Evaluation of the wheezing infant.

Wheezing is a very common symptom or sign in the infant. The evaluation of the underlying cause of the wheezing is difficult and requires a thorough assessment of several factors in the history and physical examination and a basic understanding of the pathophysiology of wheezing. With the increased availability of infant pulmonary function equipment, the evaluation of serial pulmonary function studies in infants with wheezing should prove helpful in continued assessment of the reversibility of their airway obstruction using bronchodilators and the effect of antiinflammatory pharmacologic agent

[Indications for artificial ventilation in status asthmaticus, adult respiratory distress syndrome and pneumonia].

Mechanical ventilation is indicated in acute respiratory failure, especially in so-called pump failure as occurs in status asthmaticus, pneumonia and ARDS due to respiratory muscle fatigue. Using clinical parameters (inspiratory paradox, respiratory alternans), together with blood gas analysis and chest X-ray morphology, the indication can be established on a rational basis. The aims of therapy are tissue oxygenation and cure of the underlying disease which has led to respiratory failure. By adapting ventilator settings to the respiratory mechanics of the individual patient, complication due t

Predictors of respiratory decline in neuromuscular disorders.

Neuromuscular diseases (NMDs) constitute a heterogeneous group of genetic and acquired disorders that lead to progressive muscle weakness. When the respiratory muscles are involved, impaired ventilation and ineffective airway clearance become a major source of morbidity and mortality. Respiratory insufficiency is one of the major contributors to mortality in NMDs, with considerable variation in onset and rate of progression across different diseases. Patients with NMD have progressive weakness of the diaphragm and accessory respiratory muscles. This weakness leads to reduced chest wall complia

Respiratory disease in foundry workers.

A survey was carried out in a steel foundry in Brisbane to evaluate the nature and frequency of respiratory symptoms and to assess ventilatory function. The foundry used many moulding processes including the Furane, Isocure, Shell, carbon dioxide, and oil sand systems. Nasal symptoms and wheeze were often reported, particularly by workers in the general foundry and core shop, and on a semiautomated line. By contrast, workers in the aftercast section not exposed to fumes or vapours from the various moulding processes reported these symptoms less often. Of 46 workers exposed to moulding fumes an

A novel physiological investigation of the functional residual capacity by the bias flow nitrogen washout technique in infants.

The dynamic functional residual capacity (FRC(dyn)), the lung volume most routinely measured in infants, is an unreliable volume landmark. In addition to the FRC(dyn), we measured the (passive) static FRC (FRC(st)) by inducing a brief post-hyperventilation apnea (PHA) in 33 healthy infants aged 7.4-127.2 weeks. A commercial system for nitrogen (N2) washout to measure FRC, and a custom made system to monitor and record flow and airway opening pressure signals in real-time were used in unison. Infants were manually hyperventilated to induce a PHA. After the last passive expiration, FRC(st) was e

Clinical utility of preoperative pulmonary function testing in pediatrics.

Perioperative respiratory adverse events pose a significant risk in pediatric anesthesia, and identifying these risks is vital. Traditionally, this is assessed using history and examination. However, the perioperative risk is multifactorial, and children with complex medical backgrounds such as chronic lung disease or obesity may benefit from additional objective preoperative pulmonary function tests. This article summarizes the utility of available pulmonary function assessment tools as preoperative tests in improving post-anesthetic outcomes. Currently, there is no evidence to support or dis

Mycoplasma pneuminia infection. A follow-up study of 50 children with respiratory illness.

Fifty children with a previous history of Mycoplasma pneumoniae respiratory tract infection were assessed clinically, and pulmonary function tests carried out after an interval ranging from 1 1/2 to 9 1/2 years (median 2 1/2). 23 suffered from recurrent wheezy bronchitis or asthma, and in 5 the index illness appeared to precipitate the wheezing tendency. All were symptom-free when respiratory function tests were performed. Simple tests of ventilatory function (PEFR, FEV, and FVC) were within normal limits. Increased bronchial reactivity after exercise (a fall in PEFR greater than 15% resting v

Effectiveness of respiratory muscle training on pulmonary function recovery in patients with spinal cord injury: a systematic review and meta-analysis.

OBJECTIVE: This study conducted a thorough review and meta-analysis to examine how respiratory muscle training (RMT) affects lung function recovery in individuals with spinal cord injury (SCI). METHODS: We conducted a systematic review of Randomized Controlled Trials (RCTs) examining the effects of RMT on lung function in patients with SCI. The search included databases such as PubMed, Embase, The Cochrane Library, Scopus, and Web of Science up to October 2025. The experimental group received RMT as the main intervention, while the control group received either no treatment, a placebo, or con

[Cardiorespiratory fitness and arterial stiffness in sedentary and not sedentary hypertensive workers].

In hypertensive subjects, arterial stiffness and arterial wave early reflection are thought to be the major factor limiting cardiac reserve and exercise capacity. Aortic augmentation index (AIx) is a measure of arterial wave reflection and stiffness, and has been associated with decreased cardiorespiratory fitness. We investigated the role of physical activity at work and its effect on such association. 25 hypertensive middle-aged workers, without history of diabetes, cardiovascular disease, renal failure and inflammatory diseases, were studied. Our study confirms that AIx provides information

The relationship of airways responsiveness to cold air, cigarette smoking, and atopy to respiratory symptoms and pulmonary function in adults.

The response to eucapnic hyperventilation with subfreezing air was studied in a population based sample of 171 adults, all of whom also completed a respiratory questionnaire, spirometry, and skin testing. A positive response to the cold air challenge was defined as [initial FEV1-post-challenge FEV1)/initial FVC) X 100) greater than or equal to 9%. Cigarette smoking was with a positive cold air response: 12 of 128 current and ex-smokers (9.4%) versus 1 of 43 nonsmokers (2.3%) (p = 0.095). Among current and ex-smokers, a positive response to the cold air challenge was significantly associated wi

Lung clearance index in patients with cystic fibrosis: Can we avoid repeating the test three times?

Multiple breath washout (MBW) testing and its derived measurements (e.g., lung clearance index (LCI)) are useful for measuring ventilation heterogeneity in patients with cystic fibrosis, monitoring disease progression, and assessing the response to treatments. However, the use of MBW in routine clinical practice is limited by the long washout time and total test duration, especially in patients with more advanced disease. Recommendations are to complete three MBW repeats. In this analysis, we examine whether two acceptable repeats would provide an accurate assessment of the LCI and could there

Looking beyond LCI: Multiple breath washout phase III slope derived indices and their application in chronic respiratory disease in children.

The multiple breath washout (MBW) test is widely reported in the context of Lung Clearance Index (LCI). LCI reflects global ventilation inhomogeneity but does not provide information regarding the localization of disease along the respiratory tree. The MBW-derived normalized phase III slope (SnIII) indices (Scond and Sacin), instead, can distinguish between convective-dependent and diffusion-convection-dependent ventilation inhomogeneity considered to occur within the conductive and acinar airways, respectively. In cystic fibrosis, Scond tends to become abnormal even earlier than LCI and spiro

Breathing Pattern Disorder: Introduction and Diagnostics.

Breathing pattern disorder (BPD) refers to a heterogenous condition, which features altered normal respiratory functioning that cannot be fully attributed to organic causes at the current time. Characteristic symptoms of this condition include dyspnea, and irregular ventilation. The diagnosis of BPD is made utilizing a combination of patient-reported experiences in the form of validated questionnaires, direct observation by experienced clinicians, and objective testing in the form of cardiopulmonary exercise testing.
