Molecular biologic studies of Hodgkin's disease.

There has been intense interest in applying the techniques of molecular biology to further our understanding of Hodgkin's disease. Numerous studies of the status of lymphocyte antigen receptor genes have been performed, with conflicting results. There is more uniformity in the results of Epstein-Barr viral (EBV) studies; evidence of EBV can be identified in the Reed-Sternberg cells of approximately 40% to 50% of cases. There is no consensus on whether the t(14;18) is present in tissues involved by Hodgkin's disease.

The biology of classical Hodgkin lymphoma.

Classical Hodgkin lymphoma (cHL) is distinguished by several important biological characteristics. The presence of Hodgkin Reed Sternberg (HRS) cells is a defining feature of this disease. The tumor microenvironment with relatively few HRS cells in an expansive infiltrate of immune cells is another key feature. Numerous cell-cell mediated interactions and a plethora of cytokines in the tumor microenvironment collectively work to promote HRS cell growth and survival. Aberrancy and constitutive activation of core signal transduction pathways are a hallmark trait of cHL. Genetic lesions contribut

Predictors of risk of relapse in classic Hodgkin lymphoma.

Using multiparametric flow cytometric analysis, in a cohort of 62 patients with classic Hodgkin lymphoma having a median follow-up period of 69.5 months, we found-patients who experienced primary resistance or disease relapse (DR) had lower percentage of rosetted Hodgkin Reed-Sternberg cells (HRS-cells) as compared with patients who achieved sustained complete remission (SCR) (p=0.022); patients >35 years of age had higher percentage of HRS-cells (p=0.017) and lower percentage of B cells (p=0.017) and the nodular sclerosis subtype had higher percentage of B-cells (p=0.046) and activated B-cell

Sudden death in an adolescent due to undiagnosed classic Hodgkin lymphoma.

Few cases of natural sudden death presenting as an undiagnosed lymphoma have been reported in the literature, especially in adolescents. Herein we provide a report of sudden death caused by undiagnosed classic Hodgkin lymphoma (cHL). We describe an 18-year-old female who collapsed after several weeks of weight loss, decreased appetite, and dyspnea. At autopsy, a bulky mass arising in the mediastinum and neck compressed the esophagus and trachea, surrounded the great vessels, obliterated the pericardial sac, and infiltrated the myocardium. The lungs were collapsed and large pleural effusions we

[Microenvironment in classical Hodgkin lymphoma].

Classical Hodgkin lymphoma (cHL) is histologically characterized by a quantitatively dominating immune cell infiltrate. Its composition differs depending on the histological subtype and EBV (Epstein-Barr-Virus) status. Current pathogenic concepts postulate that the malignant cells, the so-called Hodgkin and Reed-Sternberg (HRS) cells, act as master recruiters, thereby actively shaping the microenvironment to support their proliferation and outgrowth. This view on the pathogenesis of cHL is further solidified by genetic studies, which have identified important mechanisms by which the HRS cells

[Intrasinusal and extrasinusal space-occupying lesion of the maxillary sinus].

A 72-year-old healthy male patient presented with a painless mass on the left cheek which had been growing for 3 months as well as swollen cervical lymph nodes. Computed tomography revealed a space-occupying lesion on the facial wall of the sinus. Biopsy and histological examination of the lesion and a lymph node revealed Hodgkin's and Sternberg-Reed cells. The diagnosis of Hodgkin's disease was confirmed immunohistologically by the detection of atypical blast cell infiltrates with reactivity for CD30 and CD15 surface antigens.

MMA monoclonal antibody is a superior anti-CD15 reagent for the diagnosis of classical Hodgkin's lymphoma?

CD15 is a useful immunohistochemical marker to identify Reed-Sternberg cells in classical Hodgkin's lymphoma (HL) and to distinguish it from HD-like neoplasms, but data from the literature concerning its expression in HL are quite variable. Using immunohistochemistry we compared the reactivity of three different anti-CD15 clones (MMA, C3D1 and BY87) and found that anti-CD15 MMA clone is a superior reagent in identifying atypical cells, detecting more numerous cells in 28.2%, and being the only positive marker in 12.8% of cases. We conclude that it is advisable to include this reagent in diagno

T-cell rich B-cell lymphoma--a case report.

T-Cell-Rich B-Cell lymphoma (TCRBCL) is a recently described variant of diffuse large B-cell lymphoma characterized by a predominance of reactive T-cells and a minority of neoplastic large cells which may resemble the Reed-Sternberg cell or its variants. TCRBCL is a notorious mimicker of both lymphocyte predominance Hodgkin's disease (LPHD) and peripheral T-cell lymphoma (PTCL). Immunohistochemistry is mandatory for a definitive diagnosis. We report a case of TCRBCL which was mistaken for HD both on fine needle aspiration cytology and histopathology due to the presence of RS like cells. Immuno

[A rare stomach tumor--Hodgkin's lymphoma of the stomach].

Gastric involvement of Hodgkin's disease is very rare and preoperative diagnosis is difficult. We report the case of a 50-year-old woman, who had an ulcerating tumor of the antrum, which was revealed as carcinoma by endoscopic biopsy. A gastrectomy with D2-lymph node dissection was performed. In the pathological specimen Hodgkin's disease was diagnosed because of microscopic detection of typical binucleated Reed-Sternberg cells. Although the patient refused additive chemotherapy, she is still disease-free after 12 months of follow-up.

Survival by Design: Cell Fate Dynamics in Classical Hodgkin Lymphoma.

