Multiscale model for forecasting Sabin 2 vaccine virus household and community transmission.

Since the global withdrawal of Sabin 2 oral poliovirus vaccine (OPV) from routine immunization, the Global Polio Eradication Initiative (GPEI) has reported multiple circulating vaccine-derived poliovirus type 2 (cVDPV2) outbreaks. Here, we generated an agent-based, mechanistic model designed to assess OPV-related vaccine virus transmission risk in populations with heterogeneous immunity, demography, and social mixing patterns. To showcase the utility of our model, we present a simulation of mOPV2-related Sabin 2 transmission in rural Matlab, Bangladesh based on stool samples collected from inf

Unmet need for COVID-19 vaccination coverage in Kenya.

COVID-19 has impacted the health and livelihoods of billions of people since it emerged in 2019. Vaccination for COVID-19 is a critical intervention that is being rolled out globally to end the pandemic. Understanding the spatial inequalities in vaccination coverage and access to vaccination centres is important for planning this intervention nationally. Here, COVID-19 vaccination data, representing the number of people given at least one dose of vaccine, a list of the approved vaccination sites, population data and ancillary GIS data were used to assess vaccination coverage, using Kenya as an

Projected mortality rates and total deaths from gastrointestinal malignancies in the United States, 1999 to 2023: A time-series analysis.

National mortality from gastrointestinal malignancies has declined, but aggregate trends may obscure demographic and geographic differences. We evaluated trends through 2023 and conditional forecasts of rates and deaths based on historical time-series patterns. We conducted an ecological time-trend study of United States residents aged 25 years or older whose underlying cause of death was a selected gastrointestinal malignancy (International Classification of Diseases, 10th Revision, C16-C20). We combined nonoverlapping records from the Centers for Disease Control and Prevention Wide-ranging O

The global, regional, and national burden of cancer, 1990-2023, with forecasts to 2050: a systematic analysis for the Global Burden of Disease Study 2023.

BACKGROUND: Cancer is a leading cause of death globally. Accurate cancer burden information is crucial for policy planning, but many countries do not have up-to-date cancer surveillance data. To inform global cancer-control efforts, we used the Global Burden of Diseases, Injuries, and Risk Factors Study (GBD) 2023 framework to generate and analyse estimates of cancer burden for 47 cancer types or groupings by age, sex, and 204 countries and territories from 1990 to 2023, cancer burden attributable to selected risk factors from 1990 to 2023, and forecasted cancer burden up to 2050. METHODS: Ca

The problem of scale in the prediction and management of pathogen spillover.

Disease emergence events, epidemics and pandemics all underscore the need to predict zoonotic pathogen spillover. Because cross-species transmission is inherently hierarchical, involving processes that occur at varying levels of biological organization, such predictive efforts can be complicated by the many scales and vastness of data potentially required for forecasting. A wide range of approaches are currently used to forecast spillover risk (e.g. macroecology, pathogen discovery, surveillance of human populations, among others), each of which is bound within particular phylogenetic, spatial

Population-based prediction of trauma volumes at a Level 1 trauma centre.

OBJECTIVE: With an ageing US population, the demographics of traumatic injuries are being significantly altered. Census projections predict that the number of Americans over age 65 will double in the next 20 years. We used stochastic methods to forecast trauma admissions in order to predict the effects of such demographic changes at our trauma centre. METHODS: Age- and sex-related rates of traumatic admission were determined using population statistics and trauma registry data from 1994 to 1999. These rates were then projected from 2000 to 2025 based on both the Lee-Carter and random walk wit

Global trends in age at onset of gout, 1990-2023: a sex-stratified and forecast analysis of the Global Burden of Disease Study 2023.

BACKGROUND: Global gout burden is rising, yet temporal shifts in age at onset remain insufficiently characterized. Understanding secular changes in incident age distribution is essential for anticipating healthcare demands in aging populations. METHODS: We conducted a secondary analysis of the Global Burden of Disease Study 2023 data to evaluate global incident gout trends from 1990 to 2023. Mean age at onset, proportion of young-onset cases (< 40 years), and sex-stratified patterns were analyzed using log-linear regression. Forecasting through 2040 was performed using time-series modeling.

Individual fertility assessment and pro-fertility counselling; should this be offered to women and men of reproductive age?

During the 1970s new contraceptive options developed and legal abortions became accessible. Family planning clinics targeting young women and men provided advice and assistance on contraception. Today, delayed childbearing, low total fertility rates and increasing use of social oocyte freezing create a need for pro-fertility initiatives. Three years ago we established a new separate unit: The Fertility Assessment and Counselling (FAC) clinic. The FAC clinic offers free individual counselling based on a clinical assessment including measurement of serum anti-Müllerian hormone and ovarian and pe

Predicting the mutational drivers of future SARS-CoV-2 variants of concern.

SARS-CoV-2 evolution threatens vaccine- and natural infection-derived immunity as well as the efficacy of therapeutic antibodies. To improve public health preparedness, we sought to predict which existing amino acid mutations in SARS-CoV-2 might contribute to future variants of concern. We tested the predictive value of features comprising epidemiology, evolution, immunology, and neural network-based protein sequence modeling, and identified primary biological drivers of SARS-CoV-2 intra-pandemic evolution. We found evidence that ACE2-mediated transmissibility and resistance to population-leve

Temporal Trends and Future Projections of P ulmonary Embolism and Top Contributing Causes of death in the United States, 1968-2040: insights from the CDC WONDER database.

Pulmonary embolism (PE) is a preventable yet often deadly cardiovascular event. We aimed to investigate PE mortality trends from 1968-2023 and forecast rates through 2040 in the U.S. We analyzed CDC WONDER death certificates for adults ≥ 25 years. Age-adjusted mortality rates (AAMRs), annual percent changes (APCs) were used to analyse trends and Auto-ARIMA/Prophet forecasting models were used for future projections, all stratified by sex, age, race, and contributing cause of death. From 1968-2023, 516,187 PE deaths occurred, with a mean AAMR of 6.06 per 100,000 (AAPC -1.85). Mean AAMRs were 6.

Nationwide patterns of cardiac-related mortality in amyloidosis cases: an epidemiologic study.

INTRODUCTION: Cardiac amyloidosis is frequently overlooked, leading to delayed diagnosis and underestimation of its true impact on cardiac mortality. METHODS: Using CDC WONDER (1999-2020), we identified U.S. decedents aged ≥ 25 years with amyloidosis (ICD-10 E85) as the underlying cause of death and at least one cardiac-related condition as a contributing cause. Age-adjusted mortality rates (AAMRs) per 100,000 population were calculated, and trends were analyzed using Joinpoint regression and autoregressive integrated moving average (ARIMA) forecasting through 2040. RESULTS: Among 16,673 dea
