Future Directions for Cost-effectiveness Analyses in Health and Medicine.

OBJECTIVES: In 2016, the Second Panel on Cost-effectiveness in Health and Medicine updated the seminal work of the original panel from 2 decades earlier. The Second Panel had an opportunity to reflect on the evolution of cost-effectiveness analysis (CEA) and to provide guidance for the next generation of practitioners and consumers. In this article, we present key topics for future research and policy. METHODS: During the course of its deliberations, the Second Panel discussed numerous topics for advancing methods and for improving the use of CEA in decision making. We identify and consider 7

Policy analysis of system responses to addressing and reversing the obesity trend in China: a documentary research.

BACKGROUND: The obesity epidemic and its established consequences on population health and the economy call for a vigorous fight against excess weight. The primary aim of this study is to investigate China's responses to address and reverse the obesity trend by analysing the official documents and guidelines issued or coordinated by the central government through the lens of the WHO Health System Six Building Blocks. METHODS: This study is based on the method of document research. We screened the official documents obtained through the initial search on databases. The basic information of the

A process for developing multisectoral strategies for zoonoses: the case of leptospirosis in Fiji.

BACKGROUND: Zoonotic diseases such as leptospirosis occur as a result of the often complex interactions that exist at the human-animal-environment interface. The most obvious consequence of this complexity is the need for the health sector to partner with institutions in other sectors of society such as agriculture, labour and local government. This multisectoral engagement is complicated by the different agendas and cultures of the various institutions and their ability to "see" their role and ant benefits in a collaborative response. METHODS: The research used a realist review methodology c

Twice bitten. Election commentary.

You might expect a reelected President Clinton and the Republican Congress to approach health care legislation like a postman who's been bitten by a dog-very carefully. But waiting isn't an option, not with Medicare woes unsolved. Will bipartisanship prevail?

Impact, use, and implications of artificial intelligence in public health decision making by elected officials: a scoping review.

INTRODUCTION: Artificial intelligence (AI) offers considerable promise for strengthening governmental public health decision making by supporting rapid, comprehensive analysis of complex data. Although AI applications have been widely examined in clinical and academic settings, their use in public health agencies and policymaking remains less well understood. METHODS: This scoping review assessed how AI has been applied to support decision making by public health professionals and elected officials in both routine and crisis contexts. Using PRISMA-ScR guidelines, we searched PubMed, OAISTER,

Rely and Toxic Shock Syndrome: a technological health crisis.

This essay examines factors leading to the identification of Toxic Shock Syndrome with the bacteria Staphylococcus aureus in 1978 and the specific role of Rely tampons in generating a technologically rooted health crisis. The concept biologically incompatible technology is offered to explain the relationship between constituent bacteria, women's menstrual cycles, and a reactive technology that converged to create the ideal environment for the S. aureus bacteria to live and flourish in some women. The complicated and reactive relationship of the Rely tampon to emergent disease, corporate intere

Barriers to evidence-based alcohol policies in France: Insights from interviews with parliament members.

BACKGROUND: In France, the evidence-based policies recommended by the World Health Organization SAFER program remain poorly implemented, despite relatively high levels of alcohol consumption and alcohol-related harms. To understand this paradox, we used Kingdon's Multiple Streams Approach (MSA) as framework to analyze how members of the French parliament perceive alcohol and how alcohol industry and public health actors influence their decision-making. METHODS: We conducted twenty-five interviews with French deputies and senators from different political parties and regions. We built the inte

Examining gender as an issue of policy priority: a case study of four Kenyan health policy implementation strategies.

BACKGROUND: Despite global and national commitments to gender equity, including Sustainable Development Goal 5, health policy processes often fail to adequately integrate gender considerations. This study explores how gender is prioritized in the development and implementation of four Kenyan Health Policy Implementation Strategies (HPIS), offering insight into the role of institutional actors, policy framing, and contextual factors. METHODS: A qualitative case study was conducted involving content analysis of four HPIS documents - KASF II 2020, NSP-NCD 2021, NSP-TB 2019, and COVID-TTS 2019 -

From complexity to parsimony: A systems thinking validation of the multiple streams framework in abortion policy agenda setting.

BACKGROUND: Abortion policy is a highly contested area of health policy. Despite international recognition of abortion as a human right, legal restrictions persist in many countries, and recent decades have seen both liberalization and retrenchment of abortion laws. While much research has examined the politics and outcomes of abortion policy reform, less attention has been paid to the upstream process of how abortion emerges on the policy agenda. This study addresses this gap by focusing on agenda setting for abortion policy, using Kingdon's multiple streams framework (MSF) and systems thinki

Trends in U.S. State Alcohol and Other Drug Use During Pregnancy Policies from 2016 to 2020: Policymaking in the Comprehensive Addiction and Recovery Act Era.

