Pathological gambling treated with cognitive behavior therapy: a case report.

A novel approach to the cognitive behavioral treatment of pathological gambling is described. The approach is based on the hypothesis that biased information may form the basis of the rules governing gambling behavior. The treatment focused on modifying assumptions and beliefs underlying the gambling behavior of a 47-year-old male with a 26-year gambling history. Based on a 6-month follow-up, the intervention successfully reduced the frequency of the client's gambling behavior from 10 times per month in the 3 years prior to treatment to three total times in the 6-month follow-up period.

Schizophrenic subtypes and differential responsiveness to reward and punishment.

Variations in responsivity to primary, secondary asocial and interpersonal rewards and punishments between major schizophrenic subgroups were investigated in a probability learning study. The subgroups compared were process and reactive, paranoid and nonparanoid, long- and short-hospitalization, and long- and short-length of illness schizophrenics. No evidence of differential responsivity to these reward-punishment/modality combinations appeared between high and low scorers on any of the four schizophrenic subgrouping dimensions. The groups then were matched closely so that the effects of each

Reversal learning in C58 mice: Modeling higher order repetitive behavior.

Restricted, repetitive behaviors are diagnostic for autism and prevalent in other neurodevelopmental disorders. These behaviors cluster as repetitive sensory-motor behaviors and behaviors reflecting resistance to change. The C58 mouse strain is a promising model for these behaviors as it emits high rates of aberrant repetitive sensory-motor behaviors. The purpose of the present study was to extend characterization of the C58 model to resistance to change. This was done by comparing C58 to C57BL/6 mice on a reversal learning task under either a 100% or 80%/20% probabilistic reinforcement schedu

MB-SwinRefiner: Mammographic Mass Segmentation with Probability-Guided Residual Refinement.

Accurate mammographic mass segmentation is crucial for computer-aided breast cancer diagnosis, but remains challenging because masses may be small, low-contrast, irregularly shaped, and partially obscured by dense fibroglandular tissue. Although recent methods have improved contextual representation and multi-scale feature extraction, reliable segmentation still requires better integration of lesion-scale representation, contour-derived auxiliary supervision, and local probability-map refinement. This paper proposes MB-SwinRefiner, a novel two-stage framework consisting of a Multi-scale Bounda

Rate and noise in human amygdala drive increased exploration in aversive learning.

To cope in uncertain environments, animals must balance their actions between using current resources and searching for new ones1. This exploration-exploitation dilemma has been studied extensively in paradigms involving positive outcomes, and neural correlates have been identified in frontal cortices and subcortical structures2-11, including the amygdala12. Importantly, exploration is just as essential for survival or well-being when trying to avoid negative outcomes, yet we do not know whether the single-neuron mechanisms that drive exploration are shared across positive and negative environ

Evaluation of Halpern's "structural component" for improving critical thinking.

Halpern (1998) proposed a four-component model for promoting the transfer of critical thinking. One of them, the "structural component," focuses on how to organize teaching so that critical thinking skills can be generalized. Here, we assess the efficiency of that type of organization. Thus, one group of university students received instruction following the suggestions specified in that component and their performance was compared with that of other university students who received instruction in the same skills but using a different procedure, and with that of a control group. In comparison

Failure to construct and transfer correct representations across probability problems.

Previous studies carried out on "purely random" situations (with dice or poker chips) show the difficulties encountered by people in such situations, however simple they may be. In fact, in this type of situation, prior knowledge guides spontaneous representations, and the "errors" observed could be explained by the activation of "implicit models" which form the basis of erroneous representations. 42 statistically naïve undergraduates were given several variants of a probability problem on which errors are common. In a learning phase, subjects were given four problems involving geometric figur

Endoscopic carpal tunnel release.

ECTR is an elegant minimally invasive operative treatment of CTS, providing a rapid rehabilitation without increasing the risk of complications. However, there is a significant learning curve and the cost of the operation is significantly higher than with conventional open technique. However, the faster rehabilitation and the probability of reducing sick leave may lead to ECTR being overall more cost-effective than open CTR.

Neural response to reward anticipation under risk is nonlinear in probabilities.

A widely observed phenomenon in decision making under risk is the apparent overweighting of unlikely events and the underweighting of nearly certain events. This violates standard assumptions in expected utility theory, which requires that expected utility be linear (objective) in probabilities. Models such as prospect theory have relaxed this assumption and introduced the notion of a "probability weighting function," which captures the key properties found in experimental data. This study reports functional magnetic resonance imaging (fMRI) data that neural response to expected reward is nonl

Machine learning-based prediction models for parathyroid carcinoma using pre-surgery cognitive function and clinical features.

Patients with parathyroid carcinoma (PC) are often diagnosed postoperatively, due to incomplete resection during the initial surgery, resulting in poor outcomes. The aim of our study was to investigate the pre-surgery indicators of PC and try to develop a predictive model for PC utilizing machine learning. Evaluation of pre-surgery neuropsychological function and confirmation of pathology were carried out in 133 patients with primary hyperparathyroidism in Beijing Chaoyang Hospital from December 2019 to January 2023. Patients were randomly divided into a training cohort (n = 93) and a validati

Aortic dissection albeit negative d-dimers and zero clinical probability - Another facet of medicine's deadly chameleon.

