Novel use of dynamic navigation for guiding a piezoelectric device during window osteotomy for maxillary sinus floor elevation in complex clinical scenarios.

In patients with severe atrophy of the posterior maxilla requiring lateral maxillary sinus floor elevation (MSFE), the window location and size are commonly designed according to the future implants and anatomical conditions. A window osteotomy becomes challenging when there is an extended edentulous space in the maxilla with no reference from the natural dentition, or when the surgical site involves anatomical variations, for example in the course of a large vessel or a sinus septum. Through preoperative planning and real-time visualization, the application of dynamic navigation allows an acc

Comparison of two kinds of bovine bone in maxillary sinus augmentation: a histomorphometric study.

PURPOSE: The purpose of this study was to compare the histomorphometric from sinus augmentation with calcium-phosphate nanocrystal-coated bovine bone (Biocera) and anorganic bovine bone matrix (Bio-Oss). MATERIALS AND METHODS: Bilateral maxillary sinus augmentations were performed on 5 patients with delayed placement of implants. The lateral bony window was created using a piezoelectric saw, and the sinus membrane was elevated to make a new compartment. Bio-Oss was grafted in one sinus as the control group and Biocera was grafted in the opposite sinus as the test group. The bony window was re

Stabilization of the Bony Pyramid in Rhinoplasty Using Dorsal Aesthetic Lines Bony Sutures.

INTRODUCTION: Modern rhinoplasty relies on osteotomies that risk destabilizing the nasal pyramid. While suture fixation is standard for cartilage, bony stabilization remains technically challenging. We present a novel piezosurgery-assisted 8-shaped "bridge suture" technique along dorsal aesthetic lines (DAL) to address this gap. MATERIALS AND METHODS: From 2021 to 2023, 28 patients underwent structural rhinoplasty with DAL bony sutures. The technique involved: (1) DAL osteotomies via piezotome, (2) two pairs of bilateral 2-3 mm perforations, and (3) 4-0 PDS figure-eight sutures securing later

Endoscopic visualization of the inferior alveolar nerve associated with somatosensory changes after impacted mandibular third molar extraction.

The aim of this study is to assess the relationship between somatosensory functional changes and inferior alveolar nerve (IAN) exposure after impacted mandibular third molars (M3M) removal. We recruited 35 patients who underwent impacted M3M extraction near the IAN. The M3Ms were extracted by combined endoscopy, piezosurgery, and contra-angle high-speed turbine handpiece. All IAN canal perforations and exposed regions were recorded and measured by endoscopy after extraction and on cone-beam computed tomography (CBCT) images before extraction. The patients were followed up 1, 7, and 35 days aft

Occlusion-activated autonomous piezoelectric implants for adaptive prevention of peri-implantitis.

More than one-third of the global population suffers from dental defects, with osseointegrated implants being the gold-standard intervention; yet, their long-term functional performance is hindered by peri-implantitis. Here, we present an autonomous piezoelectric implant that delivers adaptive antibacterial and anti-inflammatory functionalities through physiological occlusal activation without any external intervention. The implant demonstrates good functional durability, sustaining consistent bioelectric outputs through over 1,000,000 loading-unloading cycles and exhibiting stable electrical

Minimally invasive surgically assisted miniscrew anchored rapid palatal expansion (sa-MARPE) technique: avoiding conventional horizontal maxillary osteotomies - a technical note.

In skeletally mature patients, transverse maxillary deficiency poses a significant challenge due to the rigidity of craniofacial sutures. This technical note describes a minimally invasive, surgically assisted MARPE (sa-MARPE) protocol designed to overcome these limitations while avoiding complete Le Fort I osteotomies. The technique involves three small mucosal incisions and limited piezosurgical osteotomies targeting the midpalatal suture, canine pillars, and pterygomaxillary junctions. By preserving mucosal integrity and vascular supply, this approach reduces postoperative morbidity and enh

Operative Technique for Extirpation of Large Melanotic Neuroectodermal Tumor of Infancy Involving Alveolus and Anterior Maxilla in the Region of the Premaxilla.

