effusion.

37 patients with pericardial effusions were managed by early subxiphoid pericardiostomy under local anaesthesia in most cases (31/37). The aetiology was pyogenic infection in 21, tuberculosis in 7, trauma in 3, cardiomyopathy in one. It was unclear in five patients. The effusion recurred in two patients (one with pyogenic, the other with traumatic pericarditis). An infant with pyogenic pericarditis developed pericardial constriction. The mortality, mainly due to associated infections or diseases, was related to aetiology: 0% for tuberculosis and trauma, 19% (4/21) for pyogenic infections, 80%

Disseminated Tuberculosis With Pericardial Effusion and Early Tamponade: A Case Report.

Tuberculous pericarditis is a rare but life-threatening complication of tuberculosis (TB), often presenting with nonspecific symptoms and leading to delayed diagnosis. This case report describes a 26-year-old male with disseminated TB involving the pericardium, pleura, and mediastinal lymph nodes, complicated by early cardiac tamponade. Despite the presence of tamponade physiology, the patient was successfully managed with anti-TB therapy and corticosteroids without invasive intervention. Diagnostic challenges, including the role of imaging, fluid analysis, and molecular testing, are discussed

[Extrapulmonary tuberculosis].

Although the respiratory tract is the major site for tuberculous lesions, all organs can be affected. The authors review the clinical files of 25 patients admitted to our Department with lung and/or pleural tuberculous disease, confirmed by bacteriology or histopathology, who simultaneously suffered from tuberculosis in other organs. We included 20 men and 5 women, aged from 10 to 74 years (mean: 37.12 years); 6 patients had pleural tuberculosis, 19 had pulmonary tuberculosis, 4 of them having miliary tuberculosis. Lymph node tuberculosis was diagnosed in 14 patients, tuberculosis of the bones

Recommendations Concerning the First-Line Treatment of Children with Tuberculosis.

This document describes the recommendations of a group of scientific societies concerning the first-line therapeutic approach to paediatric tuberculosis (TB). The treatment of pulmonary TB should be based on the existence of parenchymal involvement and the risk of antibiotic resistance. The treatment of extra-pulmonary TB is based on the regimens used for severe pulmonary TB. The administration of corticosteroids is recommended only in cases of miliary TB, tuberculous meningitis and tuberculous pericarditis. Vitamin B6 may be indicated in the case of isoniazid-treated TB in breastfeeding infan

Pathological lesions in swine at slaughter. I. Baconers.

An extended disease recording programme in pigs has been carried out by the meat inspection service at Sentralslakteriet, Forus, Stavanger. A data system including 57 disease codes has been applied. In the period 1975–1977 an average of 85,000 baconers were slaughtered yearly. About 39 % of these were given disease remarks; 42–47 % of the lesions were directly related to the parasites Sarcoptes scabiei (rind lesions) and Ascaris suum (white spots in the livers). The thoracic cavity was the most commonly affected part of the body with 30–35 % of all recorded lesions. Sixteen disease codes occur

Intracardiac left atrial tuberculoma in an eleven-month-old infant: case report.

BACKGROUND: Cardiac tuberculosis is rare and usually manifests as tuberculous pericarditis. Involvement of other part of the heart is unusual and descriptions in the pediatric literature are confined to few case reports regarding mainly myocardial tuberculosis. CASE PRESENTATION: We describe a case of pulmonary miliary tuberculosis associated with intracardiac left atrial tuberculoma in an immunocompetent eleven-month-old infant successfully treated with surgery and antituberculous therapy. CONCLUSION: Although unusual, involvement of endocardium in disseminated tuberculosis should be kept i

Mycobacterium kansasii pericarditis in a kidney transplant recipient: a case report and comprehensive review of the literature.

Mycobacterium kansasii is the second most common non-tuberculous mycobacteria in kidney transplant recipients (KTRs) and has been reported to cause disseminated infection in KTRs. We report the first case to our knowledge of M. kansasii pericarditis after kidney transplantation in a 54-year-old man. The patient was admitted with a 2-month history of intermittent fever and myalgia, treated with oral prednisolone and mycophenolate mofetil prior to admission. Chest computed tomography showed enlarged mediastinal lymph node and small amount of pericardial effusion. Mediastinoscopic biopsy of media

[Spontaneous pneumopericardium. Apropos of a case report].

A patient operated for carcinoma of the bladder complicated by infection by anaerobic organisms developed pneumopericardium. Spontaneous pneumopericardium may or may not follow effraction of the pericardium. The following causes have been described: fistula with a tuberculous cavernoma, parenchymatous or pleural infection, carcinoma of the bronchus; oesophageal or gastro-pericardial fistulae arising from carcinoma or ulceration of the stomach or oesophagus; rupture of a mediastinal, hepatic or subphrenic abscess and, exceptionally, pericarditis complicated by fistulisation to the tracheo-bronc

[Pericardial tuberculosis is still an important differential diagnosis in Denmark].

We report a case where tuberculous pericarditis was diagnosed in a 46-year-old man with Turkish background. The patient had typical symptoms of tuberculosis in the form of chest pain, cough and breathlessness and systemic symptoms such as fever, weight loss and fatigue. An echocardiography showed pericardial effusion, and diagnosis was confirmed by culture of Mycobacterium tuberculosis from gastric aspirate and lymph node biopsy. Since there is an efficient cure for this potentially deathly infection it is of great importance to find the specific diagnose, and it is important that all physicia

Forgotten cardiovascular diseases in Africa.

Much of the global burden of cardiovascular disease is now carried by low and middle income countries. Unfortunately, many of these regions are still grappling with poverty and infection-related cardiovascular diseases, such as endomyocardial fibrosis, tuberculous pericarditis and rheumatic heart disease. In addition, Africa has its unique diseases that occur more commonly in Africans as peripartum cardiomyopathy or, almost uniquely in Africa, as subvalvular aneurysm. We present our perspective on forgotten cardiovascular disease in Africa in the context of the epidemic of cardiovascular disea

In spite of the lung being the target organ par excellence of tuberculosis, any other organ and system can be affected. In this article we review the forms of extrapulmonary tuberculosis, with the exception of the pleural form that requires the use of good diagnostic skills. From the dreaded tuberculous meningitis, by way of the insidious affectation of the spondylodiscitis, the so-called ganglionary affectation , the genitourinary affectation, pericarditis, and ending with less frequent forms such as ocular and cutaneous tuberculosis. In each section we will indicate what is most characterist
