Alterations in respiratory heart rate variability in brain-injured neurointensive care unit patients compared with healthy humans.

OBJECTIVE: Respiratory heart rate variability (RespHRV), the variation in heart rate in phase with breathing, is mainly generated by central brainstem mechanisms. Its characteristics in brain-injured patients are poorly understood. The objective of the study was to characterize RespHRV amplitude and phase in brain-injured patients compared to healthy participants. METHODS: We analyzed 55 brain-injured patients and 31 healthy controls. Physiological signals were recorded and processed to extract RespHRV amplitude and phase. Group differences were assessed using Mann-Whitney and Watson-Williams

Improving Obstructive Sleep Apnoea Mitigates Dampened Heart Rate Responses to Respiratory Events in Children With Down Syndrome.

Children with Down syndrome (DS) have a dampened heart rate (HR) response at obstructive respiratory event termination compared with typically developing children. Whether improving obstructive sleep apnoea (OSA) severity improves the HR response to both obstructive and central events remains unknown. Twenty-four children (3-19 years at baseline) were included. Children were grouped into improved (decrease in obstructive apnoea-hypopnoea index to ≤ 50% of baseline; n = 12; seven treated between studies) and unimproved (n = 12; two treated between studies) 2 years following baseline study. Beat

Heart rate surge at respiratory event termination in preterm and term born children with sleep disordered breathing.

BACKGROUND: Repetitive surges in heart rate (HR) at respiratory event termination underpin the altered autonomic HR control associated with sleep disordered breathing (SDB). As children born preterm are at greater risk of adverse cardiovascular outcomes, we aimed to determine whether the HR response to obstructive respiratory events was elevated compared to term-born children. METHODS: Fifty children (3-12 years) born preterm, were matched for SDB severity, age and gender with term born children. Multilevel modelling determined the effect of preterm birth and arousal on HR changes between a 1

Increasing Cytomegalovirus Detection Rate from Respiratory Tract Specimens by a New Laboratory-Developed Automated Molecular Diagnostic Test.

Lots of automated molecular methods for detecting cytomegalovirus (CMV) DNA in the blood are available, but seldom for various clinical specimens. This study was designed to establish a highly sensitive automated assay to detect CMV DNA in non-blood specimens. We designed a new QMT assay using QIAGEN artus CMV RG polymerase chain reaction (Q-CMV PCR) kit applied on the BD MAX system and compared with the other assays, including an RGQ assay (LabTurbo auto-extraction combined Q-CMV PCR kit on Rotor-Gene-Q instrument), and in-house PCR assay. A total of 1067 various clinical samples, including 4

Long-Term Survival Rate in Patients With Acute Respiratory Failure Treated With Noninvasive Ventilation in Ordinary Wards.

OBJECTIVE: Noninvasive ventilation is a life-saving technique increasingly used to treat acute respiratory failure. Noninvasive ventilation has been applied mostly in ICUs, but several reasons brought to an increasing application of noninvasive ventilation in ordinary wards. Few articles evaluated the outcomes of patients receiving noninvasive ventilation including long-term follow-up. The aim of the present study was to assess 1-year survival rate of patients treated with noninvasive ventilation outside the ICU for acute respiratory failure of heterogeneous causes and to identify the predicto

[Respiratory day hospital: Impact on the rate of hospital admissions due to chronic obstructive pulmonary disease exacerbations].

BACKGROUND AND OBJECTIVE: Acute exacerbations of chronic obstructive pulmonary disease (AE-COPD) are considered to be the most important factor in determining the course of the disease, and have great impact on the health care system. We evaluated the effects of a Respiratory Day Hospital (RDH) on the rate of hospital admissions due to AE-COPD. PATIENTS AND METHODS: During 14 months, every case of AE-COPD seen at the RDH and emergency services was recruited prospectively. Patients were excluded when another known cause of exacerbation was present and/or when they required invasive mechanical

Traumatic respiratory insufficiency: comparison of conventional mechanical ventilation to high-frequency positive pressure with low-rate ventilation.

Eleven patients suffering severe traumatic respiratory insufficiency were mechanically ventilated using a new system which combined high-frequency positive-pressure ventilation (HFPPV) with low-rate conventional mechanical ventilation (LRCMV). Ten similar patients were ventilated by conventional mechanical ventilation (CMV) with PEEP. HFPPV patients were fully conscious and cooperative during ventilation and did not need sedatives or muscle relaxants. Arterial oxygenation was significantly (p less than .005) better in HFPPV than CMV patients (89.91 +/- 10.24 vs. 78.43 +/- 11.13 torr, respectiv

INFLUENZA : III. RAPID ALTERATIONS IN THE RESPIRATORY RATE OF EMBRYONATED EGGS APPARENTLY CAUSED BY INFLUENZA VIRUS TOXIN.

Allantoic fluid from embryonated eggs infected with influenza A virus contains a toxic agent which can be demonstrated and quantitatively measured by its rapid effect on oxygen consumption when it is introduced in new series of fertile eggs. The effects were measured 90 minutes after the injection of the infected fluid, and were seen following both intra-allantoic injection and injection into the yolk sac. This toxin, in concentrations resulting from the injection of 0.5 cc. or less of the infected fluid, has no effect on oxygen consumption. The injection of 0.75 to 2.0 cc. of the fluid striki

Respiratory phase shapes brain-heart dynamics: evidence from recurrence analysis of EEG band power and heart rate.