BACKGROUND: Melanotic neuroectodermal tumors of infancy are rare, benign neoplasms predominantly affecting the craniofacial region, and they are typically managed through resection with minimal need for reconstruction. However, in exceptional cases, larger or more complex tumors may necessitate open craniofacial approaches, with limited literature detailing the surgical strategies for these scenarios. The authors report a distinctive case of an aggressively expanding melanotic neuroectodermal tumor of infancy in a pediatric patient, describing their approach for the tumor's resection. METHODS

A novel 3D-printed computer-assisted piezocision guide for surgically facilitated orthodontics.

Surgical interventions on the alveolar ridges aimed at facilitating orthodontic tooth movement have been extensively reported. However, unexpected events or complications still occur in daily practice. The purpose of this report was to present a novel 3-dimensional (3D) computer-assisted piezocision guide (CAPG) designed to be translucent for increased visibility, rigid for enhanced support during guidance, and porous for profuse irrigation during procedure. Such a design can function to minimize the risk of surgical complications. In this case, we present a novel 3D-printed CAPG to facilitate

Issues with the surgical treatment of antiresorptive agent-related osteonecrosis of the jaws.

Antiresorptive agent-related osteonecrosis of the jaw is a rare but significant complication in patients using antiresorptive agents such as bisphosphonates and denosumab. Although the disease is well recognized, and many studies have been performed on the management of this condition, the treatment of severe osteonecrosis is still a challenge. Most recent studies have shown an advantage of surgical treatment over conservative treatment for stage 2/3 patients, but there is no consensus on the appropriate surgical procedures for antiresorptive agent-related osteonecrosis of the jaw. Furthermore

Combined Approach to Treat Medication-Related Osteonecrosis of the Jaws.

Introduction: The proper therapeutic plan for medication-related osteonecrosis of the Jaw (MRONJ) is still lacking long-term data up to today. They were several high-technological appliances proposed for the different intervention steps, in addition to tissue repair promoters. The reason for proposing an integrated technique is justified, beyond better compliance of the patients associated to the pain and inflammation reduction and bleeding control, there is also achieving better hard and soft tissues healing. Methods: Patients diagnosed with bisphosphonates-related osteonecrosis of the jaws (

Hydrodynamic ultrasonic maxillary sinus lift: review of a new technique and presentation of a clinical case.

OBJECTIVES: Placing implants in the posterior maxillary area has the drawback of working with scarce, poor quality bone in a significant percentage of cases. Numerous advanced surgical techniques have been developed to overcome the difficulties associated with these limitations. Subsequent to reports on the elevation of the maxillary sinus through the lateral approach, there were reports on the use of the crestal approach, which is less aggressive but requires a minimal amount of bone. Furthermore, it is more sensitive to operator technique, as the integrity of the sinus membrane is checked in

Undesirable course of an oral implant rehabilitation in a patient with a long history of bulimia nervosa: case report and review of the literature.

BACKGROUND: Bulimia nervosa is an eating disorder resulting in an intended weight loss due to decreased food intake, induced vomiting, or hyperactivity, and is observed frequently between 12 and 25 years of age. One of the complications is early tooth loss. Moreover, since bulimia nervosa patients suffer from increased atrophy of the alveolar processes, oral rehabilitation even with short dental implants may be impossible. In these cases, lateralization or transposition of the inferior alveolar nerve (IAN) followed by implant placement can be useful. CASE PRESENTATION: A 40-year-old woman wit

Functional surgery for the treatment of dentigerous cyst in the maxillary sinus.

BACKGROUND: Dentigerous cyst (DC) in the maxillary sinus is rarely seen. Several complications are observed after the Caldwell-Luc operation, which is combined with endoscopy. Still, a more perfect treatment method is yet to be discovered. OBJECTIVES: The purpose of this study was to explore and assess a new functional surgical treatment with fewer complications, which not only insured normal maxillary sinus cilia restoration and bony integrity but also did little damage to maxillary sinus natural ostium. METHODS: Dentigerous cyst in the maxillary sinuses of 20 patients had conditions diagno
