History of symptoms consistent with REM sleep behavior disorder in a population with Parkinson's Disease.

REM Sleep Behavioral Disorder (RBD) is a parasomnia marked by the maintenance of muscle tone during REM sleep. Evidence has placed RBD as one of the possible prodromal stages of Parkinson's Disease (PD), but data on the proportion of people with PD who have had symptoms of RBD are limited. This study aimed to investigate the history of symptoms compatible with RBD in a population with PD. The sample was composed by 73 patients with clinically diagnosed PD being followed up at a reference outpatient setting, compared to 73 age- and sex-matched individuals with no PD. The evaluation of symptoms

The non-coding GBA1 rs3115534 variant is associated with REM sleep behavior disorder in Nigerians.

BACKGROUND: Damaging coding variants in GBA1 are a genetic risk factor for rapid eye movement sleep behavior disorder (RBD), which is a known early feature of synucleinopathies. Recently, a population-specific non-coding variant (rs3115534) was found to be associated with PD risk and earlier disease onset in individuals of African ancestry. OBJECTIVES: To investigate whether the GBA1 rs3115534 PD risk variant is associated with RBD. METHODS: We studied 709 persons with PD and 776 neurologically healthy controls from Nigeria. The GBA1 rs3115534 risk variant status was imputed from previous ge

Sweet or Bland Dreams? Taste Loss in Isolated REM-Sleep Behavior Disorder and Parkinson's Disease.

BACKGROUND: Hyposmia and isolated REM sleep behavior disorder are well-established features of prodromal Parkinson's disease (PD). OBJECTIVES: The objective of the present study was to evaluate whether taste loss (reported in PD and possibly suggesting brain stem involvement) is present at the isolated REM sleep behavior disorder stage. METHODS: We assessed taste function using the Taste Strip Test (evaluating 4 concentrations of bitter, sweet, sour, and salty) in 44 participants with isolated REM sleep behavior disorder, 19 with PD, and 29 controls. All participants underwent video-polysomn

Increased muscle sympathetic nerve activity and impaired baroreflex control in isolated REM-sleep behavior disorder.

OBJECTIVE: Changes in baroreflex sensitivity have been reported in patients with idiopathic Parkinson's disease (PD). We sought to investigate the hypothesis that patients with isolated rapid eye movement (REM)-sleep behavior disorder (iRBD), known to be a prodromal stage for PD, will show abnormalities in baroreflex control. METHODS: Ten iRBD patients were compared to 10 sex- and age-matched healthy controls. Their cardiovascular parameters and muscle sympathetic nerve activity (MSNA) were evaluated at rest and during baroreflex stimulation. RESULTS: MSNA at rest was higher in iRBD patients

Clonazepam for probable REM sleep behavior disorder in Parkinson's disease: A randomized placebo-controlled trial.

BACKGROUND: Clonazepam is considered to be a first-line treatment for rapid eye movement sleep-related behavior disorder (RBD) in Parkinson's disease (PD). The purpose of this study was to determine the short-term efficacy and safety of clonazepam for the treatment of probable RBD (pRBD) in patients with PD. METHODS: We conducted a four-week, randomized, double-blind, placebo-controlled trial of clonazepam (0.5 mg/day at bedtime) compared to a placebo for RBD symptoms in patients with PD. Patients aged 30 years or older who had a caregiver that could observe RBD symptoms were recruited betwee

[REM-sleep behavior disorder and sleepwalking in a patient with Parkinson's disease and essential tremor].

REM-sleep behavior disorder (RBD), a form of parasomnia, is characterized by motor and/or speech activity during sleep corresponding to the dream content. RBD is currently considered as a marker of onset of neurodegenerative diseases (sinucleinopathies) and as a most specific early symptom of Parkinson's disease (PD). The authors describe a clinical case of a patient with the combination of essential tremor, initial signs of PD and RBD. A polysomnographic study has confirmed clinical signs of RBD and determined the disturbances of sleep architecture: the increase in falling asleep, number of a

Relevance of substantia nigra hyperechogenicity and reduced odor identification in idiopathic REM sleep behavior disorder.

