[Treatment outcome of psychodynamic trauma therapy in an inpatient setting].

OBJECTIVE: Chronic traumatization with the beginning in childhood may result in a number of additional problems not included in the diagnosis of Posttraumatic Stress Disorder (PTSD). The efficacy of a three-stage psychodynamically oriented inpatient treatment program (PITT) and the role of chronic childhood threat on treatment outcome was investigated. METHODS: A six-week treatment group of 84 inpatients were compared with 43 wait-list controls with "treatment as usual" (outpatient psychotherapy or psychiatric treatment in the meanwhile). Using measures on PTSD, dissociation, depression, self

Abuse, invalidation, and lack of early warmth show distinct relationships with self-criticism, self-compassion, and fear of self-compassion in personality disorder.

BACKGROUND: Cultivating self-compassion is increasingly recognized as a powerful method to regulate hyperactive threat processes such as shame and self-criticism, but fear of self-compassion (FSC) can inhibit this. These difficulties are underexplored in personality disorder (PD) despite their prevalence. Furthermore, little evidence exists regarding how these factors relate to adverse childhood experiences (ACEs) and attachment. METHOD: Fifty-three participants with a diagnosis of PD completed measures including childhood abuse/neglect, invalidation, early warmth, self-compassion, shame, sel

Disorganized infant, child, and adult attachment: collapse in behavioral and attentional strategies.

This presentation focuses on the disorganized/disoriented (Group D) categories of infant, child, and adult attachment. The infant D category is assigned on the basis of interruptions and anomalies in organization and orientation observed during Ainsworth's strange situation procedure. In neurologically normal low-risk samples, D attachment is not substantially related to descriptions of infant temperament, and usually appears with respect to only one parent. At six, former D infants are often found to be role-inverting (D-Controlling) towards the parent, while drawings and separation-related n

Treating childhood trauma.

This review begins with the question "What is childhood trauma?" Diagnosis is discussed next, and then the article focuses on treatment, using 3 basic principles-abreaction, context, and correction. Treatment modalities and complications are discussed, with case vignettes presented throughout to illustrate. Suggestions are provided for the psychiatrist to manage countertransference as trauma therapy proceeds.

Marital psychological and physical aggression and children's mental and physical health: direct, mediated, and moderated effects.

Relations between marital aggression (psychological and physical) and children's health were examined. Children's emotional insecurity was assessed as a mediator of these relations, with distinctions made between marital aggression against mothers and fathers and ethnicity (African American or European American), socioeconomic status, and child gender examined as moderators of effects. Participants were 251 community-recruited families, with multiple reporters of each construct. Aggression against either parent yielded similar effects for children. Children's emotional insecurity mediated the

Interpersonal problems and personality features as mediators between attachment and intimate partner violence.

We examined whether hostile dominant interpersonal problems (HDIP), antisocial features, and borderline features mediated the relationship between attachment (anxiety or avoidance) and intimate partner violence (IPV) with a sample of 132 male partner abusers. We conducted two path analyses with avoidant attachment as the predictor in one model and anxious attachment as the predictor in a second model. In both models, HDIP, antisocial features, and borderline features were the mediators with IPV as the criterion. For both models, the attachment variable had statistically significant path values

Attachment in young children with incarcerated fathers.

The present study examined young children's attachment behaviors during paternal incarceration and reported on initial validity of a new measure used to rate children's attachment-related behaviors and emotions during visits in a corrections setting. Seventy-seven children, age 2 to 6 years, and their jailed fathers and current caregivers participated in the home visit portion of the study, whereas 28 of these children participated in the jail visit. The results indicated that 27% of children witnessed the father's crime and 22% of children witnessed the father's arrest, with most children who

Tests of Theories of Crime in Female Prisoners.

Several general theories of crime were tested with path models on 293 female prisoners in a U.S. State prison. The theories tested included Social Bond and Control, Thrill/Risk Seeking, and a new attachment-based Developmental Dynamic Systems model. A large battery of different instruments ranging from measures of risk taking, to a crime addiction scale, to Childhood Adverse Events, to attachments and clinical issues were used. The older general theories of crime did not hold up well under the rigor of path modeling. The new dynamic systems model was supported that incorporated adverse childho

Helping children with asthma by repairing maternal-infant bonding problems.

Studies about the psychology of childhood asthma have revealed that parenting difficulties are related to the development of asthma in some children. Disruptions in maternal-infant bonding are highly correlated with pediatric asthma and are presented as a cause for these parenting problems. Bonding problems are known to be caused most often by physical separation at birth or by some recent trauma in the mother's life. By using hypnosis to remove the pain of the separation or trauma in the mother, and by creating a new birth history in her imagination, some children's asthmatic symptoms have be

When Nowhere Is Safe: Interpersonal Trauma and Attachment Adversity as Antecedents of Posttraumatic Stress Disorder and Developmental Trauma Disorder.

Developmental trauma disorder (DTD) has been proposed as clinical framework for the sequelae of complex trauma exposure in children. In this study, we investigated whether DTD is associated with different traumatic antecedents than posttraumatic stress disorder (PTSD). In a multisite sample of 236 children referred from pediatric or mental health treatment, DTD was assessed using the DTD Structured Interview. Trauma history was assessed using the Traumatic Events Screening Instrument (TESI). On an unadjusted basis, both DTD, odds ratios (ORs) = 2.0-3.8, 95% CI [1.17, 7.19]; and PTSD, ORs = 1.8

Mindfulness Meditation for Fibromyalgia: Mechanistic and Clinical Considerations.

