Efficacy and safety of condylectomy with minimally invasive surgery in the treatment of interdigital corns of the lesser toes compared to conservative treatment.

BACKGROUND: Minimally invasive surgery (MIS) procedures cause less trauma to the patient and might improve recovery. This study aimed to determine the efficacy and safety of condylectomy with MIS to treat interdigital corns of the lesser toes. METHODS: This prospective cohort study was conducted in seven podiatry centers. Patients with interdigital corns of the lesser toes, progressing for more than a year, with one or more recurrences in the last year following conservative treatments were eligible. The recruited patients were classified according to their treatment: conservative or surgical

Electrodiagnosis and nerve conduction studies.

The use of electrodiagnostic techniques in evaluation of complaints in the lower extremities provides an objective method of assessment. A basic understanding of principles of neurophysiology, EMG and NCV methodology, and neuropathology of peripheral nerves greatly enhances physical diagnosis and improves the state of the art in treatment of the lower extremity, especially foot and ankle injuries. Familiarity with the method of reporting electrodiagnostic studies and appreciation of the electromyographer's interpretation of the EMG/NCV studies also reflects an enhanced fund of knowledge, skill

A Systematic Review of Digital Deformities in Pediatric Patients and Their Podiatric Treatments.

BACKGROUND AND OBJECTIVES: Digital deformities in pediatrics are one of the reasons why parents go to podiatry clinics with their children. There is a great diversity of digital alterations in the feet; the origin of the vast majority is genetic. Due to this diversity, different treatments are offered depending on deformity and age, ranging from monitoring progress and changing footwear to deformities that are directly evaluated for surgery. The main objective of this study is to describe and identify the different podiatric digital deformities present in pediatric patients, as well as the mos

Prevalence of podiatric medical problems in veterans versus nonveterans.

BACKGROUND: Lower-extremity pathologic abnormalities have been common in military recruits for many years. Many of these conditions can become chronic and persist even after retiring from military service. We hypothesized that certain foot abnormalities are more prevalent in veterans versus nonveterans. The purpose of this study was to evaluate what foot and ankle disorders are associated with veteran status while controlling for other demographic factors. METHODS: The National Health Interview Survey (Podiatry Supplement) from 1990 was used for this secondary data analysis. The data were div

Ease of Application and Removal of Common Total-Contact Cast Systems Used in the Podiatric Medical Community.

BACKGROUND: Although total-contact cast (TCC) systems are considered the gold standard for off-loading plantar ulcers, less than 6% of patients with diabetic foot ulcers receive them due to negative perceptions of special technique requirements and time investment in their application and removal. We compared the ease of use and casting time of four TCC systems. METHODS: Four novice casters applied each of the four TCC systems three times using the manufacturer's written instructions for cast application and removal of each cast type. For each TCC system, casters also provided ratings of qual

The biology, medical management, and podiatric implications of menopause.

Approximately 36 million women in the United States are in the postmenopausal phase of life, creating unique challenges for the provision of compassionate, comprehensive podiatric medical treatment. Long-term estrogen deprivation arising from menopause in association with age-related factors disproportionately increases the risk of ischemic heart disease, osteoporosis, and concomitant podiatric complications. This article discusses the physiologic basis of menopause, hormone replacement therapy and its effects on osteoporosis, and other podiatric implications of menopause. Podiatric physicians

Foot screening technique in a diabetic population.

Foot complications are a well known factor which contribute to the morbidity of diabetes and increases the chance of amputation. A total of 126 consecutive diabetic patients were evaluated by diabetic foot screening. Forty-one patients showed an impaired protective sense when tested with Semmes-Weinstein monofilament 5.07 (10 g), and 92% of them showed peripheral polyneuropathy in nerve conduction study (NCS). The mean vibration score of the Rydel-Seiffer graduated tuning fork in patients with peripheral polyneuropathy in nerve conduction (NCV) study was 5.38+/-2.0, which was significantly dif

Foot Conditions in Adults: Foot Pain and Overuse Disorders.

Foot overuse injuries are caused by repetitive stress over time. They have a gradual onset and progressive nature. Signs of inflammation (eg, swelling, redness, pain) can occur in the acute phase. The etiology may be intrinsic, extrinsic, or a combination of the two. Intrinsic factors can include patient age, sex, genetic predisposition, body weight, and blood supply. Extrinsic factors can include training errors, environmental conditions, equipment malfunction, and shoes. Common causes of foot pain and overuse injury include Achilles tendinopathy, Morton neuroma, and plantar fasciitis. The hi

Provision and use of focused rigidity casts in the public health setting: a modified Delphi study.

OBJECTIVE: Focused rigidity casts (FRCs) are used by podiatrists in the promotion of wound healing and to offload bony prominences and foot ulcers. The aim of this study was to gain expert consensus on the best method of FRC application to inform the development of a clinical guideline. METHOD: A modified Delphi study was conducted comprising three rounds. The first round consisted of open-ended questions around clinical guideline content, from which three researchers independently identified themes from expert responses. Survey rounds two and three sought to gain agreement on statements iden

Navigating diagnostic uncertainty in children's chronic lower limb pain: A qualitative study of management strategies using vignette-based focus groups.

BACKGROUND: Chronic lower limb pain is common in children and adolescents and is frequently managed by podiatrists. Due to the complexities of understanding the cause of chronic pain, clinicians may experience uncertainty around the diagnosis, which in turn may impact their communication and management approaches. Limited research explores how podiatrists manage chronic lower limb pain in children, especially in the presence of diagnostic uncertainty. This study aimed to explore the management strategies including language that podiatrists report using to address the pain experience of childre

Establishing a multidisciplinary partnership integrating podiatric care into the Quebec public health-care system to improve diabetic foot outcomes: A retrospective cohort.

Diabetic foot ulcers (DFUs) are one of the main complications of diabetes affecting many Canadians that need to be effectively managed. There is limited data concerning outcomes of Canadian patients with DFUs treated with a team approach in the public health system. Podiatrists are known to be key members of a multidisciplinary team approach to DFUs management, but in Quebec, Canada, they are only available in private practice. The aim of this study is to evaluate diabetic foot outcomes after integrating podiatric care into in-hospital wound care clinic settings. A 12-month retrospective cohor

A personal revolution in the development of clubfoot correction.

This article presents the author's experience of having worked in London, England, under Denis Browne, whose method of treating equinovarus in the newborn involved manipulation of the baby feet until full correction had been achieved at the very first visit. The feet were then strapped to the sole plates of the Denis Browne splint, which ensured that the baby's normal kicking action maintained mobility at the subtalar joints and also a basic plantigrade posture. When he transferred to India, the author found a large population of children who had been born with talipes equinovarus, but who had

Talus bone: normal anatomy, anatomical variations and clinical correlations.

Talus is a pivotal bone that assists in easy and correct locomotion and transfers body weight from the shin to the foot. Despite its small size, it is implicated in many clinical disorders. Familiarity with the anatomy of the talus and its anatomical variations is essential for the diagnosis of any disorder related to these variations. Furthermore, orthopedic surgeons must be fully aware by this anatomy during podiatry procedures. In this review, we attempt to present its anatomy in a simple, updated and comprehensive manner. We have also added the anatomical variations and some clinical point
