Infantile Nystagmus and Abnormalities of Conjugate Eye Movements in Down Syndrome.

PURPOSE: Subjects with Down syndrome (DS) have an anatomical defect within the cerebellum that may impact downstream oculomotor areas. This study characterized gaze holding and gains for smooth pursuit, saccades, and optokinetic nystagmus (OKN) in DS children with infantile nystagmus (IN). METHODS: Clinical data of 18 DS children with IN were reviewed retrospectively. Subjects with constant strabismus were excluded to remove any contribution of latent nystagmus. Gaze-holding, horizontal and vertical saccades to target steps, horizontal smooth pursuit of drifting targets, OKN in response to ve

Neuro-Ophthalmological Findings in Children and Adolescents with Chronic Ataxia.

Chronic ataxia is a challenging problem in paediatric neurology. It is caused by a multitude of disorders that at least initially have similar or non-specific phenotype. Some of these disorders have associated neuro-ophthalmological signs (N-OS). The aims of this study are to describe the N-OS and their frequencies in general and by disease aetiology in paediatric patients with chronic ataxia. The authors identified 184 patients under age 17 years with chronic ataxia (>2 months duration or recurrent) during 1991-2008 from multiple sources. Diagnoses and N-OS were ascertained following charts r

Crouzon Syndrome: Relationship of Eye Movements to Pattern Strabismus.

PURPOSE: To characterize conjugate eye movements in Crouzon syndrome (CS) patients with and without strabismus. METHODS: Smooth pursuit, saccades, horizontal optokinetic nystagmus (OKN), and horizontal vestibulo-ocular reflex (VOR) were recorded using binocular video-oculography (VOG) in 10 children with CS (5 orthotropic, 5 strabismic) and 12 age-matched controls. Hess-Lancaster plots were generated from Orbit 1.8 using rectus muscle pulley locations from computed tomography imaging. Two-dimensional eye scan paths from VOG recordings were compared with the Hess-Lancaster plots. RESULTS: Tar

[Quantitative ocular-motor tests analysis in multiple sclerosis].

UNLABELLED: Abnormalities in the ocular-motor reflex recorded in electronystagmography may indicate central vestibular system impairment. Multiple sclerosis (MS) is the most common chronic, debilitating disease characterized by focal demyelinization that develops throughout the central nerves system at varying time. The aim of the study was to apply the quantitative analysis of ocular-motor tests to evaluation degree of the central system impairment in MS patients. MATERIAL AND METHODS: The study was carried on 60 MS patients consulted in Balance Division, Otolaryngology Clinic, Medical Unive

[Cerebellar Control of Ocular Movements: Application to the Topographical Diagnosis of Cerebellar Lesions].

Over the last decade, substantial information on cerebellar oculomotor control has been provided by the use of sophisticated neuroanatomical, neurophysiological, and imaging techniques. We now know that an intact cerebellum is a prerequisite for normal oculomotor performance. This review clarifies the current knowledge on structure-function correlations of the cerebellum in relation to ocular movements and allows them to be applied to topographical diagnosis of cerebellar lesions. The cerebellar regions most closely related to oculomotor function are: (1) the flocculus/paraflocculus for VOR su

Regional microstructural damage and patterns of eye movement impairment: a DTI and video-oculography study in neurodegenerative parkinsonian syndromes.

Characteristic alterations of eye movement control are a common feature of neurodegenerative parkinsonism, including Parkinson's disease (PD), progressive supranuclear palsy (PSP), and multiple system atrophy (MSA). Regional microstructural alterations as assessed by diffusion tensor imaging (DTI) have been reported in PD, PSP, and MSA. Therefore, we investigated the specific association between eye movement disturbances and microstructural impairment in these diseases. Video-oculographic recordings of smooth pursuit and visually guided reactive saccades as well as fractional anisotropy (FA) m

Abnormalities of the oculomotor function in type 1 diabetes and diabetic neuropathy.

AIMS: Abnormalities in the oculomotor system may represent an early sign of diabetic neuropathy and are currently poorly studied. We designed an eye-tracking-based test to evaluate oculomotor function in patients with type 1 diabetes. METHODS: We used the SRLab-Tobii TX300 Eye tracker®, an eye-tracking device, coupled with software that we developed to test abnormalities in the oculomotor system. The software consists of a series of eye-tracking tasks divided into 4 classes of parameters (Resistance, Wideness, Pursuit and Velocity) to evaluate both smooth and saccadic movement in different di

Eye Tracking to Assess the Functional Consequences of Vision Impairment: A Systematic Review.

BACKGROUND: Eye tracking is a promising method for objectively assessing functional visual capabilities, but its suitability remains unclear when assessing the vision of people with vision impairment. In particular, accurate eye tracking typically relies on a stable and reliable image of the pupil and cornea, which may be compromised by abnormalities associated with vision impairment (e.g., nystagmus, aniridia). OBJECTIVES: This study aimed to establish the degree to which video-based eye tracking can be used to assess visual function in the presence of vision impairment. DATA SOURCES: A sys

How Do I Examine for a Supranuclear Gaze Palsy?

