Outcomes of reoperative aortic root surgery.

OBJECTIVE: The study objective was to determine the impact of reoperative aortic root replacement on short-term outcomes and survival. METHODS: This was a retrospective study of aortic root operations from 2010 to 2018. All patients with a complete aortic root replacement were included, and patients undergoing valve-sparing root replacements were excluded. Patients were dichotomized by first-time sternotomy versus redo sternotomy, which was defined as having had a prior sternotomy for whatever reason. Within the redo sternotomy group, reoperative aortic root replacements were identified, bein

Reoperative Pectus Repair Using Biomaterials.

BACKGROUND: Reoperation after failed pectus repair, Open or Nuss, is complex. In the majority of patients, metal bars or plates are used. Recently, an absorbable bar (poly-L-lactide [PLA]) was introduced for rib fixation. This series is my experience of using this biomaterial for reoperative pectus surgery. METHODS: We respectively reviewed the medical records of all patients who were referred to our institution for pectus abnormalities; 180 patients were evaluated, 62 patients (34%) underwent reoperation. RESULTS: Sixty-two patients underwent reoperative Open repair. Median age was 38 years

Risk factors in reoperations in colorectal surgery.

BACKGROUND: Reoperations in colorectal surgery are usually a consequence of major surgical complications. Recently, the rate of reoperation has been proposed as a marker of surgical performance. Yet, the incidence of re-intervention varies significantly in literature, ranging from 5.2% to 13%. Therefore, in this study we investigated 30-day reoperation rates and made an attempt to identify risk factors of re-intervention following colorectal resection at our institution. METHODS: This is a retrospective study of patients who had undergone colorectal resection at a single institution from 2013

Reoperative experience with papillary thyroid cancer.

BACKGROUND: Intense postoperative monitoring has resulted in increasing detection of patients with recurrent papillary thyroid cancer (PTC). Our goals included quantifying successful reoperation, and analyzing surgical complications and reasons for relapse. METHODS: From 1999 to 2008, a total of 410 patients underwent reoperation for PTC relapse. We analyzed post-reoperative disease outcomes, reasons for relapse, and complications. RESULTS: Bilateral reoperative thyroidectomy was performed in 13 (3 %) patients; lobectomy, 34 (8 %); central neck (VI) soft tissue local recurrence excision, 47

Radioguided parathyroidectomy for hyperparathyroidism in the reoperative neck.

BACKGROUND: The purpose of this study was to determine if radioguided parathyroidectomy (RGP) is effective for hyperparathyroidism (HPT) in the reoperative neck. METHODS: We retrospectively reviewed all patients with HPT and a history of neck surgery who underwent RGP over a 7-year period. Data are reported as mean +/- SEM. RESULTS: We identified 110 patients with primary (n = 94), secondary (n = 7), or tertiary (n = 9) HPT who underwent 138 previous neck operations. The average hospital stay was 0.6 +/- 0.1 days. The in and ex vivo counts obtained with the gamma probe were 310 +/- 26 and 13

[Peculiarities of sanitizing middle ear reoperations].

This paper is devoted to the problem of surgical rehabilitation of patients with the so-called <<operated ear disease>>. Twenty such patients underwent second operation following sanitization ear surgery for the treatment of recurrent (more than once a year) otorrhoea in the previous period. The operation was performed in two (sanitizing and reconstructive) steps. All the patients were allocated to 3 groups based on the results of preoperative examination and the cause of purulent discharge. Group 1 comprised patients with the <<open>> Eustachian tube, group 2 included patients with the cariou

Reoperative Crohn's surgery: tricks of the trade.

Crohn's disease is a chronic inflammatory disease of the intestinal tract that often results in the need for surgical intervention to treat complications of the disease. The transmural nature of the inflammation can lead to intestinal perforation, intraabdominal abscesses, intestinal strictures, and fistula development. Because there is no cure for Crohn's disease, many patients will require multiple operations during their lifetime. Index surgery and reoperative surgery in these patients is often complex and challenging. There are many preoperative planning and technical aspects of Crohn's su

[Reoperations for CTS because of recurrence or for correction].

