Surgical morbidities of sagittal split ramus osteotomy versus intraoral vertical ramus osteotomy for the correction of mandibular prognathism: a randomized clinical trial.

The sagittal split ramus osteotomy (SSRO) and intraoral vertical ramus osteotomy (IVRO) are two common orthognathic procedures for the treatment of mandibular prognathism. This randomized clinical trial compared the surgical morbidities between SSRO and IVRO for patients with mandibular prognathism over the first 2 years postoperative. Ninety-eight patients (40 male, 58 female) with a mean age of 24.4±3.5 years underwent bilateral SSRO (98 sides) or IVRO (98 sides) as part or all of their orthognathic surgery. IVRO presented less short-term and long-term surgical morbidity in general. The SSRO

Comparison of condylar morphology changes and position stability following unilateral and bilateral sagittal split mandibular ramus osteotomy in patients with mandibular prognathism.

BACKGROUND: Unilateral sagittal split ramus osteotomy (USSRO) is not widely used given the postoperative instability caused by the inevitable rotation of the mandibular segment during surgery. However, the influence of mandibular movement on the condylar morphology and position stability has not been completely explored. The aim of the study was to quantitatively evaluate the effect of USSRO on the condylar surface morphology changes and postoperative stability in patients with mandibular lateral prognathism and compare these findings with the classic bilateral sagittal split ramus osteotomy (

Association of Growth Hormone Receptor Gene Polymorphisms (rs6180, rs6182, rs6184) with Skeletal Class III Malocclusion and Prognathic Mandibles in the Deutero-Malay Race.

OBJECTIVE: A prognathic mandible, characterized by the anterior protrusion of the mandible, is often associated with skeletal class III malocclusion. Polymorphisms rs6180, rs6182, and rs6184 in the growth hormone receptor (GHR) gene are thought to influence the development of this condition among various races. This study explores the link between these GHR gene polymorphisms and class III skeletal malocclusion in individuals with prognathic mandibles within the Deutero-Malay race. MATERIALS AND METHODS: Saliva samples from 104 participants were analyzed using polymerase chain reaction sequen

Maxillary molar intrusion and transverse decompensation to enable mandibular single-jaw surgery with rotational setback and transverse shift for a patient with mandibular prognathism and asymmetry.

When performing single-jaw surgery for mandibular setback, the distal segment of the mandible is brought distally along the occlusal plane, leaving the relationship of the B-point and pogonion unchanged. Double-jaw surgery for rotation of the maxillomandibular complex can be considered for solving this problem. However, maxillary surgery for rotational setback of the mandible can be replaced with orthodontic intrusion of the maxillary molars. Correcting the mandibular asymmetry that frequently accompanies mandibular prognathism often requires corrections of roll, yaw, and transverse shift of t

Orthognathic surgery with removal of lipoma in the asymmetric mandibular prognathism of a patient with a mandibular bone defect due to intramuscular lipoma on the medial aspect of the ramus: a case report.

Lipomas, the most common soft-tissue mesenchymal neoplasms in adults, are characterized by the proliferation of mature white adipocytes without cytologic atypia. Lipomas are rarely observed in the head and neck region. We present a case of resection and orthognathic surgical removal of an intramuscular lipoma of the mandible with involvement of the mandibular ramus and condylar head and neck. An 18-year-old female patient was referred to our hospital for orthognathic surgery for the management of facial asymmetry and mandibular prognathism. The patient did not present with facial swelling, pai

Comparable aesthetic outcomes of a hybrid modified intraoral vertico-sagittal ramus osteotomy technique and conventional bilateral sagittal split ramus osteotomy for asymmetric mandibular prognathism in a youthful population.

Conventional bilateral sagittal split ramus osteotomy (BSSRO) for asymmetric mandibular prognathism carries risks to the temporomandibular joints and inferior alveolar nerve. A hybrid technique, combining a modified L-shaped intraoral vertico-sagittal ramus osteotomy (mIVSRO) with a contralateral SSRO, may mitigate these risks, but its aesthetic outcomes are unproven. The aim of this study was to compare the aesthetic efficacy of the hybrid versus conventional BSSRO techniques, adjusting for baseline asymmetry. This retrospective cohort study compared 30 patients with asymmetric mandibular pro
