Prostate Cancer Mortality in Men Aged 70 Years Who Recently Underwent Prostate-Specific Antigen Screening.

IMPORTANCE: Continuing prostate-specific antigen (PSA) screening after age 70 years might benefit men at high risk of prostate cancer-specific mortality (PCSM) or metastatic prostate cancer (mPCa), but the relative value of clinical factors (race and ethnicity, competing mortality, and PSA history) in identifying men at higher vs lower risk is unknown. OBJECTIVE: To examine the value of PSA levels, race and ethnicity, and competing mortality in risk stratification for PCSM and mPCa in men after age 70 years. DESIGN, SETTING, AND PARTICIPANTS: In this cohort study, clinical data of all men re

Stage IV Prostate Cancer Presenting With a Low Prostate-Specific Antigen Level: A Case Report.

Prostate cancer is one of the most common cancers diagnosed in men and is typically monitored through prostate-specific antigen (PSA) levels. Metastatic prostate cancer usually presents with elevated PSA levels. However, a minority of these patients have low to minimal elevation. Here, we describe the case of a 70-year-old male with a history of essential hypertension and hypertriglyceridemia who presented to the emergency department after a fall, which ultimately revealed lytic spinal lesions on computed tomography. The lesions raised the concern for possible malignancy, possible osteoporotic

Sparse-view reconstruction and noise reduction in 99m Tc-prostate specific membrane antigen prostate imaging.

PURPOSE: This preliminary study evaluates the feasibility and clinical value of sparse-view acquisition in 99m Tc-prostate specific membrane antigen (PSMA) single photon emission computed tomography (SPECT) prostate imaging. This study also compares the performance of traditional Gaussian filtering vs. an edge-preserving nonlocal means (NLM) filter in the sparse-view SPECT approach. METHODS: Ten patients with biopsy-proven prostate cancer (Gleason 4-10) underwent full-angle acquisitions (60 views), which were decimated to 30, 15, and 10 views. Images were reconstructed with ordered-subsets ex

Prostate cancer risk-associated single-nucleotide polymorphisms impact the conformational dynamics of prostate-specific antigen.

BACKGROUND: Two non-synonymous single-nucleotide polymorphisms (SNPs) rs61752561 (D95N substitution) and rs17632542 (I163T substitution) in the KLK3 gene encoding prostate-specific antigen (PSA), a chymotrypsin-like serine protease, are associated with prostate cancer risk and have been shown to reduce the activity of PSA. However, the structural impact of these SNPs on PSA, which may underlie the observed risk associations and functional alterations, has not been fully explored. RESULTS: Computational modelling predicted that the variants D95N and I163T do not cause drastic structural change

Targeted Therapy for Prostate Cancer by Prostate-Specific Membrane Antigen-Targeted Small-Molecule Drug Conjugates.

In the aging global population, prostate cancer is a worldwide health problem because the incidence rate of this disease increases at advanced ages. Although early-stage prostate cancer can be treated by total prostatectomy, the surgery causes side effects, such as incontinence and dysuria, that lower QOL. Once the disease progresses to metastatic castration-resistant prostate cancer (mCRPC), there are no effective chemotherapeutic agents without systematic side effects. Therefore, targeted therapies for mCPRC are urgently needed. Traditional antibody-drug conjugate treatments for prostate can

[Research progress on prostate-specific membrane antigen ligand positron emission tomography imaging of prostate cancer].

Prostate cancer is the most common malignant tumor in male urinary system, and the morbidity and mortality rate are increasing year by year. Traditional imaging examinations have some limitations in the diagnosis of prostate cancer, and the advent of molecular imaging probes and imaging technology have provided new ideas for the integration of diagnosis and treatment of prostate cancer. In recent years, prostate-specific membrane antigen (PSMA) has attracted much attention as a target for imaging and treatment of prostate cancer. PSMA ligand positron emission tomography (PET) has important ref

68Ga-Prostate-Specific Membrane Antigen PET/CT Findings in Extra-axial Metastasis From Prostate Carcinoma.

