Abnormalities of the thoracic bellows: stress fractures of the ribs and hiatal hernia.

The thoracic bellows mechanism consists of the rib cage and the diaphragm. The purpose of this study was to determine if nontraumatically acquired lesions of the bellows were secondary to underlying disease. Abnormalities of the bellows, specifically stress fractures of the ribs and hiatal hernia, were found in 21 dogs and cats with underlying cardiopulmonary disease, neuromuscular disease, or metabolic disease. A case-control study of Bulldogs demonstrated that hiatal hernia was associated with the more severe manifestations of brachycephalic syndrome. Stress fractures occurred mostly in fema

Correlation analysis between the pulmonary function test and the radiological parameters of the main right thoracic curve in adolescent idiopathic scoliosis.

BACKGROUND: Scoliosis causes thoracic deformities, and it is necessary to assess these changes in pulmonary function test (PFT). To determine how measurements of spinal and thoracic cage deformities are related to pulmonary function. METHODS: Seventy-two patients with main right thoracic curvature in adolescent idiopathic scoliosis (AIS) underwent a PFT and a radiological parameter evaluation of spinal and thoracic cage deformities. Simple and multiple linear regressions were also used to note whether a combination of variables might better predict PFT values. Means were compared using the tw

Multidetector CT findings of skeletal chest injuries secondary to cardiopulmonary resuscitation.

INTRODUCTION: Rib and sternal fractures are frequent complications of cardiopulmonary resuscitation (CPR) in adults. This is the first study to evaluate the MDCT findings of chest injuries secondary to CPR, by comparing with the findings of radiography. METHODS: For 40 patients who underwent MDCT after CPR for a non-traumatic cause of cardiac arrest, we evaluated the MDCT findings of the CPR associated traumatic chest injuries and compared the diagnostic performance of chest radiography and MDCT for the evaluation of chest injuries. RESULTS: MDCT revealed that 26 patients (65%) had rib fract

A cortical thickness and radiation dose mapping approach identifies early thinning of ribs after stereotactic body radiation therapy.

BACKGROUND AND PURPOSE: High rates of spontaneous rib fractures are associated with thoracic stereotactic body radiation therapy (SBRT). These fractures likely originate within the cortical bone and relate to the cortical thickness (Ct.Th). We report the development and application of a novel Ct.Th and radiation dose mapping technique to assess early site-specific changes of cortical bone in ribs. MATERIALS AND METHODS: Rib Ct.Th maps were constructed from pre-SBRT and 3month post-SBRT CT scans for 28 patients treated for peripheral lung lesions. The Ct.Th at approximately 50,000 homologous p

Action of the isolated canine diaphragm on the lower ribs at high lung volumes.

The normal diaphragm has an inspiratory action on the lower ribs, but subjects with chronic obstructive pulmonary disease commonly have an inward displacement of the lateral portions of the lower rib cage during inspiration. This paradoxical displacement, conventionally called 'Hoover's sign', has traditionally been attributed to the direct action of radially oriented diaphragmatic muscle fibres. In the present study, the inspiratory intercostal muscles in all interspaces in anaesthetized dogs were severed so that the diaphragm was the only muscle active during inspiration. The displacements o

Predictive Modelling of Waist Reduction in the RibXcar Technique: Correlation Between Angularity of Monocortical Fracture and Aesthetic Result.

BACKGROUND: Aesthetic rib remodeling redefines the waistline through structural modification of the lower rib cage. Ultrasound-guided percutaneous RibXcar reduces rib angularity without skin incisions and contributes to waist narrowing. However, the relationship between rib angulation changes and long-term waist reduction remains insufficiently quantified. OBJECTIVES: To evaluate the association between perioperative waist measurements, ultrasound-based rib angulation, and waist circumference at 12 months, and to develop a clinically applicable formula for expected waist reduction. METHODS:

The spine and surgical treatment in osteogenesis imperfecta.