Classically, a supranuclear gaze palsy (SNGP) is a conjugate gaze limitation that can be overcome (i.e., corrected) by the vestibulo-ocular reflex (VOR). It involves vertical eye movement most commonly. In practice, SNGP can be encountered as a limitation in excursion of self-generated conjugate saccades (fast eye movements) that can be improved or overcome with verbal command, visual targets, or VOR. Thus, mild SNGPs may be overcome with verbal commands (e.g., "look down") or visual targets (e.g., "look down at my hand"), whereas moderate SNGPs may be overcome by smooth pursuit mechanisms and

Eye movement dysfunctions in psychiatric patients: a review.

Impairments of smooth pursuit eye movements occur in a high proportion of schizophrenic patients and in a lower but significant percentage of patients with affective psychoses, as well as patients with structural and metabolic disorders of the central nervous system. These findings have been confirmed using a wide range of tracking tasks, recording techniques, and scoring procedures, and therefore cannot be attributed to measurement artifact. The eye movement disruption in schizophrenics does not seem to result from drug treatment or simple inattention. Eye tracking pattern appears to be under

Eye movements in patients with Wallenberg's syndrome.

We studied electro-oculographic and videotape recordings of eye movements in six patients with Wallenberg's syndrome. With fixation, all patients had a spontaneous rotatory nystagmus with the fast phase directed toward the intact side. With loss of fixation, the patients' eyes deviated tonically toward the side of the lesion. Voluntary and involuntary saccades had larger amplitude when directed toward the side of the lesion than away from it. The spontaneous nystagmus predictably interacted with all slow eye movements, producing asymmetric smooth-pursuit, optokinetic, and vestibular responses.

The eyes have it: Alcohol-induced eye movement impairment and perceived impairment in older adults with and without alcohol use disorder.

BACKGROUND: While alcohol has been shown to impair eye movements in young adults, little is known about alcohol-induced oculomotor impairment in older adults with longer histories of alcohol use. Here, we examined whether older adults with chronic alcohol use disorder (AUD) exhibit more acute tolerance than age-matched light drinkers (LD), evidenced by less alcohol-induced oculomotor impairment and perceived impairment. METHOD: Two random-order, double-blinded laboratory sessions with administration of alcohol (0.8 g/kg) or placebo. Participants (n = 117; 55 AUD, 62 LD) were 40-65 years of ag

Eye Movement Abnormalities in Amyotrophic Lateral Sclerosis.

It is generally believed that eye movements are completely spared in amyotrophic lateral sclerosis (ALS). Although a series of eye movement abnormalities has been recognized in recent years, the findings are highly controversial, and bulbar disabilities should be considered in relation to eye movement abnormalities. The present study aimed to determine whether eye movement abnormalities are present in ALS and, if so, to investigate their characteristics and their association with bulbar disability in ALS patients. A total of 60 patients and 30 controls were recruited and underwent the standard

Central nervous system effects of haloperidol on THC in healthy male volunteers.

In this study, the hypothesis that haloperidol would lead to an amelioration of Δ9-tetrahydrocannabinol (THC)-induced 'psychotomimetic' effects was investigated. In a double-blind, placebo-controlled, partial three-way crossover ascending dose study the effects of THC, haloperidol and their combination were investigated in 35 healthy, male mild cannabis users, measuring Positive and Negative Syndrome Scale, Visual Analogue Scales for alertness, mood, calmness and psychedelic effects, saccadic and smooth pursuit eye measurements, electroencephalography, Body Sway, Stroop test, Visual and Verbal

Selective, circuit-wide sparing of floccular connections in hereditary olivopontine cerebellar atrophy with slow saccades.

We present a systems-oriented histopathologic analysis of the ocular motor control circuits in the cerebellum and brainstem from a patient with a hereditary form of olivopontine cerebellar atrophy of the Wadia type, which has a characteristic ocular motor presentation of slow saccades but relative preservation of smooth pursuit and gaze-holding. This differential pattern of clinical involvement is associated with a lobule-specific pattern of cerebellar degeneration. We asked whether these patterns of sparing and degeneration were consistent throughout the associated deep cerebellar and brainst

Ultralow vestibuloocular reflex time constants.

We report detailed oculomotor studies in 3 patients with central nervous system lesions and markedly decreased time constants (less than 2 seconds) of the vestibuloocular reflex (VOR). In 1 patient with Chiari type I malformation, serial measurements over 3 years documented a progressive decrease in the duration of postrotatory nystagmus (100 deg/sec steps, acceleration 140 deg/sec2) until finally there was no sustained nystagmus. At this time, the patient had no response to caloric stimulation or to sinusoidal rotation below 0.2 Hz but normal gain (peak slow-phase eye velocity/peak chair velo

Eye movement abnormalities in Alzheimer's disease and other neurodegenerative dementias: insights from current evidence and priorities for future research.

Eye movement abnormalities are increasingly recognized as early and sensitive markers of neurodegenerative dementias, particularly Alzheimer's disease (AD). Disruptions in saccadic, antisaccadic, smooth pursuit, fixation, and naturalistic eye movement tasks reflect dysfunction in frontal, parietal, subcortical, and cerebellar circuits that are vulnerable to neurodegeneration. Studies have consistently demonstrated that AD patients show prolonged saccadic latencies, increased antisaccade error rates, reduced smooth pursuit gain, and fixation instability. Such deficits correlate with cognitive i
