Von Frey testing revisited: Provision of an online algorithm for improved accuracy of 50% thresholds.

BACKGROUND: In the pain field, it is essential to quantify nociceptive responses. The response to the application of von Frey filaments to the skin measures tactile sensitivity and is a surrogate marker of allodynia in states of peripheral and/or central sensitization. The method is widely used across species within the pain field. However, uncertainties appear to exist regarding the appropriate method for analysing obtained data. Therefore, there is a need for refinement of the calculations for transformation of raw data to quantifiable data. METHODS: Here, we briefly review the fundamentals

Attentional bias towards pain-related information in chronic pain; a meta-analysis of visual-probe investigations.

Research investigating the presence of attentional bias in chronic pain has produced mixed results. The purpose of this review is to summarise former research using the visual-probe task to explore attentional bias in pain populations, and meta-analyse the results of controlled investigations comparing individuals with chronic pain to healthy controls. Ten eligible studies were included in this analysis (chronic pain n=515, control n=314). Overall, individuals with chronic pain were found to show significantly greater bias towards pain-related information compared to controls, with an effect s

Psychophysical magic: rendering the visible 'invisible'.

What are the neural correlates of conscious visual awareness? Tackling this question requires contrasting neural correlates of stimulus processing culminating in visual awareness with neural correlates of stimulus processing unaccompanied by awareness. To produce these two neural states, one must be able to erase an otherwise visible stimulus from awareness. This article describes and assesses visual phenomena involving dissociation of physical stimulation and conscious awareness: degraded stimulation, visual masking, visual crowding, bistable figures, binocular rivalry, motion-induced blindne

Immediate-pressure urticaria--a distinct disorder.

We report a patient who has an immediate wealing response to pressure in the absence of symptomatic dermographism, which we designate 'immediate-pressure urticaria'. Our patient also suffered from cholinergic urticaria and chronic idiopathic urticaria in the past, demonstrating the clustering of urticarias which frequently occurs.

A 3D-Printed Biochemical-Mechano-Thermally Responsive Multicellular Tumor Organoid Platform Decodes Biochemical-Physical Regulation of Cancer-Immune Crosstalk and Chemoresistance.

Oral tumors are not shaped by biochemical signals alone; they also evolve within mechanically active and thermally fluctuating microenvironments that critically regulate malignant progression, immune remodeling, and therapeutic resistance. Here, we develop a dynamic bioorthogonal organoid platform that couples a 3D-printed photothermal elastic scaffold with an interpenetrating hydrogel network derived from decellularized oral mucosa extracellular matrix, enabling the reconstruction of in vivo-like structural support, matrix-derived biochemical cues, spatial cellular compartmentalization, and d

Does threshold nighttime electrical stimulation benefit children with spina bifida? A pilot study.

BACKGROUND: Electrical stimulation is an established treatment for muscle weakness. However, traditional tectonic stimulation is poorly tolerated in children as a result of discomfort. Threshold electrical stimulation performed at night reportedly increases muscle strength in a variety of neuromuscular conditions and has been well tolerated in children. QUESTIONS/PURPOSES: We asked whether threshold electrical stimulation treatment at night would increase strength and function in children with myelomeningocele. METHODS: In a pilot study we prospectively followed 15 treated children who serve

Systematic review of tactile-based interventions combined with multisensory stimulation protocols in the rehabilitation of patients with disorders of consciousness.

Social touch interventions have the potential to modulate psychological and physiological stress in intensive care settings. In individuals with disorders of consciousness (DOC), tactile stimulation combined with other sensory modalities, (e.g., visual, auditory, olfactory, gustatory, and proprioceptive), has been proposed to enhance arousal, behavioural responsiveness, and recovery, while mitigating sensory deprivation. However, tactile stimulation encompasses heterogeneous forms of touch and the extent to which clinical benefits can be attributed to specific tactile qualities remains unclear

The significance of the Babinski sign in the newborn-a reappraisal.

A study of the clinical significance and the stimulus methods used to elicit the Babinski sign in normal newborns has been carried out. Although the most frequent response to cutaneous nociceptive stimulation of the lateral sole in neonates was extension of the great toe, a small but significant number of flexor responses were obtained. Consequently, the Babinski sign has limited clinical usefulness as a reflex indicating the presence or absence of neurological disease in the newborn. In reviewing previous studies in the literature, it was apparent that the widely differing percentages of exte

Estimation of habituation and signal-to-noise ratio of cortical evoked potentials to oesophageal electrical and mechanical stimulation.