Classical Hodgkin lymphoma (cHL) is singular because rare Hodgkin and Reed-Sternberg (HRS) cells survive within an abundant inflammatory tumor microenvironment (TME) - despite originating from germinal center (GC) B cells that would normally be negatively selected, they persist through coordinated disruption of lineage fidelity, immune recognition, and cell-death execution. The present review integrates mechanistic, translational, and therapeutic evidence on apoptosis resistance, survival signaling, microenvironmental support, immune evasion, Epstein-Barr virus-associated rescue mechanisms, an

Expression of enolases in T cell tumors and Hodgkin's disease.

Frozen biopsy specimens taken from 30 cases with T cell tumors (8 with T cell acute lymphoblastic leukemia, 8 with T cell lymphoblastic lymphoma, and 14 with peripheral T cell lymphomas), and from 12 with Hodgkin's disease, were investigated using a direct immunohistochemical method to detect alpha-, beta- and gamma-enolases. Normal thymus and lymph node specimens with reactive lymphadenitis were also investigated. Subcortical thymocytes and the majority of deep cortical thymocytes showed reactivity of alpha-/beta-/gamma- approximately +/- -enolases, and medullary thymocytes and small lymphocy

Characterization of giant cells in Hodgkin's lymphomas by immunohistochemistry applied to randomly collected diagnostic biopsies from the German Hodgkin Trial.

A panel of 10 monoclonal antibodies reactive with formalin-resistant epitopes was applied to characterize the giant cells of the Reed-Sternberg, Hodgkin, and lacunar cell types in diagnostic biopsies from the German Hodgkin Trial. The 94 tissue samples examined had been sent to the Reference Center by 44 different laboratories which made the initial diagnoses. A board of four referees re-classified the primary diagnoses established by the 44 different histopathologists, providing subtyping of Nodular Sclerosis according to Bennett et al. (1985, 1989). Only cases with unequivocal agreement amon

Spatially Resolved Tumor Microenvironment Predicts Treatment Outcomes in Relapsed/Refractory Hodgkin Lymphoma.

PURPOSE: About a third of patients with relapsed or refractory classic Hodgkin lymphoma (r/r CHL) succumb to their disease after high-dose chemotherapy followed by autologous stem-cell transplantation (HDC/ASCT). Here, we aimed to describe spatially resolved tumor microenvironment (TME) ecosystems to establish novel biomarkers associated with treatment failure in r/r CHL. PATIENTS AND METHODS: We performed imaging mass cytometry (IMC) on 71 paired primary diagnostic and relapse biopsies using a marker panel specific to CHL biology. For each cell type in the TME, we calculated a spatial score

CD40 ligand is constitutively expressed in a subset of T cell lymphomas and on the microenvironmental reactive T cells of follicular lymphomas and Hodgkin's disease.

Although CD40 has been extensively studied in B- and T-cell non-Hodgkin's lymphomas (NHLs)/leukemias, and more recently in Hodgkin's disease (HD), little is known about the expression of its ligand (CD40L) in lymphoproliferative disorders other than T-cell NHLs/leukemias. A series of 121 lymphoma/leukemia samples, including 35 cases of HD, 34 T-cell and 39 B-cells NHLs, 2 cases of adult T-cell leukemia/lymphoma, and 11 cases of T-cell acute lymphoblastic leukemia, were evaluated for CD40L expression by immunostaining of frozen tissue sections and flow cytometry with the anti-CD40L monoclonal a

An eosinophilic fibrohistiocytic lesion of bone marrow in a patient with Hodgkin's disease. A potential for morphologic confusion.

The eosinophilic fibrohistiocytic lesion of bone marrow (EFLBM) is characterized by collections of elongated "fibrohistiocytic" cells in association with lymphocytes, eosinophils, and plasma cells. The fibrohistiocytic cells are mast cells, and many investigators include this lesion (EFLBM) as a localized form of mastocytosis. The etiology and clinical significance of these lesions remains unclear. The morphologic features of these lesions point to a wide differential diagnosis that includes Hodgkin's disease. Currently, however, there are no recorded cases of well-defined Hodgkin's disease wi

Hodgkin's disease in a patient with common variable immunodeficiency.

A 61 year old man with long standing common variable immunodeficiency presented with pyrexia, anaemia and leucopenia. A diagnoses of Hodgkin's disease of the bone marrow was made. The typical histopathological and immunophenotypic appearances were clearly distinct from those of T cell lymphoma with Reed-Sternberg-like cells which, in contrast to Hodgkin's disease, is a known complication of common variable immunodeficiency. Complete clinical and histological remission was achieved with combination chemotherapy. The latter was complicated by severe myelosuppression, unusually severe erosive muc

Advances in Hodgkin lymphoma research.

Hodgkin lymphoma (HL) has been and still is the most enigmatic lymphoid malignancy in humans. Since the first molecular analysis of isolated Hodgkin and Reed-Sternberg (HRS) tumor cells of classic HL 30 years ago, substantial advances in our understanding of HL have been made. This review describes the cellular origin of HL, summarizes the current knowledge about the genetic lesions in HRS cells, and highlights the role of Epstein-Barr virus (EBV) in HL pathogenesis. Moreover, the pathobiological roles of altered gene expression and deregulated signaling pathways are discussed and key aspects

[Role of Epstein-Barr Virus in lymphoproliferative disorders].

The EBV plays a major role in the development of lymphoproliferative disorders in immunosuppressed patients. After organ transplantation most of lymphoproliferative disorders associated with EBV are polymorphic, with various expression of clonality. The pattern of EBV latency genes expression is rather the same as in lymphoblastoid cells lines and the EBV infected cells strongly expressed activation and adhesion molecules in most cases. In AIDS-related lymphomas the frequency of EBV as well as the expression of latency genes are related to the localization and to the histological subtypes. Whi