INTRODUCTION: Respiration is increasingly recognized as a key modulator of brain-body interactions, influencing both neural and autonomic dynamics. While prior work has demonstrated respiratory-driven entrainment, it remains unclear whether distinct phases of the breathing cycle give rise to different dynamical regimes of cortical activity. METHODS: In this study, we investigated respiratory-phase-dependent modulation of brain-heart interactions during controlled breathing, focusing on breath-hold periods following inhalation and exhalation. EEG band power time series and heart rate (HR) were

HRV-Based Respiratory-Rate Estimation in Older Adults: Error Modeling and Pilot Validation for Wearable Sleep Monitoring.

Respiratory frequency is a critical biomarker in sleep medicine and circadian biology. We investigated whether the high-frequency (HF) component of heart-rate variability (HRV)-which reflects respiratory sinus arrhythmia (RSA)-can serve as a non-invasive proxy for breathing rate estimation from ECG or PPG. We employed a two-stage validation design: (1) a physiologically calibrated simulation study (N = 30 per condition, five conditions, 180 s recordings) for controlled error characterization using the PhysioNet ECG-ID Database (Electrocardiogram Identification Database, DOI: 10.13026/C2XW26) a

Differential Effects of Sleep Respiratory Event Types on Heart Rate Variability: Central Apnea as the Most Significant.

Background: Sleep-disordered breathing (SDB) is frequently accompanied by autonomic nervous system (ANS) dysfunction, which is closely associated with an increased incidence of cardiovascular diseases and elevated mortality risk. Heart rate variability (HRV) serves as a classic metric for evaluating sympathovagal balance; however, the specific impacts of four distinct types of respiratory events-obstructive apnea (OA), central apnea (CA), mixed apnea (MA), and hypopnea (HYP)-on HRV remain underinvestigated. Utilizing ultra-short-term HRV analysis, this study aimed to evaluate the immediate eff

Dyspnea is severe and associated with a higher intubation rate in de novo acute hypoxemic respiratory failure.

BACKGROUND: Dyspnea is a key symptom of de novo acute hypoxemic respiratory failure. This study explores dyspnea and its association with intubation and mortality in this population. METHODS: This was a secondary analysis of a multicenter, randomized, controlled trial. Dyspnea was quantified by a visual analog scale (dyspnea-VAS) from zero to 100 mm. Dyspnea was measured in 259 of the 310 patients included. Factors associated with intubation were assessed with a competing risks model taking into account ICU discharge. The Cox model was used to evaluate factors associated with 90-day mortality

Respiratory Rate as a Factor in Lung Injury-Not Just What You Set, but How You Set.

Tubal abortion is characterized by the extrusion of the foetus into the abdominal (peritoneal) cavity. It can either be a complete extrusion or incomplete with residual tissue remaining in the fallopian tube. It is a type of ectopic pregnancy that is difficult to determine the exact incidence of tubal pregnancies. Identifying cases of tubal abortions is crucial for individualized care since it can lead to a more conservative treatment approach. The diagnosis should be based on ultrasound imaging, b-hCG levels and visual conformation during exploratory surgery, either open or laparoscopic. The

Continuous heart rate dynamics preceding in-hospital pulseless electrical activity or asystolic cardiac arrest of respiratory etiology.

INTRODUCTION: Respiratory failure is a common cause of pulseless electrical activity (PEA) and asystolic cardiac arrest, but the changes in heart rate (HR) pre-arrest are not well described. We describe HR dynamics prior to in-hospital cardiac arrest (IHCA) among PEA/asystole arrest patients with respiratory etiology. METHODS: In this retrospective study, we evaluated 139 patients with 3-24 hours of continuous electrocardiogram data recorded preceding PEA/asystole IHCA from 2010-2017. We identified respiratory failure cases by chart review and evaluated electrocardiogram data to identify patt

Spontaneous and Stimulus-Evoked Respiratory Rate Elevation Corresponds to Development of Allodynia in Spinal Cord-Injured Rats.

Respiratory complications frequently accompany spinal cord injury (SCI) and slowed breathing has been shown to mitigate pain sensitivity. It is possible that elevated respiratory rates (RRs) signal the emergence of chronic pain after SCI. We previously validated the use of remote electric field sensors to noninvasively track breathing in freely behaving rodents. Here, we examined spontaneous (resting) and stimulus-evoked RRs as potential indices of mechanical hypersensitivity following SCI. Adult male Long-Evans rats received a lower thoracic hemisection or contusion SCI, or sham surgery, and

Does caffeine improve respiratory rate during remifentanil target controlled infusion sedation? A case report in endoscopic sedation.

Sedation for endoscopic procedures may be challenging when facing patients with high risk. Traditional techniques, as propofol or meperidine/midazolam administration, cannot ensure an adequate level of safety and efficacy for these patients. Remifentanil infusion is a common alternative, but the incidence of apneic events does not allow achieving safely a good level of analgesia. To overcome with this issue, the authors borrowed suggestions from other medical fields. The clinical practice has recognized a wide utility of methylxanthines (caffeine, theophylline, etc). The positive effect of caf