A 67-year-old man with no previous history of malignancy presented with trigeminal neuralgia. MRI of the brain revealed extra-axial lesion along right trigeminal nerve suggestive of neuroma. On histopathology, the lesion was found to be metastasis from adenocarcinoma prostate. Patient underwent Ga-prostate-specific membrane antigen PET/CT for assessing the whole-body status of the disease, which revealed primary prostate lesion, metastatic bone lesions, and extra-axial lesion along the right trigeminal nerve. Extra-axial, intracranial metastasis from prostate carcinoma is an uncommon phenomeno

Prostate-specific Membrane Antigen PET: Clinical Utility in Prostate Cancer, Normal Patterns, Pearls, and Pitfalls.

Prostate-specific membrane antigen (PSMA) is a transmembrane glycoprotein that is overexpressed in prostate cancer. Radiolabeled small molecules that bind with high affinity to its active extracellular center have emerged as a potential new diagnostic standard of reference for prostate cancer, resulting in images with extraordinary tumor-to-background contrast. Currently, gallium 68 (68Ga)-PSMA-11 (or HBED-PSMA) is the most widely used radiotracer for PSMA positron emission tomography (PET)/computed tomography (CT) or PSMA PET/magnetic resonance (MR) imaging. Evolving evidence demonstrates sup

68Ga-Prostate-Specific Membrane Antigen Expression in Neurocysticercosis Lesions in a Patient With Prostate Carcinoma.

Ga-prostate-specific membrane antigen (PSMA) ligand is a well-known tracer used in prostate cancer. Nevertheless, there are reports of unusual Ga-PSMA uptake in variety of benign lesions. We report a case of prostate cancer in which Ga-PSMA PET/CT revealed tracer avidity in the enhancing walls of cystic lesions in the brain consistent with the features of neurocysticercosis. The uptake of Ga-PSMA in neurocysticercosis is not known and may be attributed to angiogenesis occurring at the periphery of these lesions.

Serum insulin level, disease stage, prostate specific antigen (PSA) and Gleason score in prostate cancer.

In the present study, we assessed the relationship of serum insulin levels and three surrogate markers of recurrence, T stage, PSA, and Gleason score, in men with localized prostate cancer. Participants in our study were found through urology and radiation oncology clinics, and all eligible patients were asked to take part. All patients were asymptomatic and had been initially diagnosed on the basis of rising PSA or abnormal physical examination. Histological confirmation of diagnosis was obtained for all subjects. Serum insulin levels were determined by chemoluminescent assay with a standard,

Screening for prostate cancer with the prostate-specific antigen test: are patients making informed decisions?

BACKGROUND: The benefits of early detection of prostate cancer are uncertain, and the American College of Physicians and the American Academy of Family Physicians recommend individual decision making in prostate cancer screening. This study reports the knowledge of male primary care patients about prostate cancer and prostate-specific antigen (PSA) testing and examines how that knowledge is related to PSA testing, preferences for testing in the future, and desire for involvement in physician-patient decision making. METHODS: The sample included 160 men aged 45 to 70 years with no history of p

Significance of free prostate-specific antigen and gamma-seminoprotein in the screening of prostate cancer.

BACKGROUND: Since free prostate-specific antigen (PSA) and gamma-seminoprotein (gamma-SM) recognize similar epitope(s) of PSA, the significance of serum-free PSA and gamma-SM in the early detection of prostate cancer was compared. METHODS: A prospective clinical trial was conducted on 701 male volunteers, age 50 years or older. Free PSA (Tandem-R free PSA, Hybritech) and gamma-SM (gamma-SM, Chugai) levels were determined, and biopsies were performed if the PSA (Tandem-R, Hybritech) level was > 4 ng/ml, or if digital rectal examination (DRE) was suspicious.
