Limited role of culture conversion for decision-making in individual patient care and for advancing novel regimens to confirmatory clinical trials.

BACKGROUND: Despite recent increased clinical trials activity, no regimen has proved able to replace the standard 6-month regimen for drug-sensitive tuberculosis. Understanding the relationship between microbiological markers measured during treatment and long-term clinical outcomes is critical to evaluate their usefulness for decision-making for both individual patient care and for advancing novel regimens into time-consuming and expensive pivotal phase III trials. METHODS: Using data from the randomized controlled phase III trial REMoxTB, we evaluated sputum-based markers of speed of cleara

Current management of glaucoma and the need for complete therapy.

OBJECTIVE: The placement of osteosynthetic materials in the leg may be complicated by hardware exposure. Successful soft tissue reconstruction often provides a critical means for limb salvage in patients with hardware exposure in the leg. Free flaps are currently considered the standard surgical procedure for soft tissue coverage of the wounds with internal hardware exposure. However, to date, no conclusive literature shows the superiority of a specific type of flap. MATERIALS AND METHODS: The current review compares data from the literature concerning outcomes and complications of free and p

[Mild cognitive impairment].

From today's point of view, patients with a mild cognitive impairment (MCI) are at risk for developing a dementia. This clinical picture occurs frequently in the population and is easily recognized through simple tests and interviews conducted in the general and neurological medical practice. A goal of the diagnostics is to detect potentially redressable causes, above all depressive disorders, and to introduce a therapy. In many cases, however, MCI is an early stage of Alzheimer's disease or other neurodegenerative processes. When such a cause is suspected, the most important medical measure i

[Imaging of bacterial infections of the sacroiliac joint].

Infection of the sacroiliac joint can be pyogenic or granulomatous and is usually unilateral. There are a number of predisposing conditions including drug abuse and intra articular steroid injection, but in 44% of cases, no definite predisposing factors can be identified. Considerable delay between presentation and diagnosis is recognized. The clinical picture may be non-specific and variable, and clinical suspicion may be low due to the relatively low incidence of the condition. This is compounded by difficulties in clinical examination of the SUs. The diagnosis is based on a history suggesti

Nonattendance to gastroenterologist follow-up after discharge is associated with a thirty-days re-admission in patients with inflammatory bowel disease: a multicenter study.

BACKGROUND: Inflammatory bowel disease (IBD) is a set of chronic inflammatory diseases associated with significant morbidity and high hospitalization rate. IBD patients are particularly prone to rehospitalization resulting in high medical cost and morbidity. The aim of this study was to assess laboratory and clinical predictors of readmission in patients who were hospitalized with IBD flare. METHODS: A multicenter, retrospective, cross-sectional analysis included IBD patients who were admitted with disease exacerbation from January 1, 2019 to January 1, 2020 in three Israeli university hospit

Practical Clinical Guidelines for Natalizumab Treatment in Patients With Relapsing Multiple Sclerosis.

Natalizumab (TYSABRI®) was the first high-efficacy monoclonal antibody disease-modifying therapy (DMT) approved as a monotherapy for the treatment of adults with relapsing forms of multiple sclerosis (MS), including clinically isolated syndrome, relapsing-remitting MS, and active secondary progressive MS. Because natalizumab is administered by intravenous infusion, infusion nurses play a key role in the care of natalizumab-treated patients. In the 16 years since approval, substantial data have been gathered on the long-term, real-world effectiveness and safety of natalizumab. This article prov

Cervical Myelopathy Presenting as Bilateral Upper Extremity Weakness.

A 57-year-old man with insidious onset of progressive bilateral upper extremity weakness was referred to physical therapy by his primary care physician. Following examination, the physical therapist referred the patient back to his primary care physician with a request for cervical magnetic resonance imaging (MRI) and neurology consultation. Cervical radiographs demonstrated multilevel degenerative changes, while cervical spine MRI revealed compressive myelopathy, significant spinal canal stenosis, and severe spondylosis J Orthop Sports Phys Ther 2017;47(9):691. doi:10.2519/jospt.2017.7287.

Hypertension and chronic kidney disease: the role of lifestyle modification and medication management.

This article explores the relationship between hypertension and chronic kidney disease (CKD), and examines lifestyle modification and medication management. About one in three U.S. adults has high blood pressure, and despite this high occurrence, only one-third of patients are actually treated to a goal blood pressure. Reasons for this vary, but can include ineffective patient teaching, lack of understanding, poor lifestyle modifications, sub-optimal treatment adherence, limited access to health care, or failure of healthcare providers to treat hypertension aggressively. There is a clear relat

Part 2. Osteogenesis imperfecta.

