Seclusion and restraint in patients with schizophrenia: clinical and biographical correlates.

Seclusion and restraint represent adverse experiences that cause negative attitudes against psychiatric treatment and psychopathologic sequels such as posttraumatic stress disorder. We examined 117 consecutive admissions with schizophrenia, with an average of 8.7 previous admissions. Positive and Negative Syndrome Scale and Global Assessment of Functioning were obtained at admission and discharge, and traumatic events in the biography were recorded using the Posttraumatic Diagnostic Scale. Twenty-four men (42.9%) and 18 women (29.0%) had experienced seclusion or restraint in their psychiatric

Interactions between corticotropin-releasing factor and endogenous opiates on the cardioaccelerator, hypothermic, and corticoid responses to restraint in the rat.

These experiments investigated the effect of either systemic opiate blockade by naloxone (5 mg/kg) or intracerebroventricular CRF (250 pmol), or the two treatments combined, on physiological and endocrine responses of male rats to two types of stress: restraint by itself (representing a psychological stress), and restraint combined with a tail clip (representing an additional mild physical nociceptive stress). Rats were restrained in a plastic container for 15 min, with or without a tail clip. Heart rate, body temperature, and serum corticosterone were measured. The first experiment showed tha

Seclusion and Restraint in Emergency Settings: A Narrative Review.

In psychiatry, restraints are considered last resort interventions to manage agitation and aggression. They are sometimes necessary, however, to ensure patient and staff safety. There are several types of restraints, including seclusion and various forms of physical and chemical restraints. While national guidelines emphasize using the least restrictive intervention, there is little empirical data to support the relative restrictiveness and safety of different restraint modalities. In this narrative perspective, we examine the rates, risks, and perceived relative benefits of these intervention

Cultivating moral sensitivity and restraint-free practice in nurses and midwives.

Nursing and midwifery students are frequently exposed to physical restraint during clinical training, often presented as necessary for patient safety. However, growing evidence challenges this assumption, highlighting the risks associated with restraint and the limited implementation of effective alternatives in practice. Studies show that alternatives are considered in only a minority of cases, while organisational and educational factors contribute to the persistence and normalisation of restraint. Although professional codes define restraint as an exceptional, last-resort measure, this prin

Short-term physical limitations in children following motor vehicle crashes.

This study describes frequency of injury and short-term physical limitation among child occupants </=15 years in motor vehicle crashes and examines the association between age, restraint use, seating position, and type of crash on the presence of physical limitations. Conducted from 1/1/2005-11/30/2007, as part of a child-specific crash surveillance system in 15 U.S. states; data were collected using claims records and parent/driver telephone surveys. Respondents were asked whether children sustained physical limitations from the crash and the duration limitations persisted. Overall, 3.3% had>

Clinical decision support system and incidence of delirium in cognitively impaired older adults transferred to intensive care.

BACKGROUND: Elderly patients with cognitive impairment are at increased risk of developing delirium, especially in the intensive care unit. OBJECTIVE: To evaluate the efficacy of a computer-based clinical decision support system that recommends consulting a geriatrician and discontinuing use of urinary catheters, physical restraints, and unnecessary anticholinergic drugs in reducing the incidence of delirium. METHODS: Data for a subgroup of patients enrolled in a large clinical trial who were transferred to the intensive care units of a tertiary-care, urban public hospital in Indianapolis we

Influence of maternal stress on the effects of prenatal exposure to methylmercury and arsenic on postnatal development and behavior in mice: a preliminary evaluation.

The present study combined maternal restraint stress with exposure to 2 environmental toxic elements, mercury and arsenic, given to mice concurrently with the restraint period (1000-1200 h, gestational days 15-18). Two groups of animals were given, by gavage, methylmercury chloride (MMC) (2 mg/kg/day), and 2 additional groups received sodium arsenite (10 mg/kg/day) on days 15-18 of gestation. Immediately after MMC or arsenite exposure, 1 group of MMC-treated mice and 1 group of arsenite-treated animals were restrained for 2 h/day. Control groups included restrained and unrestrained pregnant mi

Agitated Psychiatric Patient.

AUDIENCE: This scenario was developed to educate emergency medicine residents about the diagnosis and management of the agitated psychiatric patient. INTRODUCTION: The prevalence of agitation among patients in the emergency department is increasing, with an estimated 1.7 million events occurring annually in the United States.1 There are various methodologies for de-escalation, including verbal and chemical de-escalation and physical restraints. Chemical and/or physical restraints are sometimes necessary to ensure patient and staff safety when verbal de-escalation is ineffective, particularly

Outcomes of Restrictive Practice Review Meetings in an Acute Mental Health Unit: A Retrospective Before-and-After Study.

