The Use of Diagnostic Ultrasound for the Assessment of Rotator Cuff Tendinopathy: A Systematic Review.

BACKGROUND: Rotator cuff tendinopathy (RCT) is a common clinical diagnosis of pain and dysfunction in the shoulder. In conjunction with clinical tests, diagnostic ultrasound (US) is beneficial in the diagnosis of RCT through its ability to visualise tendon structural change. However, there is currently no standardised method to assess RCT with US. AIMS/PURPOSE: The primary aim of this systematic review was to identify the common US parameters used to diagnose RCT with US. The secondary aim was to determine whether a nominal or ordinal scale should be used to diagnose RCT with US. METHODS: A

Arthroscopic Repair of Rare Transtendinous Rotator Cuff Tear: Utilizing Established Portals and a Posterior Superior Accessory Portal.

There is a high incidence of rotator cuff injuries in the adult population, and there are well-described techniques for the most common types. Full-thickness transtendon rotator cuff tears are an uncommon injury, and there is limited literature on the appropriate treatment. These rare injuries are important to recognize due to the risk of repair failure using traditional techniques. Simply anchoring the medial portion of the tendon to the anatomic footprint after debriding the lateral tendon risks overtensioning the tendon. Previous techniques share the problem of poor visualization and diffic

Acellular porcine Achilles tendon patch encapsulating tendon-derived stem cells for rotator cuff repair in a rabbit model.

Currently, the predominant method for repairing rotator cuff involves surgical suture techniques, but the failure rate remains notably high. Failure of the rotator cuff insertion to provide adequate biomechanics during early healing is considered a major cause of failure. Addressing this problem, biological augmentation emerges as a promising strategy for enhancing the biomechanical properties during early stages. Tendon-derived stem cells (TDSCs), which facilitate the differentiation of repair-supportive cells, hold the potential to improve the efficacy of patch application. The study aims to

Volumetric classification: unveiling the true extent of rotator cuff tears.

INTRODUCTION: Rotator cuff injury diagnosis involves comprehensive clinical, physical, and imaging assessments, with magnetic resonance imaging (MRI) being pivotal for detecting and classifying these injuries. However, the absence of a universally accepted classification system necessitates a more precise approach, advocating for the use of three-dimensional (3D) modeling to better understand and categorize rotator cuff tears. METHODOLOGY: This research was conducted as a prospective, single-institution study on 62 patients exhibiting full-thickness rotator cuff tears. Utilizing preoperative

Primary traumatic anterior shoulder dislocation in patients 40 years of age and older.

During three consecutive ski seasons (1991 to 1994), 125 patients 40 years of age and older without previous shoulder injuries or surgery sustained a traumatic first-time anterior shoulder dislocation. At a minimum of 2 years' follow-up, patients were contacted to determine long-term outcome and to identify factors leading to prolonged morbidity or the need for surgical intervention. Fifty-two patients were available for interview. A modified Rowe shoulder score showed 32 excellent, nine good, eight fair, and three poor results. Eighteen (35%) rotator cuff tears were subsequently identified, w

Editorial Commentary: Static and Dynamic Synergy May Be the Evolving Role of Biceps and Tendon Transfers in the Treatment of Irreparable Rotator Cuff Tears.

The challenge of managing massive irreparable rotator cuff tears is not solved through technical complexity alone but through achieving balance between biological restoration and mechanical reinforcement. Superior capsule reconstruction using the long head of the biceps tendon offers static and dynamic advantages, whereas tendon transfers provide additional posterior force vectors to restore external rotation. The combined "static-dynamic" strategy may represent an evolving biologically based alternative for selected patients who are not candidates for arthroplasty.

Delamination in rotator cuff tears: Explanation of etiology through anatomical dissection.

The prognostic significance of delaminated rotator cuff tears remains controversial. However, as the surgical goal is to maximize the contact area between layers, the macroscopic appearance of partial delaminated rotator cuff tears is essential. The aim of this anatomical study was to investigate the morphology of delaminated rotator cuff tears. We hypothesized that delamination zones at the intersection of the supraspinatus and infraspinatus tendon fibers are the origin of articular-side degenerative rotator cuff tears. Forty anatomical specimens were evaluated in this study. The supraspinatu

Effect of rotator cuff muscle imbalance on forceful internal impingement and peel-back of the superior labrum: a cadaveric study.

BACKGROUND: Throwing athletes with shoulder pain have been shown to have decreased rotator cuff muscle strength. Shoulder internal impingement and labral peel-back mechanism, as may occur during the late cocking phase of throwing motion, are thought to cause rotator cuff injury and type II superior labrum anterior and posterior lesions. Therefore, the objective of this study was to assess the effect of rotator cuff muscle force on internal impingement and the peel-back of the superior labrum by quantifying maximum external rotation, glenohumeral contact pressure, and position of the cuff inser

Partial rotator cuff tears in adolescents: factors affecting outcomes.

INTRODUCTION: In the adult population, rotator cuff tears are common and established treatment methods yield satisfactory results. In adolescents, however, these injuries are uncommon and few treatment methods and outcome reports exist. The purpose of this study was to examine a series of adolescent rotator cuff tears, identify associated pathology, and report treatment outcomes. METHODS: A retrospective comparative analysis of adolescent patients treated for rotator cuff tears diagnosed by magnetic resonance imaging (MRI) or arthroscopy between 2008 and 2010 was performed. The prevalence of rotator cuff pathology in the setting of acute proximal biceps tendon rupture.

BACKGROUND: The prevalence and severity of concomitant rotator cuff pathology in the setting of proximal biceps tendon ruptures are poorly understood. Concomitant rotator cuff disease may have important implications in the prognosis and natural history of this shoulder condition. Therefore, an observational cohort of patients with an acute rupture of the long head of the biceps tendon (LHBT) was evaluated to determine the prevalence and severity of concomitant rotator cuff disease. METHODS: Thirty consecutive patients diagnosed with acute proximal biceps tendon rupture were prospectively enro

Subacromial Bursa: A Neglected Tissue Is Gaining More and More Attention in Clinical and Experimental Research.

The subacromial bursa has long been demolded as friction-reducing tissue, which is often linked to shoulder pain and, therefore, partially removed during shoulder surgery. Currently, the discovery of the stem cell potential of resident bursa-derived cells shed a new light on the subacromial bursa. In the meanwhile, this neglected tissue is gaining more attention as to how it can augment the regenerative properties of adjacent tissues such as rotator cuff tendons. Specifically, the tight fibrovascular network, a high growth factor content, and the large progenitor potential of bursa-derived cel
