Deep learning for automated grading of radiographic sacroiliitis.

BACKGROUND: Grading assessment of sacroiliitis via X-ray is widely used in clinical evaluation. The aim of this study was to develop and validate an artificial intelligence (AI) system to help physicians in assessing and diagnosing sacroiliitis from standard X-ray images. METHODS: In this retrospective study, a deep learning model for the automated grading assessment of radiographic sacroiliitis was developed using pelvic X-ray images from a training set of 465 individuals (930 single sacroiliac joints) and a validation set of 195 individuals (390 single sacroiliac joints). The algorithm was …

Severe tertiary hyperparathyroidism as a rare mimicker of sacroiliitis.

A 42-year-old female patient was referred for rheumatology evaluation due to suspected sacroiliitis, incidentally detected on a pelvic computerized tomography (CT). Her medical history was notable for chronic kidney disease (CKD) complicated with tertiary hyperparathyroidism. A few months before rheumatologic evaluation, she underwent total parathyroidectomy. CT imaging was reviewed revealing prominent erosions of the iliac articular surfaces of the sacroiliac joints. Given the absence of inflammatory low back pain and the patients history of severe hyperparathyroidism, the sacroiliac changes …

Sacroiliitis in children and adolescents with familial Mediterranean fever.

BACKGROUND: Familial Mediterranean fever (FMF) is an autoinflammatory disease characterized by recurrent episodes of fever and serositis. Sacroiliitis can be observed in some FMF patients. This study aimed to compare the demographic, clinical, and laboratory findings, and treatment in children with FMF and sacroiliitis, and children with juvenile spondyloarthropathy (JSpA). METHODS: In total, 1687 pediatric FMF patients that were followed-up between May 2010 and June 2020 were evaluated retrospectively. Among them, those with sacroiliitis (n = 63) were included in the study and compared to pa…

MRI Assessment of Sacroiliitis With High-Resolution Protocol.

Background: Sacroiliac (SI) joint subchondral resorption on high-resolution magnetic resonance imaging (MRI) may be an early sign of the development of sacroiliitis. At our institution, high-resolution intermediate-weighted (proton density) MRI sequences are used in the workup of suspected spondyloarthritis (SpA). Questions/Purpose: We sought to test the hypothesis that SI joint subchondral resorption might be a useful MRI feature in the diagnosis of sacroiliitis. Methods: We retrospectively reviewed the records of patients with suspected SpA from a single rheumatologist's practice from Januar…

Sacroiliitis in cancer patients: a review of current literature.

The sacroiliac (SI) joint can be directly jeopardized by malignancy and indirectly by ergonomic changes of pelvic obliquity that introduces … ructural changes that affect symmetric body functioning and posturing. No frank association between sacroiliitis and cancer has been identified. Therefore, we believe there is a need to characterize any relationship between cancer and SI joint dysfunction and pain.

MRI of the sacroiliac joints: what is and what is not sacroiliitis?

PURPOSE OF REVIEW: MRI has, as the only imaging modality, the ability to visualize both the inflammatory and destructive aspects of sacroiliitis and is a crucial element in the diagnosis and classification of axial spondyloarthritis (axSpA). However, the MRI appearance of several potential differential diagnoses may resemble axSpA sacroiliitis. RECENT FINDINGS: The appearances of sacroiliac joint (SIJ) MRIs of various diseased and healthy populations have recently been intensively studied. BME, the key requirement in the Assessment of Spondyloarthritis international Society (ASAS) definition …

Diagnostic dilemma in a case of Salmonella Typhi sacroiliitis.

Sacroiliitis is a rare complication of enteric fever, seen in <1% of the cases and its concomitant presence with hepatitis has been reported only once. Incorrect or delayed diagnosis of enteric fever may lead to serious complications. Here, we present a complicated case of enteric fever in a 15 years old female who was misdiagnosed elsewhere to be a case of dengue fever owing to thrombocytopenia at presentation along with a weak positive dengue IgM immunochromatography test. The patient eventually developed a rare combination of complications (sacroiliitis, hepatitis, ascites and pleural effus…

Imaging of sacroiliitis: Current status, limitations and pitfalls.

The clinical need to diagnose sacroiliitis at an earlier stage has led to the sacroiliac joints being more frequently imaged, particularly with magnetic resonance imaging (MRI). This review outlines the imaging approach to sacroiliitis, emphasizing the imaging protocols, diagnostic criteria, limitations and potential mimics of MRI examination. The value of imaging-guided intervention in sacroiliac joint disease is also briefly outlined.

Sacroiliitis in Axial Spondyloarthritis: Assessing Morphology and Activity.

OBJECTIVE: To review the strengths, limitations, and new insights in the anatomy and magnetic resonance imaging (MRI) features of active and structural lesions of sacroiliitis in spondyloarthritis. DISCUSSION: MRI plays a key role in the diagnosis and follow-up of sacroiliitis in spondyloarthritis. MRI of the sacroiliac joints in affected patients may show active lesions such as bone marrow edema, capsulitis, enthesitis, or synovitis as well as structural changes such as erosion, fat infiltration, sclerosis, backfill, and ankylosis. Active lesions of sacroiliitis on MRI are particularly impor…

Epithelioid sarcoma of the right ilium mimicking sacroiliitis.

Epithelioid sarcoma represents less than 1% of soft tissue sarcomas and has a high recurrence rate and strong ability to invade the regional lymphatic pathways. Classic epithelioid sarcoma has a predilection for young men and usually affects the distal extremities, while the proximal-type is characterized by different histological features and most commonly occurs in the proximal part of the extremities, trunk and pelvis. Intraosseous tumor development is rare. We describe a 47 year old patient with sacroiliac pain mimicking sacroiliitis. A computed tomography of the pelvis showed a destructiv…

[Successful therapy of sacroiliitis in SAPHO syndrome by etanercept].

Painful, aseptic osteitis remains the major problem in the treatment of patients with SAPHO syndrome. We present a child suffering of both sacroiliitis and acne conglobata in the context of SAPHO syndrome. While acne lesions responded well to systemic isotretinoin, sacroiliitis associated pain could be controlled neither by NSAR nor by intralesional or systemic steroid injection. Worse pain limited substantially patient's mobility. This changed immediately after starting etanercept. Within a few days, pain resolved and the patient regained his mobility. This favourable response lasted for 8 mo…

Sacroiliitis due to Salmonella Typhi: A case report.

CONTEXT: Pyogenic infections of the sacroiliac joint are observed quite rarely. The most frequent causative microorganisms are Staphylococcus aureus, Streptococcus species, and Pseudomonas aeruginosa that are commonly found in patients under intravenous medication. In this paper, a rare sacroiliitis case that developed due to Salmonella Typhi is discussed. CASE REPORT: A woman at the age of twenty applied to our clinic with complaints of fever, headache and diarrhea with which she had been suffering for five days. On physical examination, she had a slight fever, with a body temperature of 38.…

Spondyloarthritis in a pediatric population: risk factors for sacroiliitis.

OBJECTIVE: Pediatric rheumatologists may have an opportunity to diagnose sacroiliitis in its early stages, prior to the development of irreversible radiographic changes. Early diagnosis frequently requires magnetic resonance imaging (MRI), the use of which is limited by expense and requirement for sedation. We set out to identify features of juvenile spondyloarthritis (SpA) associated with the highest risk of sacroiliitis, to identify patients who may be candidates for routine MRI-based screening. METHODS: We reviewed the charts of 143 children seen at Texas Scottish Rite Hospital for Childre…
