Risk stratification in the management of atrial fibrillation in the community.

This study assessed whether risk stratification in patients with atrial fibrillation (AF) in the community had a bearing on the likelihood of receiving aspirin or warfarin therapy. Seven hundred and fifty patients were identified from 14 practices by means of diagnostic READ codes or repeat prescriptions for digoxin from practice computers. The study demonstrates that general practitioners appreciate the importance of antithrombotic therapy in patients who have suffered stroke, but take poor account of increasing age and other independent risk factors. A more proactive approach to risk identif

Management of otitis media in a high risk population.

BACKGROUND: Rural and remote Aboriginal and Torres Strait Islander children have extremely high rates of otitis media and hearing loss. Despite considerable evidence, clinical practice continues to vary. This may be partly related to the failure of recent guidelines to be explicit about which factors should influence decision making. OBJECTIVE: To provide rural and remote GPs caring for young Aboriginal and Torres Strait Islander children with an evidence based guide to the principles that determine the clinical management of otitis media and hearing loss. DISCUSSION: While population health

Assessing and managing risk of violence in the mentally disordered.

This review paper explored the concepts of risk assessment and management in relation to 'serious' violence by individuals with mental disorder. The paper highlighted the political context in which concern over serious violence by those with mental disorder is increasing. Government guidance on risk assessment was considered and identified as having an actuarial bias. Correlational studies, which provide actuarial indicators of risk, were explored and a picture was developed that demonstrated the difficulty of determining risk of violence to others simply on the basis of psychiatric symptomato

A risk-benefit assessment of corticosteroids in the management of croup.

Croup is an acute clinical syndrome of childhood characterised by a barking cough, hoarse voice, stridor and a variable degree of respiratory distress. A meta-analysis and subsequent controlled trials clearly demonstrate that corticosteroids are efficacious in the management of croup, with their benefits conclusively outweighing their risks. In mild to moderate cases of croup either systemic or nebulised corticosteroids decrease symptoms and need for hospitalisation. Most reports use IM dexamethasone 0.6 mg/kg, although it is likely that dexamethasone 0.15 mg/kg has a similar effect. In contro

Epilepsy and pregnancy: good management reduces the risks.

It is vital to review medication before pregnancy to minimise the risk of a malformed fetus. The risk of fetal malformation can be at least double for women on the commoner anticonvulsants, especially if these are taken in combination. Two newer anticonvulsants (gabapentin and lamotrigine) appear to be less harmful to the fetus. One-third of women with epilepsy have an increase in seizures during pregnancy. The fetus appears to be relatively resistant to the mother's seizures, though (rarely) intrauterine deaths have occurred. As well as fetal malformation, children born to women with epilepsy

Fetal macrosomia: prediction, risks, proposed management.

The outcome of delivery of infants weighing more than 4000 g born during two time periods 15 years apart was studied retrospectively. The increased use of cesarean section and other obstetric advances did not reduce the risk of fetal asphyxia and trauma associated with large fetal size. Maternal factors were identified for risk categorization of fetal macrosomia during pregnancy. Macrosomia was rare at 37 weeks and increasingly common thereafter. Fetal size assessment by ultrasound at 36 to 38 weeks' gestation would permit induction of labor for the macrosomic infant before the size became exc

Risk-benefit considerations in the management of polymyalgia rheumatica.

Management of the polymyalgia rheumatica syndrome (PMR) is controversial. Gratifying symptomatic response usually occurs after treatment with nonsteroidal antiinflammatory drugs or low doses of corticosteroids. However, some PMR patients are at risk of sudden blindness from an associated disease, giant cell arteritis (GCA). Blindness can be prevented by using higher and more toxic doses of corticosteroids. Temporal artery biopsy can be an aid in diagnosis, though it is not a completely sensitive test for GCA. This study employs decision analysis and derived risk-benefit ratios (equivalent to u

The role of assessing frailty in cardiometabolic risk management.

AIMS: Frailty is highly associated with morbidity and mortality in type 2 diabetes mellitus (T2DM). This viewpoint article reviews the importance of considering frailty in T2DM and cardiometabolic risk factor management and recommends its systematic integration into clinical practice. We propose that frailty is not merely a complication of metabolic disease, but an independent modifier of risk, treatment response, and outcomes. DATA SYNTHESIS: We conducted a literature review on the epidemiology of frailty in T2DM, bidirectional biological mechanisms linking frailty to T2DM and cardiometaboli

Dosage of lipoproteins for risk management according to guidelines.