BACKGROUND: Substantia nigra (SN) hyperechogenicity determined by transcranial sonography (TCS) and olfactory dysfunction are common findings in Parkinson disease (PD), which may reveal a prodromal synucleinopathy in idiopathic REM sleep behavior disorder (iRBD). METHODS: TCS and the Odor Stick Identification Test for Japanese (OSIT-J) were performed in 34 consecutive patients with iRBD (67.9+/-6.1years), 17 consecutive patients with PD (66.4+/-6.7years), and 21 control group subjects (64.4+/-5.8years). RESULTS: There was a significantly increased area of echogenicity in the SN in the iRBD g

Exercise Intervention for REM Sleep Behavior Disorder in Parkinson's Disease: Mechanisms and Implications for Neurorehabilitation.

BACKGROUNDS: REM sleep behavior disorder (RBD) is a prodromal non-motor symptom of Parkinson's disease (PD) and other α-synucleinopathies, affecting approximately 30%-60% of PD patients. It is closely associated with α-synuclein (α-syn) aggregation, with its primary pathophysiology involving the impairment of brainstem circuits, resulting in movement disorders and circadian rhythm dysfunction. Although exercise is recognized for improving motor symptoms and delaying neurodegenerative disease progression, a comprehensive framework of its underlying mechanisms for RBD symptoms has not yet been t

Altered topological properties in PD patients with probable REM sleep behavior disorder through fuctional MRI.

PURPOSE: Rapid eye movement (REM) behavior disorder (RBD) is a common non-motor symptom in Parkinson’s disease (PD) patients. However, its pathogenesis remains unclear. Our study aimed to investigate the topological properties of the functional brain network in PD patients with probable REM sleep behavior disorder (PD-pRBD). METHOD: Using the RBD screening questionnaire, 32 PD patients without RBD (PD-nRBD) and 17 PD-pRBD patients were included. All participants underwent resting-state functional Magnetic Resonance Imaging (rs-fMRI) scan and clinical assessment. We compared the differences bet

Characterization of patients with isolated REM sleep behavior disorder and cerebrospinal fluid α-synuclein negativity.

Isolated REM sleep behavior disorder (IRBD) represents the prodromal phase of synucleinopathies. We investigated the characteristics of IRBD patients who tested negative for misfolded α-synuclein (αsyn) in CSF. CSF αsyn was analyzed by seed amplification assay (SAA) in 191 consecutive patients. Using logistic regression, demographic and clinical variables were compared between subjects with negative (αsyn-) and positive (αsyn+) αsyn. αsyn- occurred in 47 (24.61%) patients who were younger (p = 0.018), had less electromyographic REM sleep activity (p < 0.001), and more frequent normal DAT-SPECT

Evolution of Motor and Nonmotor Characteristics in an Idiopathic/Isolated REM Sleep Behavior Disorder Cohort.

BACKGROUND AND OBJECTIVES: Years before diagnosis of Parkinson disease (PD), dementia with Lewy bodies (DLB), or multiple system atrophy (MSA), mild prodromal manifestations can be detected. Longitudinal follow-up of people with prodromal synucleinopathy, particularly idiopathic/isolated REM sleep behavior disorder (iRBD), enables in-depth clinical phenotyping of early disease, which could facilitate stratification for clinical trials, provide the definition of appropriate end points, or predict phenoconversion more precisely. The aim of this study was to update and expand on previous studies

Enrichment of gut-derived metabolites in a Parkinson's disease subtype with REM sleep behavior disorder.