Aortic dissection, the rupture of the aorta's intimal and medial layers, leading to the formation of a false lumen, is a relatively common disease with high mortality. So far, while not addressing penetrating aortic ulcer or intramural hematoma, current guidelines take a negative value for d-dimers for a sufficient method to rule out aortic dissection in patients with a low clinical probability, as calculated by the ADD risk score. We present two cases of patients with acute aortic dissection, albeit presenting with a low clinical probability for acute aortic dissection and negative values for

Beta burst dynamics in the motor cortex are reshaped through sensorimotor refinement.

Beta-band activity (15-30 Hz) in motor cortex is closely linked to movement-related processing, yet its transient burst dynamics during long-term learning remain poorly understood. We used high-density electroencephalography (HD-EEG) to examine how beta bursts change over nine sessions of a bimanual coordination task, under adaptive and non-adaptive training conditions. Both training conditions led to motor skill learning, with the adaptive group improving more during training, but the non-adaptive group showing better retention. With training, contralateral primary motor cortex showed stronge

Obesity prediction: Novel machine learning insights into waist circumference accuracy.

AIMS: This study aims to enhance the precision of obesity risk assessments by improving the accuracy of waist circumference predictions using machine learning techniques. METHODS: We utilized data from the NHANES and Look AHEAD studies, applying machine learning algorithms augmented with uncertainty quantification. Our approach centered on conformal prediction techniques, which provide a methodological basis for generating prediction intervals that reflect uncertainty levels. This method allows for constructing intervals expected to contain the true waist circumference values with a high degr

Enablers to enhance school-based adolescents with intellectual disabilities' learning: A narrative approach.

Society has always understood individuals with intellectual disabilities from a stance of deficiency, resulting in limited expectations and prospects for school-going adolescents with intellectual disabilities. The study used the social model of disability to explore teachers' perceptions of enablers to enhance school-based adolescents with intellectual disabilities' learning and took cognisance of a wave of policies aimed at including adolescents with intellectual disabilities in mainstream education. A qualitative narrative approach was used for the study. Using a narrative methodological fr

Neuronal representation of saccadic error in macaque posterior parietal cortex (PPC).

Motor control, motor learning, self-recognition, and spatial perception all critically depend on the comparison of motor intention to the actually executed movement. Despite our knowledge that the brainstem-cerebellum plays an important role in motor error detection and motor learning, the involvement of neocortex remains largely unclear. Here, we report the neuronal computation and representation of saccadic error in macaque posterior parietal cortex (PPC). Neurons with persistent pre- and post-saccadic response (PPS) represent the intended end-position of saccade; neurons with late post-sacc

Impaired Statistical Learning in Developmental Dyslexia.

PURPOSE: Developmental dyslexia (DD) is commonly thought to arise from phonological impairments. However, an emerging perspective is that a more general procedural learning deficit, not specific to phonological processing, may underlie DD. The current study examined if individuals with DD are capable of extracting statistical regularities across sequences of passively experienced speech and nonspeech sounds. Such statistical learning is believed to be domain-general, to draw upon procedural learning systems, and to relate to language outcomes. METHOD: DD and control groups were familiarized w

A Machine Learning Ensemble Classifier for Early Prediction of Diabetic Retinopathy.

The main complication of diabetes is Diabetic retinopathy (DR), retinal vascular disease and it leads to the blindness. Regular screening for early DR disease detection is considered as an intensive labor and resource oriented task. Therefore, automatic detection of DR diseases is performed only by using the computational technique is the great solution. An automatic method is more reliable to determine the presence of an abnormality in Fundus images (FI) but, the classification process is poorly performed. Recently, few research works have been designed for analyzing texture discrimination ca

Classifying 2-year recurrence in patients with dlbcl using clinical variables with imbalanced data and machine learning methods.

BACKGROUND: Treatments are limited for patients with relapsed/refractory Diffuse large B-cell lymphoma (DLBCL), and their survival rate is low. Prediction of the recurrence hazard for each patient could provide a reference regarding chemotherapy regimens for clinicians to extend patients' period of long-term remission. As current strategies cannot satisfy such need, we have established predictive models to classify patients with DLBCL with complete remission who had recurrences in 2 years from ones who did not. METHODS: We assessed 518 patients with DLBCL and measured 52 variables of each pat

cAPM: Continual AI-assisted pace-mapping with active learning.

Ventricular tachycardia (VT) is a life-threatening rhythm disorder and a major cause of sudden cardiac death. Pace-mapping is a standard clinical procedure for identifying the intervention target during catheter ablation of VT. It requires the clinicians to pace different sites in ventricles and rapidly interpret the resulting electrocardiograms (ECGs) to determine where to pace next or whether a target site has been identified. Active learning-based AI models have been proposed to guide clinicians to the next pacing site, showing promise in reducing the number of pacing sites and improving th

Determining the Survival Impact and Cost-Effectiveness of Multi-Gene Panel Sequencing in Metastatic Colorectal Cancer With Super Learning Approaches.

OBJECTIVE: To determine the effectiveness and cost-effectiveness of multi-gene panel sequencing compared to single-gene KRAS testing for metastatic colorectal cancer (mCRC). STUDY SETTING AND DESIGN: British Columbia, Canada (BC) is a provincial single-payer public healthcare system, and it was the first province to publicly reimburse multi-gene sequencing for mCRC. Panels expand treatment de-escalation by expanding RAS testing for more precise targeting of anti-EGFR therapies. Reimbursement of panels remains unequal across healthcare systems given uncertain clinical and economic impacts. Our