In the osteogenesis imperfecta patient, spinal deformity should be expected, particularly in the severely affected nonambulator with chest deformity. Curves develop early (age five to six) and generally progress rapidly. Early bracing, although somewhat effective, may well compress the soft osteoporotic rib cage without controlling the spinal curvature. The pulmonary compromise created by the scoliosis is compounded by the chest cage deformity secondary to bracing. In the patient with severe disease (thin bones and numerous fractures), posterior correction and fusion, with or without Harringto

[Thoracic epidural analgesic (TEA) or controlled ventilation in the treatment of patients with multiple rib fractures (author's transl)].

In the last seven years 283 trauma patients were treated for multiple rib fractures and/or flail chest. Primary management consisted of only morphine analgesia in 16 patients, TEA in 112 patients, and mechanical ventilation in 155 patients. The indication for mechanical ventilation was always associated injuries (cerebral contusion, para- and tetraplegia, aspiration, severe lung contusion) and not the instability of the thoracic cage.

Association between ribs shape and pulmonary function in patients with Osteogenesis Imperfecta.

The aim of the present study was to test the hypothesis that ribs shape changes in patients with OI are more relevant for respiratory function than thoracic spine shape. We used 3D geometric morphometrics to quantify rib cage morphology in OI patients and controls, and to investigate its relationship with forced vital capacity (FVC) and forced expiratory volume in 1 s (FEV1), expressed as absolute value and as percentage of predicted value (% pred). Regression analyses on the full sample showed a significant relation between rib shape and FEV1, FVC and FVC % pred whereas thoracic spine shape w

Experimental study exploring the factors that promote rib fragility in the elderly.

Rib fractures represent a common injury type due to blunt chest trauma, affecting hospital stay and mortality especially in elderly patients. Factors promoting rib fragility, however, are little investigated. The purpose of this in vitro study was to explore potential determinants of human rib fragility in the elderly. 89 ribs from 13 human donors (55-99 years) were loaded in antero-posterior compression until fracture using a material testing machine, while surface strains were captured using a digital image correlation system. The effects of age, sex, bone mineral density, rib level and side

From birth to triumph: A rare case report of rib chondrosarcoma with unprecedented growth patterns.

RATIONALE: Primary tumors of ribs are uncommon in clinical practice. These tumors can be benign or malignant and often present unique challenges in diagnosis and treatment. For instance, rib chondrosarcoma is a rare type of chondrosarcoma that occurs in the rib cage, representing a significant clinical diagnosis challenge due to its potential for local recurrence and metastasis. With rib chondrosarcoma being more or less common compared to other locations such as the long bones and pelvis. Generally, rib chondrosarcoma develops rapidly and mostly occurred in middle-aged or elderly crowds. Rib

Performance of thoracic ultrasonography compared with chest radiography for the detection of rib fractures using computed tomography as a reference standard.

OBJECTIVE: Although there is growing evidence that ultrasonography is superior to X-ray for rib fractures' detection, X-ray is still indicated as the most appropriate method. This has partially been attributed to a lack of studies using an appropriate reference modality. We aimed to compare the diagnostic accuracy of ultrasonography and X-ray in the detection of rib fractures, considering CT as the reference standard. MATERIALS AND METHODS: Within a 2.5-year period, all consecutive patients with clinically suspected rib fracture(s) following blunt chest trauma and available posteroanterior/an

Surgical treatment of thoracic deformity in Poland's syndrome.

In 1841, Poland described congenital deficiency of the pectoralis major and minor muscles associated with syndactyly. This syndrome is a spectrum, often involving chest wall and breast deformity as well. Identification of the various musculoskeletal components involved permits optimal thoracic reconstruction in the small proportion of patients who will require it. From 1955 to 1988, 75 patients (40 males and 35 females) with Poland's syndrome were treated or evaluated. Patients with isolated deficiencies of the pectoral muscles, breast, or hand deformity were excluded. The complex was right-si

Multi-scale engineering of biodegradable Zn-Mg interbody cages for stronger, faster spinal fusion.