LDL cholesterol (LDL-C) measurement is a key component in assessing cardiovascular disease risk and managing dyslipidemia. Despite irrefutable evidence that LDL-C-targeted strategies effectively reduce the risk of cardiovascular events, there is considerable variability among individuals in response to lipid-lowering therapies and in the resulting reduction of cardiovascular risk. Focusing solely on LDL-C assessment and management is no longer an optimal strategy for all patients. A major concern also lies in the potential for substantial errors in risk estimation, given the recognized measure

Candida in the ICU, Risk Management and Patient Safety.

Endogenous candidiasis remains an underrecognized yet clinically relevant complication in non-neutropenic critically ill patients. This study examines Candida spp. infections in the intensive care unit (ICU) within a patient-safety and risk-management framework, focusing on the identification of patients at highest risk and the development of an early diagnostic and therapeutic strategy. The target population comprises long-stay ICU patients requiring prolonged mechanical ventilation who develop multiple organ dysfunction syndrome (MODS) associated with immunoparalysis, typically reflected by

[Management of Risk Factors and Comobordities in Atrial Fibrillation].

Atrial fibrillation (AF) is the most common sustained arrhythmia and has gained increasing importance due to demographic changes. The development of AF is multifactorial, including both non-modifiable risk factors such as age and genetic predisposition, as well as modifiable factors like obesity and alcohol consumption. Common comorbidities include arterial hypertension, heart failure, sleep apnea syndrome, and diabetes mellitus. Current studies show that treatment of these risk factors and comorbidities prevents the progression of AF and improves the results of rhythm control therapy. Prevent

Olezarsen for Managing Severe Hypertriglyceridemia and Pancreatitis Risk.

BACKGROUND: Patients with severe hypertriglyceridemia have an increased risk of acute pancreatitis. The efficacy and safety of olezarsen, an antisense oligonucleotide targeting apolipoprotein C-III messenger RNA, have not been established in this population. METHODS: We conducted two double-blind, randomized, placebo-controlled trials (CORE-TIMI 72a and CORE2-TIMI 72b). Patients with severe hypertriglyceridemia were assigned in a 1:1:1 ratio to receive olezarsen at a dose of 50 mg, olezarsen at a dose of 80 mg, or placebo monthly for 12 months. The primary outcome was the percent change in th

Plozasiran for Managing Persistent Chylomicronemia and Pancreatitis Risk.

BACKGROUND: Persistent chylomicronemia is a genetic recessive disorder that is classically caused by familial chylomicronemia syndrome (FCS), but it also has multifactorial causes. The disorder is associated with the risk of recurrent acute pancreatitis. Plozasiran is a small interfering RNA that reduces hepatic production of apolipoprotein C-III and circulating triglycerides. METHODS: In a phase 3 trial, we randomly assigned 75 patients with persistent chylomicronemia (with or without a genetic diagnosis) to receive subcutaneous plozasiran (25 mg or 50 mg) or placebo every 3 months for 12 mo

Illness uncertainty and risk management for people with cancer.

BACKGROUND: Patients with cancer live with considerable uncertainty. This uncertainty can be related to the process of diagnosis, treatment, remission or palliative care, and therefore it can be experienced repeatedly or continuously throughout a patient's life. For patients with low literacy or low numeracy, it can be difficult to access, understand and interpret risk, so shared decision making may be difficult. OBJECTIVE: The aim of this article is to address the challenges of managing anxiety and uncertainty for patients with cancer in the general practice setting. DISCUSSION: The diagnos

A dynamic risk-based approach to managing a pandemic.

A pandemic is a unique natural disaster that will pose challenges for any organisation. During the COVID-19 pandemic, for example, organisations of all types have struggled to maintain operations while assuring the health and wellbeing of the various persons who work on their behalf. Certainly, many organisations have found that their risk management and business continuity plans fail to consider adequately the disruption associated with a pandemic caused by a novel pathogen. As this paper discusses, this suggests a need to revisit risk assessments and business impact analyses; the assumptions

Fragility Fractures: Risk Factors and Management in the Elderly.

Given the progressive ageing of Western populations, the fragility fractures market has a growing socioeconomic impact. Fragility fractures are common in the elderly, negatively impacting their quality of life, limiting autonomy, increasing disability, and decreasing life expectancy. Different causes contribute to the development of a fractures in frail individuals. Among all, targeting fragile patients before the development of a fracture may represent the greatest challenge, and current diagnostic tools suffer from limitations. This study summarizes the current evidence on the management of

Female athletes: managing risk and maximizing benefit.

PURPOSE OF REVIEW: The purpose of this review is to provide an updated summary on the unique aspects of caring for female youth and adolescent athletes. RECENT FINDINGS: Recent research on female athletes has led to a better understanding of injuries and conditions that are more likely to occur in female youth and adolescent athletes, including the frequency and severity of concussions, musculoskeletal injury such as involving the anterior cruciate ligament, and the female athlete triad. Social factors, such as the growing pressure to specialize in a sport particularly at an early age, also p