Recent reports indicate that Parkinson's disease (PD)-like changes in gut microbial signatures develop in idiopathic REM sleep behavior disorder (iRBD), a prodrome of α-synucleinopathies. However, <50% of PD cases exhibit RBD at onset, underscoring PD's heterogeneity. Using untargeted metabolomics, plasma samples from PD patients with and without RBD, iRBD subjects, and healthy controls were analyzed to characterize the metabolic differences in PD patients with and without RBD. Metabolomic analysis revealed the enrichment of gut microbial-origin metabolites, such as secondary bile acids and p-

Frequency and longitudinal course of autonomic reflex testing abnormalities in isolated REM sleep behavior disorder.

STUDY OBJECTIVES: Autonomic dysfunction is common across the α-synucleinopathies including isolated RBD (iRBD); however, the presence, severity, and distribution of autonomic dysfunction as a risk factor for phenoconversion in iRBD remains unclear. We aimed to characterize autonomic reflex testing (ART) abnormalities in a cohort of participants with iRBD and assess them as phenoconversion biomarkers. METHODS: We performed ART on 45 individuals with iRBD and evaluated the ability of ART components (sympathetic cholinergic, cardiovagal, and sympathetic adrenergic) to predict phenoconversion usi

Actigraphy-based detection of isolated REM sleep behavior disorder: multicenter validation across devices and populations.

Scalable home-based detection of isolated REM sleep behavior disorder (iRBD) is essential for early care, prevention trials, and identifying candidates for neuroprotective interventions against synucleinopathies. We previously showed that high-resolution wrist actigraphy (Axivity AX6) could identify iRBD based on abnormal sleep (AUC of 0.916) and rest-activity-rhythms (RAR, AUC of 0.856) using machine learning. Here, we aimed to assess generalizability across: (1) other actigraphs using lower resolutions; (2) different populations. We tested the analysis pipeline directly in cohorts from the I

CSF markers of neurodegeneration Alzheimer's and Lewy body pathology in isolated REM sleep behavior disorder.

We investigated the biomarker profile of neurodegeneration, Alzheimer's and Lewy body pathology in the CSF of 148 polysomnography-confirmed patients with isolated REM sleep behavior disorder (IRBD), a condition that precedes Parkinson's disease (PD) and dementia with Lewy bodies (DLB). We assessed misfolded α-synuclein (AS) by RT-QuIC assay, amyloid-beta peptides (Aβ42 and Aβ40), phosphorylated tau (p-tau), and total tau (t-tau) by CLEIA and neurofilament light chain (NfL) by ELISA. We detected AS in 75.3% of patients, pathologically decreased Aβ42/Aβ40 ratio in 22.5%, increased p-tau in 15.5%

Cuneus atrophy and Parkinsonian phenoconversion in cognitively unimpaired patients with isolated REM sleep behavior disorder.

Isolated rapid-eye-movement sleep behavior disorder (iRBD) is a strong predictor of Parkinson's disease and Dementia with Lewy bodies. Previous studies indicate that cortical atrophy in iRBD patients may be linked to cognitive impairment, but the pattern of atrophy is inconsistently reported. This study aimed to elucidate cortical atrophy patterns in a cognitively unimpaired iRBD cohort, focusing on regions associated with cognitive functions, particularly the cuneus/precuneus, and evaluated the predictive value for future phenoconversion. We conducted voxel-based morphometry and region of int

Constipation is linked to accelerated cognitive and motor decline in isolated REM sleep behavior disorder.

INTRODUCTION: Constipation is associated with higher clinical severity and predicts cognitive decline in Parkinson's disease (PD). Whether the non-motor marker is associated with unfavorable motor and cognitive trajectories from the prodromal stage remains unclear. METHODS: In a longitudinal prospective cohort of subjects with isolated REM sleep behavior disorders (iRBD), subjects underwent repeated MDS-UPDRS and Mini-Mental Status Examination (MMSE) assessments. Generalized-estimating-equations (GEE) regression model was used to compare the time-dependent trajectories of MDS-UPDRS-III and MM