Spinal interbody fusion cages must be able to bear heavy loads while integrating seamlessly with the surrounding bone. However, the cages currently used in spinal surgery often fall short on both fronts. To meet the multi-faceted requirements, here, we introduce, for the first time, additively manufactured, biodegradable Zn-Mg interbody fusion cages with multi-scale structural control, combining eutectic microstructure, heterogeneous grain architecture, and gyroid lattices reinforced by interpenetrating ribs. The resulting cages showed a compressive strength comparable to that of cortical bone

Mapping of common rib fracture patterns and the subscapular flail chest associated with operative scapula fractures.

BACKGROUND: Rib fractures occur in approximately 10% of trauma patients and are associated with more than 50% of patients with scapula fractures. This study investigates the location and patterns of rib fractures and flail chest occurring in patients with operatively treated scapula fractures. Novel frequency mapping techniques of rib fracture patterns in patients who also injure the closely associated scapula can yield insight into surgical approaches and fixation strategies for complex, multiple injuries patients. We hypothesize that rib fractures have locations of common occurrence when pre

Rib Healing and Heterotopic Ossification After Surgical Stabilization of Rib Fractures.

Background: Little is known about the rate of rib fracture healing after Surgical Stabilization of Rib Fractures (SSRFs). We sought to evaluate the radiographic evidence of rib healing and hypertrophic ossification (HO) in patients after SSRFs. Methods: A single-center retrospective cohort study was conducted on all patients who had undergone SSRFs from 1 January 2010 to 31 March 2023 and had a computed tomography (CT) of the chest performed greater than 6 months after SSRFs. The rib fracture locations were mapped on the initial trauma CT scan and evaluated on the follow-up CT scan for healing

Posterior Spinal Fusion With Rib Resection Allows for Improved Deformity Correction as Well as Patient Satisfaction.

STUDY DESIGN: Retrospective cohort study. OBJECTIVE: This study aimed to compare radiographic and clinical outcomes in adolescent idiopathic scoliosis (AIS) patients undergoing posterior spinal fusion (PSF) with and without thoracoplasty. BACKGROUND: Scoliosis is a three-dimensional deformity. As a result, patients often have an associated rib cage deformity, with clinical and aesthetic implications. Direct vertebral rotation (DVR) allows for some reduction of the rib hump; however, the deformed ribs remain deformed. Rib resection has been utilized to further reduce the rib hump, however, th

Thoracic trauma in children.

Isolated thoracic trauma is rare in children. Because of their small body size, the trauma often also affects other spaces, such as the abdomen and head, and these coexistences significantly increase the rate of mortality. However, in isolated thoracic traumas, the children can quickly recover if they can survive the initial period of trauma. Pediatric thoracic trauma cases can have a different clinical course compared to adults due to the unique anatomic and physiologic properties of children's thoracic cages. Their ribs are nonossified and are very elastic, and therefore, as their ribs can s

Nutritional Secondary Hyperparathyroidism and Fibrous Osteodystrophy in a Captive African Penguin (Spheniscus demersus) Similar to Osteomalacia in Poultry.

A 9-yr-old female black-footed African penguin (Spheniscus demersus) was presented for necropsy after a history of reproductive abnormalities, paresis of limbs, weakness, and sudden death. Postmortem examination revealed soft keel, collapsed rib cage with beading of the ribs, and bilateral parathyroid enlargement. Classic histologic lesions of fibrous osteodystrophy with osteomalacia were observed in the ribs, vertebrae, and to a lesser extent in the femur and tibiotarsus associated with hyperplasia of parathyroid glands. This represents the first report of nutritional secondary hyperparathyro

Frequency of intrathoracic injuries in children younger than 3 years with rib fractures.

BACKGROUND: Research documents that among children admitted to trauma intensive care units the number of rib fractures sustained indicates the child's likelihood of having and severity of intrathoracic injury. This has been misused in court to argue that children with multiple rib fractures who lack intrathoracic injury have abnormal bone fragility rather than inflicted injury. OBJECTIVE: To determine frequency of intrathoracic injuries in children younger than 3 years with rib fractures in cases of child abuse and accidental trauma. MATERIALS AND METHODS: We conducted a retrospective review
