A Case of Acute Pancreatitis With Fat Saponification Mimicking Carcinomatosis in an Adolescent: Is It Carcinomatosis or Fat Necrosis?

Acute pancreatitis, the most common pancreatic disease in children (1-13 cases per 100,000 children per year), uncommonly presents with extrapancreatic fat necrosis and saponification. This uncommon manifestation can radiologically and intraoperatively resemble peritoneal carcinomatosis or neoplasms, often necessitating histopathological confirmation. Integrating clinical and radiological findings is crucial for accurate diagnosis and to avoid unnecessary interventions. We present a case of an adolescent with acute pancreatitis and extrapancreatic fat saponification mimicking carcinomatosis (w

Intra-abdominal carcinomatosis after prophylactic oophorectomy in ovarian-cancer-prone families.

Prophylactic oophorectomy was performed in 28 female members of 16 families at high risk of ovarian carcinoma. 3 of these women subsequently developed disseminated intra-abdominal malignancy; there was uncertainty about the primary site despite extensive investigation. These tumours were indistinguishable histopathologically from ovarian carcinoma. It would seem that in cancer-prone families the susceptible tissue is not limited to the ovary, but includes other derivatives of the coelomic epithelium, from which primary peritoneal neoplasms may arise.

Significance of psammoma bodies in serous cavity fluid: a cytopathologic analysis.

BACKGROUND: Psammoma bodies (PBs) are encountered only rarely in body cavity fluids (BCF). Although to the authors' knowledge their presence in certain neoplasms (e.g., those of the thyroid, ovary, lung, brain, etc.) is established, their significance in BCF has not been well defined. METHODS: Diagnoses concerning 3335 BCF samples were reviewed from the cytopathology files for the presence of PBs over an 8-year period. Cytologic preparations included cytospin preparations and Millipore filters stained with the Papanicolaou stain. Clinicopathologic correlation was performed on any subsequent s

Clinical characteristics and outcomes of appendiceal neoplasms in inflammatory bowel disease: A tertiary care center experience.

BACKGROUND AND AIMS: A direct causal association between inflammatory bowel disease (IBD) and appendiceal neoplasm (AN) is unclear. METHODS: Patients with IBD and AN were identified from 1992 to 2023 using bioinformatics and natural language processing tools. RESULTS: Thirty-one patients were identified. The most common type of AN was appendiceal mucinous neoplasm (83.9 %). Three patients with ulcerative colitis (9.7 %) had recurrence after surgical resection due to peritoneal seeding. CONCLUSIONS: Incidence and recurrence of AN in patients with IBD is low. Further studies to compare AN in

Dedifferentiated Liposarcoma Presenting as Primary Diffuse Peritoneal Sarcomatosis: A Case Report and Literature Review.

BACKGROUND: Dedifferentiated liposarcoma (DDLPS) typically presents as a solitary retroperitoneal mass; diffuse granular and nodular peritoneal sarcomatosis as the inaugural manifestation is exceptional. We report such a case to highlight a critical diagnostic consideration: distinguishing primary sarcomatous peritoneal dissemination from the more common epithelial peritoneal carcinomatosis. CASE PRESENTATION: A 72-year-old Chinese man with no prior abdominal surgery presented with incidentally discovered multifocal intra-abdominal masses. CT demonstrated multiple peritoneal and retroperitone

[Controlled stomach fistula for acute operated ulcer. Case report].

The authors analyze the case of a 65 old woman which was hospitalized for sigmoidian stenosant and haemorrhagical neoplasm, confined to the colic wall, without peritoneal or hepatic metastases, and without peritoneal or parietal invasion. Surgical management included sigmoidectomy and termino-terminal anastomosis for reconstructing intestinal transit followed by peritoneal drainage. In early postoperative stage the aspect of generalized peritonitis occurs and there is suspicion of anastomotic fistulae. On surgery, acute and perforated gastric ulcer is found, located in close vicinity to the ca

Cytoreductive Surgery and Hyperthermic Intraperitoneal Chemotherapy to Treat Pseudomyxoma Peritonei of Ovarian Origin: A Retrospective French RENAPE Group Study.

BACKGROUND: Ovarian pseudomyxoma peritonei (OPMP) are rare, without well-defined therapeutic guidelines. We aimed to evaluate cytoreductive surgery (CRS) and hyperthermic intraperitoneal chemotherapy (HIPEC) to treat OPMP. METHODS: Patients from the French National Network for Rare Peritoneal Tumors (RENAPE) database with proven OPMP treated by CRS/HIPEC and with histologically normal appendix and digestive endoscopy were retrospectively included. Clinical and follow-up data were collected. Histopathological and immunohistochemical features were reviewed. RESULTS: Fifteen patients with a med

Abdominal hernia and the unexpected final diagnosis.

Mucinous neoplasm of the appendix are rare entities, among these, mucinous cystadenomas contribute to 31-34%. Cystadenomas often produce extensive dilatation of the appendix with epithelial atypia. Spontaneous perforation often occurs in 20%, leading to mucin distribution throughout the peritoneal cavity. Half of the patients are completely asymptomatic and are detected as an incidental diagnosis, others symptoms include a palpable mass, hernias, weight loss, peritonism or even intestinal obstruction. We present a case of a 71-year-old female, she presented with a palpable mass in the upper ab

['Malignant peritoneal mesothelioma': a condition difficult to diagnose].

