Dissociation between caloric test and the video head impulse test in individuals with auditory neuropathy spectrum disorders.

OBJECTIVES: The present study aimed to measure an association between the caloric test and vestibulo-ocular reflex gain and refixation saccades in individuals with auditory neuropathy spectrum disorders. METHODS: Twenty individuals (9 males and 11 females), in the age range of 17 to 38 years, diagnosed with auditory neuropathy spectrum disorders, and 20 normal-hearing individuals (age and gender matched) participated in the study. All the individuals underwent case history, pure tone audiometry, immittance evaluation, otoacoustic emissions, auditory brainstem response, caloric tests and video

Paresis of contralateral smooth pursuit and normal vestibular smooth eye movements after unilateral brainstem lesions.

Pursuit and vestibular smooth eye movements were measured in patients with lesions of the caudal brainstem tegmentum identified by magnetic resonance imaging (MRI) and computed tomography (CT), with neuropathological correlation in 1 patient. Contralateral smooth pursuit gain was significantly lower than ipsilateral gain in each patient. Ipsilateral smooth pursuit gain was also subnormal in patients with unilateral pontine damage that caused slowing of ipsilateral saccades. Horizontal vestibulo-ocular reflex gain and phase were normal. These quantitative correlations indicate that lesions of t

Ocular movements in essential blepharospasm.

In essential blepharospasm histopathologic and electrophysiologic evidence supports the existence of lesions in proximity to brainstem nuclei controlling ocular movements. We studied horizontal ocular movements in eight patients who had been treated previously with surgery or botulinum toxin injection to control essential blepharospasm (mean age, 58 years) and compared these with seven control subjects who did not have blepharospasm (mean age, 68 years). We examined fixation stability, saccades, the vestibulo-ocular reflex, visual enhancement and suppression of the vestibulo-ocular reflex, opt

Plasticity of the human otolith-ocular reflex.

The eye movement response to earth vertical axis rotation in the dark, a semicircular canal stimulus, can be altered by prior exposure to combined visual-vestibular stimuli. Such plasticity of the vestibulo-ocular reflex has not been described for earth horizontal axis rotation, a dynamic otolith stimulus. Twenty normal human subjects underwent one of two types of adaptation paradigms designed either to attenuate or enhance the gain of the semicircular canal-ocular reflex prior to undergoing otolith-ocular reflex testing with horizontal axis rotation. The adaptation paradigm paired a 0.2 Hz si

Update on the medial longitudinal fasciculus syndrome.

The medial longitudinal fasciculus is a key structure for conjugate horizontal eye movements by relaying signals from the abducens internuclear neurons to the medial rectus subdivision of the contralateral oculomotor nucleus. Thus, lesions involving the medial longitudinal fasciculus give rise to a typical neuro-ophthalmological sign, the internuclear ophthalmoplegia, which is characterized by impaired adduction of the ipsilesional eye and dissociated abducting nystagmus of the contralateral eye during attempted contralesional horizontal gaze. In addition, medial longitudinal fasciculus lesion

[Functional structure and cerebrovascular circulation of the vestibulo-ocular formations of the brain stem of the human cerebrum].

This work is dedicated to the study of anatomo-physiological peculiarities of the construction and cerebrovascular circulation of the vestibulo-ocular formations of the brain stem of the human cerebrum. The analysis of anatomo-physiological data enable to convince that the arches of the vertical and horizontal vestibulo-ocular reflexes include many structures of the brain stem of the human cerebrum, which are located along its longitude from the superior colliculi to caudal sections of brain stem. There is an existence of separate mechanisms of horizontal and vertical vestibulo-ocular interact

The effect of hand dominance on results in the Functional Head Impulse Test.

OBJECTIVE: The vestibulo-ocular reflex is essential for maintaining visual stability during head movements. Individual neurophysiological characteristics, such as hand dominance, may influence vestibular responses through hemispheric lateralisation. This study aimed to investigate the effect of hand dominance on functional vestibulo-ocular reflex performance using the Functional Head Impulse Test. METHODS: Fifty-eight healthy volunteers were classified as right- or left-handed using the Edinburgh Handedness Inventory. Functional Head Impulse Test reading scores were obtained during passive ri

Vestibular neuronitis evaluated by the combined galvanic test.

Forty-two patients with unilateral vestibular neuronitis were examined by the combined galvanic test (CGT), consisting of the galvanic eye movement test (GEMT) and the galvanic body sway test (GBST). Results of the CGT were evaluated with respect to the time period from the onset of vestibular neuronitis. A favorable recovery was observed earlier with the GBST than the GEMT. We believe that this discrepancy may be due to the difference in compensation between the vestibulo-ocular reflex and the vestibulo-spinal reflex.

On the search for markers of poor vestibular compensation.

The effectiveness of pursuit gain, cancellation of the vestibulo-ocular reflex, and a clinical oscillopsia test were assessed as vestibular function tests and tests that may allow prediction of which patients would compensate poorly after vestibular surgery. Cancellation of the vestibulo-ocular reflex in 17 patients and 17 control subjects was compared. Pursuit gain for 17 patients was determined for three frequencies at peak velocities of 25 and 50 degrees/sec. The oscillopsia test was administered to seven patients during at least the first 6 postoperative months. We are unable to state that

Nucleus prepositus hypoglossi lesions produce a unique ocular motor syndrome.