Electrical and mechanical stimulation of the oesophagus has been recently proposed to examine the physiological effects of autonomic stimulation in humans. Cortical evoked potentials (EPs) to oesophageal stimulation provide an assessment of afferent fibres and central processing. However, habituation takes place during averaging of cortical EPs and reduces the signal-to-noise ratio (SNR) as the number of stimuli increases. The SNR of cortical EPs to oesophageal stimulation is computed for 15 normal subjects. Habituation is characterised by the Euclidean distance between the EEG response to sin

Tapping the head activates the vestibular system: a new use for the clinical reflex hammer.

We investigated the use of skull taps with a modified clinical reflex hammer as a method of vestibular activation. Using recently described EMG techniques to measure vestibulocollic reflexes in response to clicks, we were able to show analogous short-latency potentials to taps. The earliest responses were invariably absent on the side of a previous vestibular nerve section but were preserved in profound sensorineural or conductive hearing loss. We propose that the taps activated the vestibular apparatus directly by a bone-conducted vibration wave.

The temporary inactivation of the red nucleus affects performance of both conditioned and unconditioned nictitating membrane responses in the rabbit.

These experiments are part of a series of studies examining the role of the red nucleus in the performance of the conditioned and unconditioned nictitating membrane reflexes in the rabbit. Specifically, the experiments test the hypothesis that the temporary inactivation of the red nucleus selectively affects the performance of the conditioned reflex. The experiments were designed to assess the effects of lidocaine and control saline microinjections on conditioned as well as unconditioned responses in both paired and unpaired trials. Rabbits were chronically implanted with cannulae through whic

Idiopathic trigeminal neuralgia: sensory features and pain mechanisms.

We present a case report of a patient with the typical sensory features of idiopathic trigeminal neuralgia (ITN). The pain was elicited by innocuous stimuli, summated with repeated stimulation, radiated outside the stimulus zone, referred to a distant site, persisted beyond the period of stimulation, and exhibited a variable refractory period. Unusual sensory features included multiple trigger zones that changed over time and involved all 3 trigeminal divisions. Our sensory evaluation indicated that the pain was evoked by repetitive activation of rapidly adapting, A beta, low-threshold mechano

Alpha stimulation of the human parietal cortex attunes tactile perception to external space.

An intriguing question in neuroscience concerns how somatosensory events on the skin are represented in the human brain. Since Head and Holmes' [1] neuropsychological dissociation between localizing touch on the skin and localizing body parts in external space, touch is considered to operate in a variety of spatial reference frames [2]. At least two representations of space are in competition during orienting to touch: a somatotopic one, reflecting the organization of the somatosensory cortex (S1) [3], and a more abstract, external reference frame that factors postural changes in relation to b

Auditory brainstem implants: current state and future directions with special reference to the subtonsillar approach for implantation.

In this article, the authors describe the current state of the auditory brainstem implant (ABI), comparing it to that of the cochlear implant (CI). The CI restores hearing by stimulating the cochlear nerve in the cochlea in patients whose deafness has been caused by inner ear disease; the ABI restores hearing by stimulating the cochlear nucleus of the brainstem in patients who are deaf because of bilateral cochlear nerve dysfunction. Up to now, about 500 patients worldwide have undergone ABI and had their hearing restored, most of whom suffer from neurofibromatosis type 2. Hearing performance,

Tactile, acoustic and vestibular systems sum to elicit the startle reflex.

The startle reflex is elicited by intense tactile, acoustic or vestibular stimuli. Fast mechanoreceptors in each modality can respond to skin or head displacement. In each modality, stimulation of cranial nerves or primary sensory nuclei evokes startle-like responses. The most sensitive sites in rats are found in the ventral spinal trigeminal pathway, corresponding to inputs from the dorsal face. Cross-modal summation is stronger than intramodal temporal summation, suggesting that the convergence of acoustic, vestibular and tactile information is important for eliciting startle. This summation

Fetal habituation to vibroacoustic stimulation in uncomplicated postterm pregnancies.

OBJECTIVES: Testing of fetal habituation to repeated vibroacoustic stimulation might give additional information concerning the fetal condition and central nervous system (CNS) functioning in postterm pregnancies. This study is designed to investigate whether healthy postterm fetuses are able to habituate and if so, if the habituation pattern of postterm fetuses differs from that of term fetuses. STUDY DESIGN: Twenty women with an uncomplicated pregnancy beyond 41 weeks gestational age (GA) participated, and 37 women with a GA between 37 and 40 weeks served as controls.