PURPOSE OF REVIEW: Fibromyalgia is a disorder characterized by widespread pain and a spectrum of psychological comorbidities, rendering treatment difficult and often a financial burden. Fibromyalgia is a complicated chronic pain condition that requires a multimodal therapeutic approach to optimize treatment efficacy. Thus, it has been postulated that mind-body techniques may prove fruitful in treating fibromyalgia. Mindfulness meditation, a behavioral technique premised on non-reactive sensory awareness, attenuates pain and improves mental health outcomes. However, the impact of mindfulness me

Maternal panic disorder: infant temperament, neurophysiology, and parenting behaviors.

OBJECTIVE: To determine whether 4- and 14-month-old infants of mothers with panic disorder (PD) would be more likely to show differences in temperament, neurophysiology (salivary cortisol and sleep), and relationships with their mothers than controls. METHOD: Two cohorts were recruited: 4-month-old infants with PD mothers (n = 25) and 4-month-old controls (n = 24), and 14-month-old infants with PD mothers (n = 27) and 14-month-old controls (n = 18). Mothers completed diagnostic interviews and questionnaires concerning infant temperament, sleep, and parenting. Infant salivary cortisol samples

Self-oxidatively crosslinked sprayable hydrogel for microenvironment remodeling and accelerated healing of recurrent aphthous ulcers.

Recurrent aphthous ulcer (RAU) is a prevalent inflammatory disorder of the oral mucosa, characterized by a persistent "vicious cycle" of excessive oxidative stress, dysregulated immune responses, and impaired tissue regeneration. Effective clinical management remains challenging due to the dynamic and hydrated oral environment, which severely limits drug retention and multi-target therapeutic efficacy. Herein, we developed a multifunctional, sprayable hydrogel (PDS Gel) using a hierarchical "oxygen-limited initiation - oxygen-controlled progression" crosslinking strategy, integrating polygalac

Predictors of problematic sexual behavior among children with complex maltreatment histories.

Exploratory analyses of sexual behavior problems (SBP) were conducted within a larger epidemiological study of 347 preadolescent children in foster and kinship care. SBP was estimated from carergiver-reported scores on the Assessment Checklist for Children. The study simultaneously examined a large number of discrete and cumulative influences on the development of children at high risk for SBP. Most children with SBP had corresponding psychopathology, most notably conduct problems, inattention, and interpersonal behavior problems suggestive of attachment disturbances. Several correlates identi

Developmental psychopathological perspectives on sexually compulsive behavior.

Highly pernicious events can result in a variety of severe adult psychiatric manifestations, including pedophilia in select individuals with a history of prior "at-risk factors." Influences such as social isolation can either increase or decrease the outcome. This article reviews some of the other sequential developmental factors that might contribute to sexual compulsivity in such biographies, including temperament, early attachment, family influences, trauma re-enactments, affect dysregulation, social isolation, vandalized love maps, self-formation, sexualization in families, and addictive c

BACKGROUND: The prevalence of mental disorders and global burden of disease increases and need an integrated psychiatric treatment at the best point of service. Severe mental disorders have medical consequences and solid psychiatric interventions are necessary. Research is required to be conducted along the bio-psycho-social paradigm. AIM: Based on the dynamics of memory and mentalization theory this paper presents clinical and therapeutic considerations for change. RESULTS: Mentalization is influenced by patterns of parental rearing styles, attachment, and traumatic incidents. In psychoanal

Injury and differentiation following inhibition of mitochondrial respiratory chain complex IV in rat oligodendrocytes.

Oligodendrocyte lineage cells are susceptible to a variety of insults including hypoxia, excitotoxicity, and reactive oxygen species. Demyelination is a well-recognized feature of several CNS disorders including multiple sclerosis, white matter strokes, progressive multifocal leukoencephalopathy, and disorders due to mitochondrial DNA mutations. Although mitochondria have been implicated in the demise of oligodendrocyte lineage cells, the consequences of mitochondrial respiratory chain defects have not been examined. We determine the in vitro impact of established inhibitors of mitochondrial r

PISA: Inflamed Surface Area of Gums Triggering Systemic Hyperinflammation.

Periodontitis, a chronic inflammatory disease, causes progressive loss of periodontal tissues. Traditional measures like clinical attachment loss and probing pocket depth fail to quantify the inflammatory burden accurately. The Periodontal Inflamed Surface Area (PISA) quantifies the surface area of bleeding pocket epithelium, offering a better measure of inflammation. This review explores the association between PISA and systemic disorders such as cardiovascular disease, diabetes, dementia, and rheumatoid arthritis (RA). Studies show higher PISA levels correlate with elevated systemic inflamma

Clinical significance of a proposed developmental trauma disorder diagnosis: results of an international survey of clinicians.

OBJECTIVE: Maltreatment, family violence, and disruption in primary caregiver attachment in childhood may constitute a developmental form of trauma that places children at risk for multiple psychiatric and medical diagnoses that often are refractory to well-established evidence-based mental health treatments. No integrative diagnosis exists to guide assessment and treatment for these children and adolescents. This study therefore assessed clinicians' ratings of the clinical utility of a proposed developmental trauma disorder diagnostic framework. METHOD: An Internet survey was conducted with

Promoting a secure attachment: A primary prevention practice model.

It is increasingly recognized that early sensitive care-giving predicts a secure attachment in infancy and early childhood. This preliminary report details the development, implementation and evaluation of a clinical programme that used a targeted prevention approach following a universally-offered screening of parent-infant interactions. The CARE-Index was used to assign dyads to low, medium and high risk groups, and interventions - featuring reflective videotape-based feedback - were tailored to each risk group in order to increase maternal sensitivity and improve infant attachment status.
