Adherence to coronary artery disease secondary prevention medicines: exploring modifiable barriers.

BACKGROUND: Non-adherence to secondary prevention medicines (SPMs) among patients with coronary artery disease (CAD) remains a challenge in clinical practice. This study attempted to identify actual and potential modifiable barriers to adherence that can be addressed in cardiology clinical practice. METHODS: This was a cross-sectional, postal survey-based study of the medicines-taking experience of patients with CAD treated at a secondary/tertiary care centre. All participants had been on SPM for ≥3 months. RESULTS: In total, 696 eligible patients were sent the survey and 503 responded (72.3

A Case for Promoting Movement Medicine: Preventing Disability in the LIFE Randomized Controlled Trial.

BACKGROUND: The movement profile of older adults with compromised function is unknown, as is the relationship between these profiles and the development of major mobility disability (MMD)-a critical clinical outcome. We first describe the dimensions of movement in older adults with compromised function and then examine whether these dimensions predict the onset of MMD. METHODS: Older adults at risk for MMD (N = 1,022, mean age = 78.7 years) were randomized to receive a structured physical activity intervention or health education control. We assessed MMD in 6-month intervals (average follow-u

Sexual safety and HIV prevention in travel medicine: Practical considerations and new approaches.

Travellers who engage in sexual contact with a new sexual partner abroad may be at high risk of HIV and other sexually transmitted infections (STIs), but these risks can be reduced through appropriate planning during the pre-travel clinic visit. Here we discuss strategies available to the clinician to maximise travellers' sexual safety during travel. Strategies may include immunizations, condoms, HIV pre-exposure prophylaxis (PrEP), HIV post-exposure prophylaxis (PEP), self-initiated treatment of symptomatic bacterial STIs, post-exposure prophylaxis for bacterial STIs and hormonal contraceptio

Discontinuation of Preventive Medicines in Older People with Limited Life Expectancy: A Systematic Review.

BACKGROUND: In the presence of multimorbidity and limited life expectancy (LLE), the need for continued use of preventive medicines becomes uncertain as they may neither improve health nor confer continued health benefits. OBJECTIVE: Our objective was to systematically review the literature to examine the discontinuation of preventive medicines in older people with LLE. METHODS: A systematic literature search was conducted using the Ovid MEDLINE, Embase, Cumulative Index to Nursing and Allied Health Literature, and the Cochrane Central Register databases. Studies investigating discontinuatio

[To investigate the strategy of Chinese medicine for prevention and treatment of atherosclerosis based on vascular aging].

Atherosclerosis, a chronic degenerative disease mainly attacks the middle-aged and the aged population as they grow old. Anti-angiocellular aging has gradually become a new strategy for atherosclerosis. In the process of atherosclerosis developing, endothelial cell renewing is speeding. Various biological function disorders that induce blood vessel aging emerge, which leads to changes of the telomere and telomerase, resulting in aged endothelial cells and dysfunction. Telomere and telomerase may play key roles in the etiological factors such as inflammation and AS plaque. In our previous work

Preventing the aortic complications of Marfan syndrome: a case-example of translational genomic medicine.

The translational path from pharmacological insight to effective therapy can be a long one. We aim to describe the management of Marfan syndrome as a case-example of how pharmacological and genomic insights can contribute to improved therapy. We undertook a literature search for studies of Marfan syndrome, to identify milestones in description, understanding and therapy of the syndrome. From the studies retrieved we then weaved an evidence-based description of progress. Marfan syndrome shows considerable heterogeneity in clinical presentation. It relies on defined clinical criteria with confir

Traditional Chinese medicine prevents inflammation in CCl4-related liver injury in mice.

Alternative medicines are being increasingly used and investigated in the management of a variety of disorders. Hepatitis is a common indication for the use of alternative therapies but evidence for the efficacy of many compounds is lacking. We have utilized a well-defined model of liver injury to study the efficacy of three herbal products designed to assist in the management of liver disease. Mice were exposed to carbon tetrachloride (CCL4) given intragastrically after they had been pretreated for five days with either saline or one of four doses of silymarin extract or CH100 (a Chinese herb

[Knowledge of medicine required for the exercise cure for prevention of fall and fracture in elderly people].