BACKGROUND: Malignant mesothelioma is an aggressive neoplasm, which arises from serous membranes, such as the pleura and the peritoneum. Malignant peritoneal mesothelioma is relatively rare, but the incidence is increasing worldwide because of intensive asbestos use during the 20th century. CASE DESCRIPTION: A 60-year-old man complained of malaise, night sweating and weight loss and was seen by a specialist in internal medicine. Additional investigation included a CT scan, 2 PET-CT scans, 2 diagnostic laparoscopies and histological examination of peritoneum biopsies. After 7 months the diagno

68Ga-FAPI-04 PET/CT Detection of Peritoneal Metastases From a Rare Uterine Tumor Resembling Ovarian Sex-cord Tumor.

Uterine tumor resembling ovarian sex-cord tumor (UTROSCT) is an extremely rare uterine stromal neoplasm of uncertain origin that accounts for less than 0.5% of uterine malignancies. Here, we report, using MRI, 18F-FDG, and 68Ga-FAPI-04 PET/CT, the imaging findings for peritoneal metastatic lesions from postoperative UTROSCT in a 58-year-old woman. The peritoneal lesions showed only mild uptake on 18F-FDG PET/CT. Subsequent 68Ga-FAPI-04 PET/CT revealed intense tracer uptake in the lesions, indicating the potential value of this modality for detecting recurrence and metastasis in UTROSCT.

Society of Onco-Anaesthesia and Perioperative Care consensus guidelines for perioperative management of patients for cytoreductive surgery and hyperthermic intraperitoneal chemotherapy (CRS-HIPEC).

Cytoreductive surgery and hyperthermic intraperitoneal chemotherapy (CRS-HIPEC) for primary peritoneal malignancies or peritoneal spread of malignant neoplasm is being done at many centres worldwide. Perioperative management is challenging with varied haemodynamic and temperature instabilities, and the literature is scarce in many aspects of its perioperative management. There is a need to have coalition of the existing evidence and experts' consensus opinion for better perioperative management. The purpose of this consensus practice guideline is to provide consensus for best practice pattern

Pseudomyxoma peritonei.

Pseudomyxoma peritonei is a relatively rare and poorly understood condition in which mucus accumulates within the peritoneal cavity. The presence of cells in the mucin, either inflammatory or neoplastic, distinguishes it from simple acellular mucus ascites caused by mucinous spillage. There is widespread seeding of the peritoneal and omental surfaces with a heavy cancerous glaze. This is principally a complication of borderline or malignant neoplasm of the ovary and/or appendix. This paper describes two cases of previously healthy women who both presented with an acute abdomen, and were diagno

Bilateral ovarian metastases from gastric carcinoma on FDG PET/CT.

We report PET/CT appearance of bilateral ovarian metastases in a 63-year-old female patient with newly diagnosed poorly differentiated adenocarcinoma of the stomach. She was also found to have peritoneal metastasis on an outside CT study. Staging PET/CT demonstrates not only the hypermetabolic gastric primary tumor and peritoneal disease but also bilateral enlarged, moderately active ovaries. The patient subsequently underwent biopsies of bilateral ovaries which revealed metastatic disease from primary gastric carcinoma. It is important to consider this form of neoplasm in the differential dia

Efficacy of Long-Term Adjuvant Therapy With Imatinib Mesylate After Extensive Surgical Treatment for Ruptured Gastrointestinal Stromal Tumors of the Small Intestine With Peritoneal Metastases: A Case Report.

Gastrointestinal stromal tumors (GISTs) are the most common mesenchymal neoplasms of the gastrointestinal tract. Although most patients with advanced GISTs benefit from imatinib mesylate (IM) as standard targeted therapy, the optimal duration of adjuvant IM for GIST patients with high risk of recurrence who underwent surgical resection remains unknown. In this article, we present a case of a ruptured GIST of the small intestine accompanied by peritoneal metastases, which was effectively treated by surgical procedure followed by long-term adjuvant therapy with IM. Surgical resection was perform

Communicating imaging findings in peritoneal mesothelioma: the impact of 'PAUSE' on surgical decision-making.

The peritoneal cavity is the second commonest site of mesothelioma after the pleural cavity. There are five histological types of peritoneal mesothelioma with variable symptomatology, clinical presentation and prognosis. Cystic mesothelioma is a borderline malignant neoplasm with a favourable prognosis, well-differentiated papillary mesothelioma is generally a low-grade malignancy, and all other varieties such as epithelioid, sarcomatoid and biphasic mesothelioma are highly malignant types of peritoneal mesothelioma with poor prognosis. Malignant peritoneal mesothelioma was considered inevitab

A 6.3 kg uterine tumor resembling an ovarian sex cord-stromal tumor (UTROSCT): A case report.

Uterine tumor resembling ovarian sex cord-stromal tumor (UTROSCT) is a rare uterine neoplasm characterized by sex cord-like differentiation. Although typically indolent and confined to the uterus, UTROSCT can occasionally exhibit atypical clinical and pathological behavior, complicating preoperative diagnosis. A 52-year-old postmenopausal woman (G3P3A0) with no significant medical history presented with a painless umbilical swelling persisting for eight months. Physical and radiological assessment revealed a paraumbilical hernia and she was scheduled for elective laparoscopic repair. Intraoper

Peritoneal angiosarcoma in a patient with a history of cervical cancer treated with radiotherapy: A case report.

Angiosarcoma is a highly malignant endothelial neoplasm that accounts for 1-2% of all soft tissue sarcomas in humans, and its marked biological aggressiveness constitutes a major clinical challenge. Early identification is particularly difficult in patients with a history of malignancies treated with radiotherapy, due to its histopathological heterogeneity and nonspecific clinical presentation, which often leads to confusion with other mesenchymal or epithelial neoplasms. We report the case of a 72-year-old woman with a history of cervical carcinoma treated with radiotherapy who presented with
