Most meta-diaphyseal femoral fractures that are treated with intramedullary nailing can be reduced satisfactorily by skeletal traction without 'opening' the fracture site and therefore, complications such as nonunion, infection and wound healing problems are reduced. In cases where adequate fracture reduction cannot be achieved by skeletal traction, 'reduction aids' have been used during the operative procedure in order to avoid the exposure of the fracture site. The 'blocking' screw, as a reduction tool, was proposed initially for the 'difficult' metaphyseal fractures of the tibia. Subsequently

Osteopetrosis is a rare, inherited disease characterized by defects in osteoclastic function that results in defective bone resorption. When fractures are encountered, fixation is extremely difficult. Osteopetrosis patients have an increased predisposition to infection. If infection develops after a fracture, treatment is more difficult. In this paper, treatment is presented of a 49-year old female to whom proximal femoral nailing was applied for fixation of an osteopetrotic proximal femur fracture; and when it was unsuccessful, revision was made with a locked anatomic plate, which subsequentl

This article describes a case of backout of the helical blade, a rare complication of proximal femoral nail antirotation. A 31-year-old man had sustained a trochanteric fracture of his right femur. Fracture fixation using proximal femoral nail antirotation and autologous bone grafting 7 months later were performed at another hospital. However, bony union was not obtained, and the patient's pain and limp persisted. Therefore, he presented to the current authors. A radiograph taken at presentation revealed backout of the helical blade and fracture nonunion. A radiograph taken 1 month later showe

A clinical, radiological and histopathological study of femoral heads from 125 patients with fracture of the neck of the femur and from 30 cadavers was carried out to identify various risk factors. The findings showed that the Singh index was unreliable as a radiological indicator of the bone content of the femoral heads; that the bone content of the femoral head in patients sustaining a fracture of the femoral neck did not differ from that of the controls; that osteomalacia was not found in any of the heads examined; and that the distribution of trabecular microfractures did not support the h

BACKGROUND: Despite the longstanding awareness of the increasing incidence and consequences of periprosthetic proximal femoral fractures (PPFFs), and the rationale protective role of the preserved bone stock, no method for its evaluation, with the potential for routine clinical application, has been available. A novel method for the evaluation of preserved proximal femoral bone stock volume (V PF) in conventional primary total hip arthroplasty (THA) on routinely available hip radiographs was introduced and compared with clinical data.
METHODS: Study was designed according to the standard prot

Introduction: Currently, cannulated screws (CSs) and dynamic hip screws (DHSs) are widely used for the treatment of femoral neck fractures, but the postoperative complications associated with these internal fixations remain high. In response to this challenge, our team proposes a new approach involving triangular-supported fixation and the development of the proximal femoral bionic nail (PFBN). The primary objective of this study is to investigate the biomechanical differences among CSs, DHSs, and the PFBN in their capacity to stabilize femoral neck fractures. Methods: A normal proximal femur

BACKGROUND: Combined ipsilateral femoral neck and shaft fractures are an uncommon type of fractures. A number of different implant options are available for the management of this injury. Two-device procedures were suggested because of the higher rate of malunion by single-device treatment. However, surgical treatment using a cephalomedullary nail is still an alternative option that provides better mechanical advantage and minimal invasion. This study details the technique of treating these pattern fractures with proximal femoral nail anti-rotation II (PFNA-II) to achieve an acceptable reducti

INTRODUCTION: Proximal femoral osteotomy (PFO) with locking compression plates (LCPs) is effective for treating developmental dysplasia of the hip (DDH) in children. However, plate insertion or removal has a risk of postoperative femoral fractures. The goal of this study was to identify and analyse the association between postoperative femoral fractures after PFO and potential risk factors for these fractures.
METHODS: Six patients with DDH and postoperative femoral fractures were retrospectively reviewed at our hospital from June 2014 to October 2020. There were 2 boys and 4 girls whose ages

PURPOSE: Due to the demographic change towards an older society, osteoporosis-related proximal femur fractures are steadily increasing. Intramedullary nail osteosyntheses are available in different lengths, where the field of application overlaps. The aim of this study was to investigate whether subtrochanteric fractures can also be treated stably using a short femoral intramedullary nail in cadaveric bones.
METHODS: A short PFNA and a long PFNA were implanted in both seven artificial bones and osteoporotic human specimens. A standardized AO 31-A3 (reverse-oblique) fracture was placed in the

CASE: A 13-year-old male patient sustained a posterior transphyseal hip fracture-dislocation while playing football. A surgical hip dislocation with a trochanteric flip osteotomy, as described by Ganz and colleagues, was performed. Drilling of the dislocated epiphysis demonstrated no blood flow. Transepiphyseal and transphyseal fracture fixation with screws was performed. Two years after the injury, the patient had no evidence of osteonecrosis or functional limitations.
CONCLUSION: Traumatic transepiphyseal fracture-dislocations are rare injuries that commonly result in osteonecrosis. Applica

INTRODUCTION: Proximal femur fractures are associated with an increased mortality rate in the elderly. Early weight-bearing presents as a modifiable factor that may reduce negative postoperative outcomes and complications. As such, we aimed to compare non-weight-bearing, partial-weight-bearing and full weight-bearing cohorts, in terms of risk factors and postoperative outcomes and complications.
METHODS: We retrospectively reviewed our database to identify the three cohorts based on the postoperative weight-bearing status the day of surgery from 2003 to 20014. We collected data on numerous ri
