Cataracts are well known to be associated with myotonic dystrophy. Less well known are the phenomena of recurrent posterior capsule opacification and capsulorhexis contracture post cataract surgery. Two cases are described herein of postoperative capsular complications requiring multiple capsulotomies in patients with myotonic dystrophy. It is proposed that a common aetiology may underlie both posterior capsule opacification and capsulorhexis contracture in myotonic dystrophy cases.

We describe a modified surgical technique of anterior capsulotomy and phacoemulsification for use in hypermature cataracts. Surgical steps included inferior linear capsulotomy with aspiration of milky cortex, inflation of the capsular sac with viscoelastic, a reverse triangular anterior capsulotomy with Vannas scissors, and in-the-bag bimanual sculpting of the nucleus. An oval 5 mm x 6 mm posterior chamber intraocular lens was used with single horizontal or no-stitch wound closure. The technique was used successfully in 20 cases of hypermature cataract. The mean time required to perform phacoe

PURPOSE: To describe methods for performing laser-assisted cataract surgery in small pupils.
METHODS: Iris retractors or a Malyugin Ring (Microsurgical Technology, Redmond, WA) were used for mechanical pupil dilation before laser capsulotomy and lens fragmentation. The laser treatment can be performed with or without use of ophthalmic viscosurgical devices (OVD).
RESULTS: In 73 eyes with small pupils, laser treatment was possible after mechanical dilation device installation. With and without an OVD, no flattening of the anterior chamber or other complications occurred during re-docking, las

We report two cases of late capsular bag distension syndrome (LCBDS), which is a rare but significant complication following cataract surgery and often presents with blurred vision years after intraocular lens (IOL) implantation. Case 1: An 80-year-old male presented with a one-year history of progressive blurred vision in the right eye. His history included uneventful phacoemulsification and multifocal IOL implantation eight years earlier. Examination revealed dense, turbid fluid behind the IOL. Scheimpflug imaging and anterior segment optical coherence tomography (AS-OCT) detected hyperinten

Thirty-seven children aged two to ten years old sustained unilateral traumatic cataract. Aspiration of the traumatic lens was followed by intraocular lens implantation in these 37 eyes. In 23 eyes an IOL was inserted during the initial surgical setting; in 14 eyes a secondary implantation was performed. In 34 eyes posterior capsulotomy and anterior vitrectomy was performed before insertion of the lens. No major post-operative complications were observed during the period of follow-up ranging from six to 24 months. In three cases, posterior capsulotomy was not carried out. Secondary surgical in

INTRODUCTION: After cataract surgery there is a higher incidence of posterior capsular opacification (secondary cataract) in myopia than in normal population, and it varies from 40 to 60% [1, 2].
METHODS: Fifty myopic eyes operated for cataract at Prof. Dr. Ivan Stankovitsh Eye Department from 1988 to 1995 were examined for capsular opacification (secondary cataract). Extracapsular cataract extraction was performed in all patients and in 32 patients the posterior intraocular lens was placed. Capsular opacification was classified by type and treated with Nd:YAG laser or discussion at least 9 m

Fifty cases of clubfeet (38 children) were evaluated 10 years after surgery. Turco's method was used in 22 cases, while 24 were managed with Achilles tendon lengthening and dorsal capsulotomy. After Turco's operation, good results were achieved in 69% and the deformity recurred in 31%. In the second group, the success rate was 63%. Recurrences are due to insufficient reduction of the talocalcaneonavicular joint. The authors conclude that Achilles tendon lengthening and posterior capsulotomy should be carried out by age 4 months. If by the age of 1 year the foot has not yet assumed its normal p

Five patients developed retinal detachments within one month of undergoing neodymium-YAG laser posterior capsulotomy. In four patients rhegmatogenous retinal detachments developed in association with typical aphakic breaks; in the fifth patient a previously stable extramacular traction detachment extended into the fovea. In all five patients scleral buckling or vitrectomy successfully reattached the retina and visual acuity improved. None of the findings associated with the detachments could be definitely attributed to the YAG laser. These included the lack of structural or positional changes

A follow-up of 120 pseudophakic eyes of 85 patients that underwent surgery for senile cataract showed a definite superiority of extracapsular surgery over intracapsular surgery as far as the longterm fate of the retina concerns. Comparable series have been chosen. Retinal detachment did not occur in extracapsular eyes that never had posterior capsulotomy, and occurred most often after intracapsular surgery. Signs of previous or existing maculopathy were searched for with the usual tests (funduscopy, visual acuity, Amsler test) and moreover with Haidinger's brushes test. The macula turned out t

Posterior capsule opacification (PCO) is the most common complication after cataract surgery, with an incidence of 30%. It tends to be considered a normal event in the natural history of cataract surgery. Better understanding of its pathophysiology and advancement of intraocular lens material and design along with the improvement of phacoemulsification technique have contributed to decrease the incidence of PCO. Although treatment by Nd: YAG laser posterior capsulotomy is quick and non-invasive, the opening of the posterior capsule may be associated with numerous complications. Prevention rema

Clinical data of 427 cases (458 eyes) of extracapsular cataract extraction and IOL implantation gave the impression that vitreous prolapse due to disruption of the zonula and the posterior capsule was a malpractical complication. Forcible capsulotomy too large or deep and uneven irrigating pressure were the common causes of zonular disruption. Obstructed passage and insufficient separation of the nucleus from the posterior cortical bed leading to difficult nuclear delivery, aspiration of cortex in the posterior chamber, poorly filled anterior chamber, insufficient room for the IOL, improper in

UNLABELLED: Nd-YAG posterior capsulotomy in treatment of secondary cataract has the main disadvantage of increasing the risk for pseudophakic retinal detachment by destroying the posterior lens capsule. This is of great interest in high myopic eyes with an increased risk for pseudophakic retinal detachment. In 219 cases of secondary cataract in our clinic a surgical posterior capsule polishing has been performed and this group was followed up over 6 years looking for intra- and postoperative complications.
PATIENTS: The examined group included 209 myopic eyes with a bulbus length > 25 mm and

Intraocular lens (IOL) design, optical configuration, and placement have potential effects on postoperative outcome. Laboratory studies have suggested that one-piece, biconvex designs may reduce or delay posterior capsular opacification and that in-the-bag fixation of the posterior chamber IOL may reduce inflammation. To document the clinical significance of IOL design and placement, we conducted a randomized, prospective, clinical trial. Six hundred uncomplicated capsulorhexis and phacoemulsification patients were randomized in a three-factor design to receive an IOL that was one-piece or thr

By administering fluorescein intravenously to 95 patients we calculated the ratio of fluorescein concentration in the vitreous at the time of its peak level compared with the estimated unbound concentration of fluorescein in the plasma at the same time. We studied 12 normal phakic and 83 aphakic eye approximately two months, one year, and more than two years after cataract extraction. All the eyes had undergone intracapsular cataract extraction or extracapsular cataract extraction, with or without posterior capsulotomy, because of senile cataract.