A structured process of reviewing incidents of seclusion and physical restraint supports nurses in managing the emotional and relational impact of restrictive practices. However, these reviews are not a routine feature of everyday acute care provision, and have historically been influenced by practices that reflect a managerial or disciplinary focus. A retrospective before-and-after study was conducted in an adult acute mental health inpatient unit in regional New South Wales, Australia. The goal was to determine the impact of strengths-based restrictive practice review meetings from January 2

Chronic stress increases estrogen and other steroids in inseminated rats.

Physical restraint, like many other stressors, can block early pregnancy, but the underlying physiological mechanisms have not been established. Exogenous estrogens in minute doses will also block early pregnancy. In the present study, female rats were exposed to 5 h of restraint daily for the first 5 days after insemination. A subset of animals was sacrificed after 1, 2, 3, 4, and 5 days of restraint, and blood was collected for radioimmunoassay. Blood was also collected from unstressed control animals on each of the first 5 days after insemination. Plasma concentrations of estradiol, as well

Effect of a Collar and Harness on Intraocular Pressure and Respiration Rate of Brachycephalic and Dolichocephalic Dogs.

BACKGROUND: Dogs are a popular pet in many countries, and for them to gain appropriate exercise, many owners opt to walk them on a leash. Despite health and welfare concerns, brachycephalic breeds remain common as pets, with limited research existing that investigates the best restraint type for these animals. OBJECTIVES: This study aimed to test the effect of a collar and harness during stationary and exercise conditions on the intra-ocular pressure (IOP) and respiration rate (RR) of brachycephalic and dolichocephalic dogs. METHODS: A total of 24 healthy dogs, both brachycephalic and dolich

Mild and Moderate Traumatic Brain Injury and Repeated Stress Affect Corticosterone in the Rat.

Traumatic brain injury (TBI) survivors suffer from a range of morbidities, including post-traumatic endocrinopathies that can cause physical and mental changes in patients, greatly compromising quality of life. This study tested the hypothesis that mild and moderate diffuse TBI leads to chronic deficiencies in corticosterone (CORT) regulation following repeated exposure to restraint stress over time. Young adult male rats (n = 9-11/group) were subjected to mild or moderate TBI induced by midline fluid percussion injury (mFPI) or control sham surgery. At 6 and 24 h post-injury, both mild and mo

The mediating role of eating behaviors in the association between adverse childhood experiences and central obesity among young adults.

Emerging evidence highlights the close association between eating behaviors and psychological factors, particularly in the context of obesity development. Although adverse childhood experiences (ACEs) are known to exert profound and lasting impacts on both physical and mental health, current research has not yet established conclusive evidence regarding their specific effects on young adult eating behaviors and subsequent obesity risk. This study therefore aims to address this knowledge gap by examining the association between ACEs and central obesity in young adults, with particular focus on

Prone restraint cardiac arrest in in-custody and arrest-related deaths.

We postulate that most atraumatic deaths during police restraint of subjects in the prone position are due to prone restraint cardiac arrest (PRCA), rather than from restraint asphyxia or a stress-induced cardiac condition, such as excited delirium. The prone position restricts ventilation and diminishes pulmonary perfusion. In the setting of a police encounter, metabolic demand will be high from anxiety, stress, excitement, physical struggle, and/or stimulant drugs, leading to metabolic acidosis and requiring significant hyperventilation. Although oxygen levels may be maintained, prolonged re

Trends in the use of coercive measures in Finnish psychiatric hospitals: a register analysis of the past two decades.

BACKGROUND: Coercive measures is a topic that has long been discussed in the field of psychiatry. Despite global reports of reductions in the use of restraint episodes due to new regulations, it is still questionable if practices have really changed over time. For this study, we examined the rates of coercive measures in the inpatient population of psychiatric care providers across Finland to identify changing trends as well as variations in such trends by region. METHODS: In this nationwide registry analysis, we extracted patient data from the national database (The Finnish National Care Reg

Diagnostic pitfalls and forensic implications of hyperactive delirium with severe agitation: an Italian case series.

Hyperactive Delirium with Severe Agitation (HDSA) is characterized by disorientation and aggressive behavior, constituting an acute, life-threatening medical emergency that requires prompt intervention. It is frequently associated with substance use. In prehospital settings, management often involves physical restraint, force, or pharmacological sedation; however, the diagnostic criteria and medico-legal implications of HDSA remain insufficiently defined. We retrospectively analyzed seven fatal cases exhibiting HDSA presentations, as defined by the American College of Emergency Physicians (ACE
