Although rare, acute poisoning with beta-blockers can be serious. Including the four personal cases of the authors, 40 cases of propranolol overdose have been published, with 8 deaths. Hypoglycaemia was not reported, but the association with alcohol can be very serious. One case of bronchospasm and one case of acute pulmonary oedema have been reported. The beta-adrenergic receptor polypeptides in synaptic membranes of rat cerebral cortex and cerebellum have been identified using the photoaffinity label [125I]iodoazidobenzylpindolol. The technique of in vitro labelling and autoradiography has been used to localize beta-adrenoceptors in sections of the splenic vascular bundle of the dog. Binding of (-)-[125I]-cyanopindolol (Cyp) to sections of splenic vascular bundle equilibrated within 150 min and slowly dissociated after addition of (-)-propranolol. Lorcainide, a new class I antiarrhythmic agent, was administered intravenously to eight patients with acute myocardial infarction for 24 hours, and thereafter given by mouth, 200 mg daily for ten days. After certain tear proteins had been found to be markers of duct cell activity in the rabbit lacrimal gland, different secretagogues were tested to increase volume and protein secretion. Carbamyl-choline and the peptides eledoisin and vasoactive intestinal peptide (VIP) stimulated volume flux as well as secretion of the marker proteins indicating duct cell activation.
