Reirradiation for recurrent head and neck carcinoma using high-dose-rate brachytherapy: A multi-institutional study.

AIM: This study presents multi-institutional individual data of reirradiation (ReRT) for head and neck cancer using brachytherapy (ReRT-BT) collected by national surveillance in Japan. METHODS AND MATERIALS: We distributed an e-mail-based questionnaire to 153 institutions equipped with high-dose-rate (HDR) brachytherapy facilities and received responses from 76 institutions (49.7%). Of these 76 institutions, only four (5.2%) performed ReRT-BT for head and neck cancers, and three provided individual patient's data. RESULTS: Six ReRT-BT cases of patients with recurrent head and neck cancer, tr

Proton therapy for thoracic malignancies: a review of oncologic outcomes.

Introduction: Radiotherapy is an integral component in the treatment of the majority of thoracic malignancies. By taking advantage of the steep dose fall-off characteristic of protons combined with modern optimization and delivery techniques, proton beam therapy (PBT) has emerged as a potential tool to improve oncologic outcomes while reducing toxicities from treatment.Areas covered: We review the physical properties and treatment techniques that form the basis of PBT as applicable for thoracic malignancies, including a brief discussion on the recent advances that show promise to enhance treat

Reirradiation With Stereotactic Radiosurgery After Local or Marginal Recurrence of Brain Metastases From Previous Radiosurgery.

PURPOSE: Brain metastases represent a major indication for stereotactic radiosurgery (SRS), but further study is needed regarding repeat SRS (SRS2) after local or marginal recurrence after prior SRS (SRS1). We report local tumor control (LC) after SRS2 and identify predictors of radiation necrosis (RN) and symptomatic RN (SRN). METHODS AND MATERIALS: Patients had biopsy-proven non-small cell lung cancer and at least 1 brain metastasis previously treated with SRS. SRS2 was performed from 2015 to 2020 and required overlap of the prescription isodose lines with those from SRS1. Patients treated

Real-World Pattern of Care Analysis of Liver Reirradiation Among European Organisation for Research and Treatment of Cancer Members.

BACKGROUND AND PURPOSE: Liver re-irradiation is an increasingly used treatment for hepatocellular carcinoma (HCC), liver metastases, and intrahepatic cholangiocarcinoma (ICCA). Technological advances now enable precise delivery with improved organ sparing, yet practice remains heterogeneous, and evidence is scarce. This study evaluated the real-world pattern of care of liver re-irradiation among EORTC (European Organisation for Research and Treatment of Cancer) professionals. MATERIALS AND METHODS: A cross-sectional survey was distributed through the EORTC network in May 2025, with a reminder

Salvage treatment strategies for ipsilateral breast tumor recurrence after breast-conserving therapy: a real-world study.

BACKGROUND: Salvage mastectomy has long been regarded as the standard treatment for ipsilateral breast tumor recurrence (IBTR) after initial breast-conserving surgery (BCS) combined with radiotherapy. However, the feasibility and oncological outcomes of alternative local treatment strategies-such as repeat BCS (R-BCS) with or without re-irradiation, or mastectomy combined with re-irradiation-remain unclear. MATERIALS AND METHODS: Using data from the Surveillance, Epidemiology, and End Results (SEER) database, we retrospectively analyzed female patients who developed IBTR after initial BCS com

Salvage brachytherapy for local recurrence of cancer after definitive irradiation: a GEC-ESTRO narrative review.

The number of irradiated cancer survivors is increasing; mainly for breast, prostate, head-and-neck, uterine and rectal cancers. Due to multiple factors including life expectancy, increasing cancer incidence and intensive cancer treatment improvements, cancer re-irradiation is an increasing phenomenon. Local recurrence of cancer after after definitive irradiation represents one of the most challenging therapeutic scenarios. The goal is to deliver to a pre-irradiated volume, a high dose (achieving secondary local control) while sparing organs at risk from deleterious over-dose (side effects). A

Development of a treatment planning protocol for the multi-centre reirradiation CURE Lung trial.

INTRODUCTION: High-dose lung cancer reirradiation is promising but associated with high toxicity risk. Development of and adherence to consensus guidelines will support safe use. MATERIALS AND METHODS: Literature review and live workshops were conducted to develop treatment planning guidelines in preparation for the Scandinavian CURE Lung trial. Relevant OARs, dose metrics, constraints, and priorities were considered. For six high-dose reirradiation lung cancer cases, the physical 3D dose distribution of previous treatment was mapped to current CT. The cases were distributed to eight radiothe

Intensity-modulated radiation therapy administered to a previously irradiated spine is effective and well-tolerated.