BACKGROUND: With an increasing number of operative procedures for CTS, the number of reoperations is increasing too. These procedures are not in general performed because of recurrence, other reasons may play a role for the failure of the initial operation or recurrent symptoms. METHODS AND CLINICAL MATERIAL: Revision procedures performed in 57 patients in the practice for peripheral neurosurgery from January to September 2004 were analyzed for incision, intraoperative findings, method of operation, electrophysiological findings, and revised diagnosis. Moreover, the data were compared with 185

Reoperative inflammatory bowel disease surgery.

With the advent of restorative proctocolectomy or ileal pouch-anal anastomosis (IPAA) for ulcerative colitis (UC), not only has there been potential for cure of UC but also patients have enjoyed marked improvements in bowel function, continence, and quality of life. However, IPAA can be complicated by postoperative small bowel obstruction, disease recurrence, and pouch failure secondary to pelvic sepsis, pouch dysfunction, mucosal inflammation, and neoplastic transformation. These may necessitate emergent or expeditious elective reoperation to salvage the pouch and preserve adequate function.

Results of reoperations in colorectal anastomotic strictures.

PURPOSE: The incidence of colorectal anastomotic strictures varies from 3 to 30 percent. Most of these anastomotic strictures are simple narrowings shorter than 1 cm that can be successfully treated by dilation or endoscopic alternatives. However, up to 28 percent of patients will require surgical correction. This can be technically difficult, with the possibility of a permanent colostomy. This study reports the outcomes after operative treatment of severe strictures of colorectal anastomoses. METHODS: From August 1992 to October 1996, 27 patients were referred for surgical treatment of sever

Anesthetic methods in reoperative carotid surgery.

It has been suggested that general anesthesia is the preferred method for reoperative carotid surgery for several reasons, including: the difficulty of the reoperative dissection; the disease may extend unusually high into the internal carotid artery; and the reconstruction required may be more complex than a typical endarterectomy. The purpose of this study is to show that reoperative carotid surgery can be performed safely under regional anesthesia. The records of 109 reoperative carotid operations performed on 96 patients over the past 25 years were reviewed. Procedures performed under regi

Reoperative parathyroidectomy in the era of localization technology.

BACKGROUND: Recurrent and/or persistent hyperparathyroidism (HPT) is an uncommon disease. Relatively few cases are seen by any one center or surgeon. Most of the prior reviews of this problem were done in the era prior to potentially accurate magnetic resonsance imaging (MRI) and sestimibi scan localization and do not reflect current preoperative localization technology. METHODS: All cases of recurrent or persistent parathyroidectomy seen in our institution between 1992 and 1996 were reviewed retrospectively to assess the predictive value of preoperative MRI, selective venous sampling, sestim

Mini-reoperative mitral valve surgery.

BACKGROUND: Reoperative surgery involving the atrioventricular valves places the patient at risk for cardiac or bypass graft injury upon reoperative sternotomy. Standard right thoracotomy can avoid these problems but is associated with a large incision and possibly more pulmonary complications. METHODS AND RESULTS: An alternative, minimally invasive approach for reoperative atrioventricular valve surgery was studied in 22 patients. Patient age was 66 +/- 10 years. Postoperative mitral regurgitation was 3.4 +/- 0.3 and New York Heart Association (NYHA) Class was III/IV, despite a mean ejection

Reoperations on the aortic root and ascending aorta.

BACKGROUND: Reoperations on the aortic root and the ascending aorta are being performed with increasing frequency and remain a challenging problem. METHODS: Eighty-one patients (age range, 14 to 78 years) underwent reoperations on the aortic root or the ascending aorta during a 14.5-year interval ending October 1996. The previous operations were aortic valve procedure (28%), coronary artery bypass grafting (25%), aortic root replacement (24%), ascending aortic replacement (19%), and miscellaneous (5%). Twenty-two patients (27%) had had two or more previous operations. The principal indication

Studies on reoperations of extrahepatic biliary tree.

Cholelithiasis is one of the most common health disorders, but biliary surgery is burdened with numerous specific complications. These reoperations may be divided into early and late reoperations in terms of their precise therapeutic and prognostic differentiation. Recently, residual stones have been successfully treated by endoscopic sphincterotomy and percutaneous or trans-T-tube tract choledochoscopy, and reoperation is not frequently required. The purpose of this study was to identify the most common complications of biliary surgery which require reoperation so as to establish the predispo