The typical medicine constituting elderly people cause of a fall is the anxiolytic and hypnotic drugs of Benzodiazepines. The danger of the fall by the prolonged action type or the medicine with a strong line relaxation action is suggested especially. Moreover, anti-depressant drugs and anti-hypertensive drugs with alpha-adrenaline interception action also causes orthostatic hypotention, and induces a fall. In elderly, there are many complications and there is a tendency for combined use medicine to increase in number. Since the danger of fall increases in proportion to the number of medicines

[Manual medicine in otorhinolaryngology. Theoretical principles-- clinical practice--principles of therapy and prevention].

Manual medicine or ¿chirotherapy¿ has become a recognized part of medical training in Germany. In the last few decades, research in various fields has shed light on the pathoanatomic and pathophysiologic causes of functional disorders of the cervical vertebral column whose previous management was characterized by treatment of symptoms. The Arthron system provides a cybernetic model in the form of a closed-loop feedback system for visualization and training purposes. A momentary isolated disorder or recurrent exogenous disorders can be represented as reversible processes; the phases of reflex b

Obesity, Pregnancy, and Cardiovascular Prevention: A Lifestyle Medicine Call to Action for Preconception Health.

Obesity in women of reproductive age is a major and growing contributor to adverse pregnancy outcomes and long-term cardiovascular disease (CVD). Pregnancy is increasingly recognized as a physiological stress test that can reveal underlying cardiometabolic vulnerability. In this context, obesity amplifies risk through inflammation, endothelial dysfunction, and metabolic dysregulation. A life-course approach to prevention identifies preconception health as a critical and actionable window for intervention. Lifestyle medicine, through its core pillars of nutrition, physical activity, sleep, stre

An evidence-informed framework for workforce well-being and burnout prevention in emergency medicine.

The practice of emergency medicine is characterized by high acuity, crowding, and frequent exposure to trauma and moral distress. Consistent with the National Academy of Medicine's Quintuple Aim, threats to workforce well-being, including high rates of burnout, remain among the most pressing challenges in emergency medicine and are strongly associated with reduced quality of care and workforce attrition. While earlier discourse has disproportionately emphasized individual "resilience," contemporary scholarship indicates that burnout and impaired well-being are fundamentally systems-driven outc

Research progress on the prevention and treatment of alcoholic liver disease with medicinal and edible herbs.

ETHNOPHARMACOLOGICAL RELEVANCE: Medicinal and edible herbs (MEHs), including ginger (Zingiber officinale), goji berries (Lycium barbarum), and hawthorn (Crataegus pinnatifida), serve dual roles as both dietary components and therapeutic agents. Their pharmacological activities are primarily attributed to natural bioactive compounds, such as polysaccharides polyphenols, flavonoids, and saponins. These constituents exhibit multifunctional properties, including antioxidant, anti-inflammatory, and immunomodulatory effects. AIMS OF THE REVIEW: This study identifies 61 herbs that exhibit protective

[Role of NLRP3 inflammasome in diabetic neuropathy and prevention and treatment with traditional Chinese medicine].

As one of the most frequent complications of diabetes, diabetic neuropathy often involves peripheral and central nervous systems. Neuroinflammation is the key pathogenic factor of secondary nerve injury in diabetes. NOD-like receptor pyrin domain-containing 3(NLRP3) inflammasome is a group of subcellular multiprotein complexes, including NLRP3, apoptosis associated speck-like protein(ASC), and pro-cysteinyl aspartate specific proteinase 1(pro-caspase-1). NLRP3 inflammasome is an inducer of innate immune responses. Its activation stimulates the inflammatory cascade reaction, promotes the releas

Target product profiles for novel medicines to prevent and treat preeclampsia: An expert consensus.

BACKGROUND: Preeclampsia and eclampsia are a leading cause of global maternal and newborn mortality. Currently, there are few effective medicines that can prevent or treat preeclampsia. Target Product Profiles (TPPs) are important tools for driving new product development by specifying upfront the characteristics that new products should take. Considering the lack of investment and innovation around new medicines for obstetric conditions, we aimed to develop two new TPPs for medicines to prevent and treat preeclampsia. METHODS AND FINDINGS: We used a multi-methods approach comprised of a lite

The Role of the "Traditional Chinese Medicine Proclamation" in Cholera Prevention and Control in Modern China.

During the period of social transformation in modern China, the sudden onslaught of malignant infectious diseases, the helplessness of the public, and the impact of the modern Western medical system promoted the profound self-revolution of traditional Chinese medicine (TCM). Multiple factors, including politics, economy or religion contributed to the formulation of paper-based "TCM proclamation". This paper, based on the prevention and control of cholera advocated in TCM proclamation, analyzes the new ways of cholera spread in China in early modern times and the complicated "country-region-med
