Quality of Postoperative Pain Management After Maxillofacial Fracture Repair.

BACKGROUND: Effective pain management is an essential component in the perioperative care of surgical patients. However, postoperative pain after maxillofacial fracture repair and its optimal therapy has not been described in detail. MATERIALS AND METHODS: In a prospective cohort study, 95 adults rated their pain on the first postoperative day after maxillofacial fracture repair using the questionnaire of the Quality Improvement in Postoperative Pain Management (QUIPS) project. Quality Improvement in Postoperative Pain Management allowed for a standardized assessment of patients' characterist

Chronic postoperative pain: recent findings in understanding and management.

Chronic postoperative pain is a poorly recognized potential outcome from surgery. It affects millions of patients every year, with pain lasting for months to years, resulting in patient suffering and ensuing economic consequences. The operations with the highest incidence of chronic postoperative pain are amputations, thoracotomies, cardiac surgery, and breast surgery. Other risk factors include preoperative pain, psychological factors, demographics, and the intensity of acute postoperative pain. Attempts to prevent chronic postoperative pain have often led to debatable results. This article p

Markov chain evaluation of acute postoperative pain transition states.

Previous investigations on acute postoperative pain dynamicity have focused on daily pain assessments, and so were unable to examine intraday variations in acute pain intensity. We analyzed 476,108 postoperative acute pain intensity ratings, which were clinically documented on postoperative days 1 to 7 from 8346 surgical patients using Markov chain modeling to describe how patients are likely to transition from one pain state to another in a probabilistic fashion. The Markov chain was found to be irreducible and positive recurrent, with no absorbing states. Transition probabilities ranged from

AMPAkines Target the Nucleus Accumbens to Relieve Postoperative Pain.

BACKGROUND: AMPAkines augment the function of α-amino-3-hydroxy-5-methyl-4-isoxazolepropionic acid (AMPA) receptors in the brain to increase excitatory outputs. These drugs are known to relieve persistent pain. However, their role in acute pain is unknown. Furthermore, a specific molecular and anatomic target for these novel analgesics remains elusive. METHODS: The authors studied the analgesic role of an AMPAkine, CX546, in a rat paw incision (PI) model of acute postoperative pain. The authors measured the effect of AMPAkines on sensory and depressive symptoms of pain using mechanical hypers

Preoperative anxiety and postoperative pain in patients undergoing septoplasty.

OBJECTIVE: The aim of the study was to determine whether psychological variables such as preoperative anxiety could serve as predictors of the postoperative pain response. METHODS: Sixty consecutive patients with deviated nasal septum were evaluated in the pre- and postoperative periods. A questionnaire that included sociodemographic features was administered to patients. Preoperative anxiety was assessed by the Spielberger State-Trait Anxiety Index. The severity of pain was recorded with a ten-point Visual Analog Scale at the immediate postoperative period (baseline) and at postoperative hou

[Cause analysis of postoperative pain after total knee arthroplasty].

Total knee arthroplasty (TKA) has been successfully applied for the treatment of the knee pathology at the end stage such as osteoarthritis and rheumatoid arthritis. Although TKA has became a very mature technology, some patients still suffer from the persistent pain after surgery. The cause of this pain have been recognized as the operation or technical error in most cases, and it usually requires a surgical intervention. Only when the cause of pain is judged accurately, can the operation and other factors be estimated correctly, determining the appropriate treatment methods. In the article,

Epidural technique for postoperative pain: gold standard no more?

Epidural analgesia is a well-established technique that has commonly been regarded as the gold standard in postoperative pain management. However, newer, evidence-based outcome data show that the benefits of epidural analgesia are not as significant as previously believed. There are some benefits in a decrease in the incidence of cardiovascular and pulmonary complications, but these benefits are probably limited to high-risk patients undergoing major abdominal or thoracic surgery who receive thoracic epidural analgesia with local anaesthetic drugs only. There is increasing evidence that less i

Internal sphincterotomy reduces postoperative pain after Milligan Morgan haemorrhoidectomy.

