Avoiding iris trauma from phacoemulsification in eyes with iridoschisis.

Iridoschisis is a rare condition frequently associated with age-related cataract. Phacoemulsification can be difficult to perform in these eyes, as the detached iris fibrils that are typically present free float in the anterior chamber and are attracted to the phaco tip and the irrigation/aspiration handpiece port. We describe a technique for performing phacoemulsification in eyes with iridoschisis. In this technique, iris fibrils are held in place by 2 ophthalmic viscosurgical devices (OVDs) that are injected into the inferior portion of the anterior chamber. An elliptical capsulorhexis is ma

Changes of blood aqueous barrier after phacoemulsification in rabbits.

PURPOSE: To evaluate the function and the morphological changes of the blood aqueous barrier (BAB) after phacoemulsification and to provide the physiological mechanism of BAB dysfunction. METHODS: Phacoemulsification was performed on 12 rabbit eyes. Aqueous humor was abstracted and the concentrations of aqueous humor protein were examined preoperatively and on postoperative days 1, 7, 14, 21, 30 with Lowry. After the iris and the ciliary body were removed from the rabbit eyes, we selected lanthanum nitrate as a tracer, then ultrastructural changes of BAB were investigated with transmission el

Intravitreal sodium hyaluronate and secondary glaucoma after complicated phacoemulsification.

We report 4 cases that presented with intractable glaucoma after complicated phacoemulsification during which sodium hyaluronate 1.0% (Healon) was used to tamponade a central posterior capsule rupture. The postoperative intraocular pressure (IOP) was uncontrolled on maximal medical therapy. In 2 patients, the IOP took 8 to 10 weeks to resolve and simultaneous ultrasound B-scans showed the presence of clear globules in the anterior and midvitreous that became smaller with conservative therapy. An early pars plana vitrectomy in 2 cases immediately controlled the IOP. The presence of globules pre

Endothelial cell loss induced by phacoemulsification occurs through apoptosis.

BACKGROUND: Cornea endothelial cells are nondividing cells containing pumping function which is crucial for cornea clarity and integrity. Endothelial cell loss occurs after cataract surgical procedures such as phacoemulsification. The authors hypothesize that endothelium damage occurs through apoptosis. METHODS: Ultrasound was achieved by placing a phacoemulsification probe in the anterior chamber and delivering 0% or 50% of maximum power for 2.5 min. The corneal tissue was harvested immediately, and at 1 and 7 days after the operation. Corneal tissue was stained by hemotoxylin and eosin (H&E

Treatment of bleb hypotony after phacoemulsification with autologous blood.

Hypotony from an inadvertent filtration bleb developed in the eye of a 66-year-old woman 1 year after phacoemulsification cataract surgery. The hypotony was treated with an autologous blood injection over the previous phacoemulsification incision. This technique successfully treated the hypotony without additional ocular surgery. The authors report an effective alternative treatment for hypotony induced by an inadvertent filtration bleb.

Low-cost optical system for laser phacoemulsification of cataracts.

SIGNIFICANCE: Laser-based phacoemulsification is the preferred treatment for surgical removal of cataracts in the eye. Conventional methods of laser phacoemulsification use expensive, high-powered lasers that use femtosecond pulses to soften and emulsify cataracts, resulting in low- to middle-income countries relying on methods that lead to longer recovery times and higher rates of infection and astigmatism. AIM: We introduce and demonstrate the potential of laser phacoemulsification using a millisecond pulsed laser that can be realized using a low-cost diode laser. APPROACH: We show proof o