Osteogenesis imperfecta (OI), the most common genetic disorder of bone, is characterized by frequent, unpredictable fractures of the long bones with progressive skeletal deformity. Often diagnosed in the early years of life, OI requires a comprehensive, interdisciplinary plan of care involving patient, family, and community. The nurse's role as caregiver, educator, and patient advocate is to foster normal physical and psychosocial growth and development and minimize the opportunity for fracture.

Amyotrophic lateral sclerosis: improving care with a multidisciplinary approach.

Amyotrophic lateral sclerosis (ALS) is a progressive neurodegenerative disease, leading to death within an average of 2-3 years. A cure is yet to be found, and a single disease-modifying treatment has had a modest effect in slowing disease progression. Specialized multidisciplinary ALS care has been shown to extend survival and improve patients' quality of life, by providing coordinated interprofessional care that seeks to address the complex needs of this patient group. This review examines the nature of specialized multidisciplinary care in ALS and draws on a broad range of evidence that has

The high-risk surgical patient: current management strategies to reduce morbidity and mortality from organ dysfunction in surgical intensive care.

A number of patients undergoing major surgery have an associated high morbidity leading to a long stay in the intensive care unit. This group of patients can be regarded as high risk, as their mortality rate is in the region of 20 to 30%. The high-risk patient is likely to have at least one of the characteristics listed in Table 1. Perioperative cardiac morbidity remains the leading cause of postoperative mortality. Management of cancer cachexia towards optimizing care delivery and patient outcomes.

Cancer cachexia is a complex syndrome characterized by progressive weight loss, muscle mass depletion, and systemic inflammation, profoundly affecting the well-being and treatment outcomes of cancer patients. Effective management of cancer cachexia demands a coordinated, multifaceted approach involving various healthcare disciplines and operational strategies. Streamlining care processes is crucial to ensure timely interventions and support, reducing delays in diagnosis and treatment initiation. Multidisciplinary collaboration is pivotal in creating integrated care plans that address the multi

Facioscapulohumeral muscular dystrophy: revisiting three prevailing assumptions through the perspective of FSHD Europe.

Facioscapulohumeral muscular dystrophy (FSHD) is one of the most common inherited muscular dystrophies and the focus of rapidly expanding therapeutic development. Yet several longstanding assumptions continue to shape discussions among researchers, clinicians, regulators, and industry stakeholders in ways that may not fully reflect the experiences of people living with FSHD. In this Patient Forum, FSHD Europe presents perspectives arising from extensive engagement with the patient community and participation in international initiatives. We address three commonly cited assumptions: that FSHD i

Navigating the complexities of progressive familial intrahepatic cholestasis-II: Genetic insights, patient outcomes, and nursing implications - A case report.

INTRODUCTION: Progressive Familial Intrahepatic Cholestasis Type 2 (PFIC-2) is a rare autosomal recessive liver disorder caused by mutations in the ABCB11 gene encoding the bile salt export pump (BSEP). The condition is characterised by persistent cholestasis, low γ-glutamyl transferase (GGT) levels, severe pruritus, hepatosplenomegaly, and progressive liver dysfunction. Due to its rarity and clinical complexity, early recognition and coordinated multidisciplinary management are essential. CASE PRESENTATION: We report the case of a 1-year-4-month-old girl with genetically confirmed PFIC-2 who

Delirium at the End of Life.

Delirium is a prevalent acute neurocognitive condition in patients with progressive life-limiting illness. Delirium remains underdetected; a systematic approach to screening is essential. Delirium at the end of life requires a comprehensive assessment. Consider the potential for reversibility, illness trajectory, patient preference, and goals of care before proceeding with investigations and interventions. Management should be interdisciplinary, and nonpharmacologic therapy is fundamental. For patients with refractory and severe agitation or perceptual disturbance, judicious use of medication

Anesthetic considerations for renal transplant surgery in patients with mitochondrial myopathy, encephalopathy, lactic acidosis, and stroke-like episodes syndrome: a case report.

Mitochondrial myopathy, encephalopathy, lactic acidosis, and stroke-like episodes syndrome is a progressive syndrome with variable involvement of multiple-organ systems. These patients require special consideration for preoperative optimization, intraoperative management, and postoperative care. The medical literature regarding perioperative management of these patients relies heavily on case reports. Here we present a novel experience providing care for a patient with mitochondrial myopathy, encephalopathy, lactic acidosis, and stroke-like episodes syndrome who underwent renal transplantation