OBJECTIVE: To describe the ocular motor abnormalities in 9 patients with a lesion involving the nucleus prepositus hypoglossi (NPH), a key constituent of a vestibular-cerebellar-brainstem neural network that ensures that the eyes are held steady in all positions of gaze. METHODS: We recorded eye movements, including the vestibulo-ocular reflex during head impulses, in patients with vertigo and a lesion involving the NPH. RESULTS: Our patients showed an ipsilesional-beating spontaneous nystagmus, horizontal gaze-evoked nystagmus more intense on looking toward the ipsilesional side, impaired p

Motion Illusion-Evidence towards Human Vestibulo-Thalamic Projections.

Contemporary studies speculated that cerebellar network responsible for motion perception projects to the cerebral cortex via vestibulo-thalamus. Here, we sought for the physiological properties of vestibulo-thalamic pathway responsible for the motion perception. Healthy subjects and the patient with focal vestibulo-thalamic lacunar stroke spun a hand-held rheostat to approximate the value of perceived angular velocity during whole-body passive earth-vertical axis rotations in yaw plane. Vestibulo-ocular reflex was simultaneously measured with high-resolution search coils (paradigm 1). In prim

Anatomical correlates of ocular motor deficits in cerebellar lesions.

Humans are able to stabilize the images of moving targets on the retina by means of smooth pursuit eye movements. After the pontine level, all smooth pursuit pathways pass through the cerebellum. Previous animal studies gave evidence that two specific lesion sites within the cerebellum cause smooth pursuit disorders: those of the flocculus/paraflocculus and the vermis including lobule VI, VII, the uvula and the deep cerebellar nuclei. To date, there have been only a few lesion studies in patients with smooth pursuit disorders that do not allow direct comparison with a control group. In the pre

Vestibulo-Ocular Responses, Visual Field Dependence, and Motion Sickness in Aerobatic Pilots.

BACKGROUND: Aerobatic flight is a challenge for the vestibular system, which is likely to lead to adaptive changes in the vestibular responses of pilots. We investigated whether aerobatic pilots, as individuals who experience intense vestibular stimulation, present modifications of the vestibular-ocular reflex, motion sickness susceptibility and intensity, visual vertical estimation, and visual dependence as compared to normal volunteers.METHODS: To evaluate vestibulo-ocular reflexes, eye movements were recorded with videonystagmography while subjects were rotated on a rotatory chair with the

Convergent innervations of mesencephalic trigeminal and vestibular nuclei neurons onto oculomotor and pre-oculomotor neurons-Tract tracing and triple labeling in rats.

In studies of vestibulo-ocular reflex (VOR), the horizontal VOR circuit is much clearer than vertical-torsional VOR. The circuit and mechanism of gravity-related vertical-torsional VOR is probably weak. "Somatosensory vestibular interaction" is a known extra source to facilitate VOR, and cervico-ocular reflex is a representative for torsional VOR compensation. Whereas, how the cervical afferents finally reach the oculomotor system is less documented. Actually, when the head tilts, which generates cervico-ocular reflex, not only the neck muscle is activated, but also the jaw muscle is stretched

Comparison of Asymmetry between Perceptual, Ocular, and Postural Vestibular Screening Tests.

BACKGROUND: A better understanding of how vestibular asymmetry manifests across tests is important due to its potential implications for balance dysfunction, motion sickness susceptibility, and adaptation to new environments. OBJECTIVE: We report the results of multiple tests for vestibular asymmetry in 32 healthy participants. METHODS: Asymmetry was measured using perceptual reports during unilateral centrifugation, oculomotor responses during visual alignment tasks, vestibulo-ocular reflex gain during head impulse tests, and body rotation during stepping tests. RESULTS: A significant corr

Markerless three-dimensional analysis of mouse eye movements using stereo vision.

BACKGROUND: Quantifying three-dimensional mouse eye rotations, especially torsion, often requires invasive implanted coils or ocular markers. We developed and validated a markerless stereo vision-based method to measure yaw, pitch, and roll eye rotations. NEW METHOD: Two aligned cameras were geometrically calibrated. A convolutional neural network detected pupil edges, which were fitted with ellipses. Matching edge points were reconstructed in three dimensions to estimate the rotation center and compute yaw and pitch; roll was derived from torsional shifts in pupil-edge irregularities. Perfor

Vestibular hyperreactivity in patients with idiopathic spasmodic torticollis.

Ocular motor tests performed on 14 patients with idiopathic spasmodic torticollis were normal. The vestibulo-ocular reflex tested in eight patients showed a significant high level in seven. It is suggested that this phenomenon is secondary rather than the cause of spasmodic torticollis.

Caloric and rotatory chair test results in patients with Ménière's disease.

OBJECTIVES: To determine whether an association exists between the parameters of the caloric and rotatory chair tests in patients with unilateral Ménière's disease. METHODS: Patients with unilateral Ménière's disease (n = 100) were subjected to the caloric and the rotatory chair test (sinusoidal harmonic acceleration and impulsive tests) on the same day. Canal paresis and directional preponderance were assessed in the caloric test, and different variables were measured in the rotatory chair test based on the existence of abnormal parameters in the vestibulo-ocular reflex at two or three conse
