Standard therapy for abdominal aortic graft infection involving resection of the graft and extraanatomic bypass carries significant morbidity and mortality rates. The present study evaluates the feasibility of combining in situ graft replacement with a highly vascular tissue flap as an alternative to this problem. We report a case of primary salmonella infection of a dacron aortofemoral bypass graft. Local graft excision with extra-anatomic bypass resulted in a successful outcome. Salmonella, whilst the commonest cause of infected abdominal aortic aneurysms, is currently a rare cause of prosthetic graft infection. However, given the increasing prevalence of salmonellosis in the community, we believe this situation will change and surgeons should be alert to this new and dangerous addition to the list of graft pathogens.

Guillain-Barré syndrome (GBS) is an acute inflammatory demyelinating polyneuropathy associated with progressive limb weakness and areflexia. Up to now, a few cases of GBS following infective endocarditis (IE), particularly prosthetic valve endocarditis, have been reported. The isolation of a Leptotrichia buccalis-like bacterium from the blood of a patient with endocarditis associated with a prosthetic aortic valve is reported. The organism was isolated from different blood cultures. It was similar but not completely identical to Leptotrichia buccalis. We present a patient with gout and a total knee arthroplasty that failed secondary to infection. The English literature records only 5 other cases of gouty attacks after total knee arthroplasty. Symptoms, presentation, and laboratory tests do not definitively differentiate between infection and gout, but crystals in the joint aspirate are diagnostic. This case stresses the importance of recognizing crystalline arthropathy as a source of a painful knee and as a complicating factor during revision surgery.

Sixty-seven cases of total arthroplasty of the hip in the presence of sepsis performed between 1974 and 1986 were reviewed. These prostheses were inserted in 57 cases to replace septic prostheses. Forty-six were infected total hip prostheses and 11 were infected head and neck replacement femoral prostheses.