BACKGROUND: Over the last few years, there has been increasing attention on surgical procedures to treat haemorrhoids. The Milligan-Morgan haemorrhoidectomy is still one of the most popular surgical treatments of haemorrhoids. The aim of the present work is to assess postoperative pain, together with other early and late complications, after Milligan-Morgan haemorrhoidectomy as we could observe in our experience before and after performing an internal sphincterotomy. METHODS: from January 1980 to May 2007, we operated 850 patients, but only 699 patients (median age 53) were included in the pr

Morphine-6-glucuronide: morphine's successor for postoperative pain relief?

In searching for an analgesic with fewer side effects than morphine, examination of morphine's active metabolite, morphine-6-glucuronide (M6G), suggests that M6G is possibly such a drug. In contrast to morphine, M6G is not metabolized but excreted via the kidneys and exhibits enterohepatic cycling, as it is a substrate for multidrug resistance transporter proteins in the liver and intestines. M6G exhibits a delay in its analgesic effect (blood-effect site equilibration half-life 4-8 h), which is partly related to slow passage through the blood-brain barrier and distribution within the brain co

Persistent postoperative pain after the Chrisman-Snook ankle reconstruction.

One complication of the Chrisman-Snook ankle reconstruction is persistent postoperative pain. The incidence of this complication ranges from 7% to 60%. We report 10 cases of surgical exploration for persistent pain after Chrisman-Snook reconstructions. In 6 of the 10 cases a surgically correctable cause was found for the pain. The identified pathologic lesions in these 6 patients could have been prevented by careful attention to detail in the index operation. Surgical exploration should be considered for any of the following findings: persistent point tenderness (especially at the graft tunnel

Postoperative pain management in infants and children: new developments.

The elimination of pain should be of outstanding importance for all people caring for children. A concept of balanced analgesia including non-steroidal anti-inflammatory drugs, opioids and local anaesthetics is widely accepted. This review focuses on extending analgesia beyond the immediate postoperative period, the understanding of pharmacokinetic-pharmacodynamic interactions of paracetamol, the side-effects of opioids during patient-controlled administration, and the position of ropivacaine in paediatric pain management. For the majority of clinical situations, however, we already have estab

[Preventive analgesia: true of preventing the postoperative pain syndrome].

A total of 152 patients subjected to operations mainly of an orthopaedic profile were divided into 8 groups for the development of the optimal method of preventive analgesia. The best results were attained by combined use of opiate premedication, regional blocking as a component of anesthesiologic care, and parenteral diclophenak-Na before and after the operation. 31.5% of patients in this group did not need any postoperative analgesia. In the rest cases we observed the longest postoperative pain-free period, the least intensity of pain, and low need in additional analgesics during the first 2

Thoracic epidural clonidine and morphine for postoperative pain relief.

This study was undertaken to evaluate the potentiation of the postoperative analgesic effect of thoracic epidural morphine by coadministration of thoracic epidural clonidine in a randomized double-blinded design. Twenty patients underwent radical gastrectomy under combined general anaesthesia (enflurane and nitrous oxide/oxygen) and epidural anaesthesia with local anaesthetics. They received a thoracic epidural bolus injection of either 0.05 mg.kg-1 morphine plus 3 micrograms.kg-1 clonidine (M+C group; n = 10) or 0.05 mg.kg-1 morphine alone (M group; n = 10) immediately before completion of su

Association of patient-reported substance use and postoperative pain.

BACKGROUND: Substance use among surgical patients is common and correlated with greater postoperative opioid use. However, the association of preoperative substance use on postoperative pain is unknown. In this context, a brief, validated tool such as the Tobacco, Alcohol, Prescription medication, and other Substance use (TAPS) Questionnaire could provide valuable information to assess the risk of poor postoperative pain control. METHODS: We prospectively identified adults aged 18 years and older undergoing common surgical procedures between December 2018 and July 2023 across three institutio