PURPOSE: This study sought to discern the clinical outcomes of intensity-modulated radiation therapy (IMRT) administered to the spine in patients who had undergone previous radiotherapy. METHODS: A total of 81 sites of 74 patients who underwent previous radiotherapy administered to the spine or peri-spine and subsequently received IMRT for the spine were analyzed in this study. The prescribed dose of 80 Gy in a biologically effective dose (BED) of α/β = 10 (BED10) was set as the planning target volume. The constraint for the spinal cord and cauda equine was D0.1 cc ≤ 100 Gy and ≤ 150 Gy of BE

Refractory ventricular tachycardia treated by a second session of stereotactic arrhythmia radioablation.

PURPOSE: Stereotactic arrhythmia radioablation (STAR) is an effective treatment for refractory ventricular tachycardia (VT), but recurrences after STAR were recently published. Herein, we report two cases of successful re-irradiation of the arrhythmogenic substrate. CASES: We present two cases of re-irradiation after recurrence of a previously treated VT with radioablation at a dose of 20 Gy. The VT exit was localized on the border zone of the irradiated volume, which responded positively to re-irradiation at follow-up. CONCLUSION: These two cases show the technical feasibility of re-irradia

Prospective randomized clinical studies involving reirradiation: update of a systematic review.

BACKGROUND: Reirradiation is a potentially useful option for many patients with recurrent cancer, aiming at cure or symptom palliation, depending on disease/recurrence type and stage. The purpose of this follow-up study to a previous review from 2016 was to summarize all recently published randomized trials. Points of interest again included identifcation of methodological strengths and weaknesses, practice-changing results, and open questions. MATERIAL AND METHODS: Systematic review of trials published between 2015 and February 2023. RESULTS: We reviewed 7 additional trials, most of which a

Constraints, Cumulative Doses, and Toxicity Outcomes in Glioma Reirradiation: A Systematic Review and Radiobiological Comparison From the Reirradiation Collaborative Group (ReCOG).

There remains considerable uncertainty regarding normal tissue tolerances to reirradiation for recurrent brain tumours. This systematic review aimed to collate reirradiation constraints, planned cumulative organ at risk (OAR) doses and corresponding toxicity outcomes for adults treated with reirradiation for recurrent glioma. The Medline, Embase, Cochrane, Web of Science and Scopus databases were searched for studies published up to 1st July 2025. To allow comparison between constraints, where necessary, equivalent doses in 2Gy fractions (EQD2Gy) were calculated. In total, 27 studies were incl

Reirradiation Options for Previously Irradiated Prostate Cancer: Is It Feasible to Randomise Between Treatment With Stereotactic Body Radiotherapy (SBRT) Versus High-Dose-Rate Brachytherapy?

AIMS: Prostate cancer which recurs within the prostate following definitive radiotherapy can be treated again using radiation, known as reirradiation. Systematic reviews of cohort studies of salvage therapies suggest that reirradiation using brachytherapy (BT) or stereotactic body radiotherapy (SBRT) offers favourable disease control with acceptable toxicity. However, the optimal radiotherapy modality in this setting remains to be determined. The study aimed to establish feasibility of randomisation between HDR BT and SBRT. MATERIALS AND METHODS: The reirradiation options for previously irrad

Beyond the first dose: Systematic review on external beam radiotherapy for locoregional breast cancer recurrence.

BACKGROUND: The optimal management of locoregional breast cancer recurrence after prior radiotherapy remains challenging. Reirradiation (ReRT) may allow repeat breast-conserving surgery as an alternative to mastectomy; however, prospective evidence is scarce, and existing data are largely retrospective, with the strongest support for brachytherapy. Despite this, external beam reirradiation (EBreRT) is widely used because of its availability. This systematic review evaluated the feasibility, oncologic outcomes, toxicity, and patient-reported outcomes of external-beam ReRT for locoregional breas

Impact of salvage surgery with or without reirradiation for skull base meningiomas recurring after prior radiotherapy.

OBJECTIVE: Long-term follow-up of meningiomas has demonstrated recurrence rates ranging from 2.5% to 48% after 10 years, depending on histology grade. There are limited data available to guide the management of recurrent and previously irradiated skull base meningiomas, and challenges related to salvage surgery, reirradiation, and lack of clear systemic therapy strategies remain. In this study, the authors analyzed data from their experience with recurrent and previously irradiated meningiomas to assess the impact of salvage surgery and reirradiation on progression-free survival (PFS).