Background: Prior to the U.S. Comprehensive Addiction and Reovery Act of 2016 (CARA) , policymaking on alcohol and drug use during pregnancy was more concentrated on alcohol than drug use policy - although the overlap between the two types of policy was high. Further, the highest levels of legislative activity were requirements to report pregnant women who used alcohol or other drugs and child abuse/neglect policy. Methods: This research brief uses rigorous legal epidemiology methodology to explore state policy activity on alcohol and other drug use during pregnancy after the U.S. Comprehensiv

A Surging Drug Epidemic: Time for Congress to Enact a Mandate on Insurance Companies and Rehabilitation Facilities for Opioid and Opiate Addiction.

This Note begins with a discussion of both the national opioid problem as well as the specific epidemic in Ohio as an example of how it has grown within all of the states. Part II discusses the differences between prescription opioids and opiates, how they can be obtained, what effects they have on the human body, and why the government has an interest in this growing problem. Next, this Note explains how and why there was an increase in access and addiction to prescription opioid pain medication. Following this explanation, the steps the government has taken to try to rectify the issue are ex

The politics of drug control in Nigeria: Exclusion, repression and obstacles to policy change.

BACKGROUND: International agencies have viewed West Africa as a major player in the global trade in cocaine and heroin and in efforts to control that trade, as there have been reports of escalating arrests of drug smugglers, large-scale drug seizures and 'narco-states' in the subregion. It is claimed that a substantial share of the drugs available in Western markets transit through West Africa today and are increasingly used there as well. Notwithstanding this growing alarm, there is little serious scholarship addressing the issue of drugs and drug policy in West Africa. METHODS: The article

Global health impacts of policies: lessons from the UK.

BACKGROUND: The UK government committed to undertaking impact assessments of its policies on the health of populations in low and middle-income countries in its cross-government strategy "Health is Global". To facilitate this process, the Department of Health, in collaboration with the National Heart Forum, initiated a project to pilot the use of a global health impact assessment guidance framework and toolkit for policy-makers. This paper aims to stimulate debate about the desirability and feasibility of global health impact assessments by describing and drawing lessons from the first stage o

Lack of evidence to support policy development for management of contacts of multidrug-resistant tuberculosis patients: two systematic reviews.

BACKGROUND: Existing international guidelines provide different recommendations for the management of contacts of multidrug-resistant tuberculosis (MDR-TB) patients. OBJECTIVE: To conduct two systematic reviews with the aim of identifying chemoprophylactic approaches that are effective in contacts of MDR-TB patients to assist in policy making. DESIGN: We systematically searched the Medline, Embase, Central, LILACS, TRIP and BIOSIS Preview databases for studies on the effectiveness of anti-tuberculosis drugs in preventing active TB in persons at risk of developing MDR-TB. This was done as an

Scientific issues in the design of metrics for inclusion of oxides of nitrogen in global climate agreements.

The Kyoto Protocol seeks to limit emissions of various greenhouse gases but excludes short-lived species and their precursors even though they cause a significant climate forcing. We explore the difficulties that are faced when designing metrics to compare the climate impact of emissions of oxides of nitrogen (NO(x)) with other emissions. There are two dimensions to this difficulty. The first concerns the definition of a metric that satisfactorily accounts for its climate impact. NO(x) emissions increase tropospheric ozone, but this increase and the resulting climate forcing depend strongly on

[Should France continue to use BCG?].

In the context of the decrease of tuberculosis incidence, a re-assessment of the French BCG vaccination policy has been undertaken. The synthesis of the data available, done by the National Institute for Public Health Surveillance (InVS) has led to the conclusion of the absence of justification of the current policy of re-vaccinating children with a negative tuberculin skin test. Data from studies and from the results of the experiences from several countries having discontinued primovaccination confirm the effectiveness of BCG vaccination on childhood tuberculosis, mainly on extra-pulmonary c

The importance of engaging policy-makers at the outset to guide research on and introduction of vaccines: the use of policy-maker surveys.

Face-to-face surveys of policy-makers and other influential leaders are a useful tool to identify, at an early stage, (a) major issues regarding the introduction of a new vaccine, (b) persons and groups in a country who play a major decision-making or influential role in the introduction of vaccines, (c) potential obstacles to the introduction of vaccines, and (d) data-needs of policy-makers to overcome these obstacles. By surveying the opinions and beliefs of those who will make or influence decisions on whether to introduce a new vaccine, these studies can help ensure that research activitie

Closing the Gap Between Evidence and Action: Lessons From a Dramatic Increase in Access to Alcohol.

Alcohol is a leading cause of death, disability, disease, trauma and social problems. In Canada, alcohol consumption is associated with 17,000 deaths annually, with costs from alcohol exceeding revenues in each jurisdiction. Leading health organisations point to policies shown to be effective in reducing alcohol harms. Yet there is a substantial and growing gap between evidence of effective policies to reduce alcohol harms and actual policies being implemented in Canada and elsewhere. This commentary critically examines one such example: the Ontario government's 2023 decision to significantly
