Lumbar fusion involving the sacrum increases dislocation risk in primary total hip arthroplasty.

AIMS: Concurrent hip and spine pathologies can alter the biomechanics of spinopelvic mobility in primary total hip arthroplasty (THA). This study examines how differences in pelvic orientation of patients with spine fusions can increase the risk of dislocation risk after THA. PATIENTS AND METHODS: We identified 84 patients (97 THAs) between 1998 and 2015 who had undergone spinal fusion prior to primary THA. Patients were stratified into three groups depending on the length of lumbar fusion and whether or not the sacrum was involved. Mean age was 71 years (40 to 87) and 54 patients (56%) were

ACR Appropriateness Criteria® Stress (Fatigue/Insufficiency) Fracture, Including Sacrum, Excluding Other Vertebrae.

Stress fractures, including both fatigue and insufficiency types, are frequently encountered in clinical practice as a source of pain in both athletes and patients with predisposing conditions. Radiography is the imaging modality of choice for baseline diagnosis. MRI has greatly improved our ability to diagnose radiographically occult stress fractures. Tc-99m bone scan and CT may also be useful as diagnostic tools. Although fatigue and insufficiency fractures can be self-limited and go onto healing even without diagnosis, there is usually value in initiating prompt therapeutic measures as inco

Spontaneous osteoporotic fractures of the sacrum causing neurological damage. Report of three cases.

Although osteoporotic fractures of the sacrum seem to be a well-known entity, their associated rate of neurological complications has not been assessed in the literature. The authors report three such cases of nerve root compromise in elderly women and conduct a literature review. Based on their review, they estimate the incidence to be approximately 2%. The true incidence is probably higher because many case reports provide only scant information on symptoms; furthermore, sphincter dysfunction and lower-limb paresthesias are the most common symptoms and can readily be overlooked or misinterpr

Emphysematous Osteomyelitis of the Lumbar Spine and Sacrum After Liver Transplantation: A Case Report.

Emphysematous osteomyelitis (EO) is a rare and potentially fatal disease that often occurs in patients with underlying conditions, most commonly diabetes mellitus. Herein, we report a case of a 62-year-old man who presented with fever, tachycardia, and hypotension 112 days after liver transplantation. Blood tests revealed an increased inflammatory response. Computed tomography demonstrated clusters of small gas collections in the first and second lumbar vertebral bodies and the right sacral ala, a finding characteristic of the pumice stone sign of EO. Septic shock due to EO was diagnosed. The

Isolated cryptococcal osteomyelitis of the sacrum in an immunocompetent patient: a case report and literature review.

BACKGROUND: Cryptococcus neoformans, an opportunistic fungal pathogen, seldom causes infection in immunocompetent people. Cryptococcal osteomyelitis is an uncommon condition in which Cryptococcus invades the bone. It usually occurs as part of a disseminated infection and rarely in isolation. The spine has been reported as the most common site of cryptococcal osteomyelitis; however, isolated case of sacrum involvement in immunocompetent patients has never been reported. CASE PRESENTATION: We report the case of a 37-year-old man without underlying disease who presented with progressive low back

Can Partially Threaded Cannulated Screws Be Better Designed to Maximize Purchase in the Sacrum?

Sacroiliac screw fixation involves the use of cannulated, partially or fully threaded screws. Current partially threaded screws have standardized thread lengths involving a small portion of the screw regardless of its overall length. Forty uninjured computed tomography images were evaluated for distances from the lateral iliac cortex to the lateral sacral cortex at the first and second sacral segments. No difference in measurements were observed for gender, age, or body mass index. Using a smooth segment value of 32 mm, a significant increase in thread lengths is achievable allowing for a nove

Sacropelvic fixation versus fusion to the sacrum for spondylodesis in multilevel degenerative spine disease.

STUDY DESIGN: Retrospective study. OBJECTIVE: For successful multilevel correction and stabilization of degenerative spinal deformities, a rigid basal construct to the sacrum is indispensable. The primary objective of this study was to compare the results of two different sacropelvic fixation techniques to conventional stabilization to the sacrum in patients with multilevel degenerative spine disease. METHODS: A total of 69 patients with multisegmental fusion surgery (mean number of stabilized functional spinal units: 7.0 ± 3.3) with a minimum of 1-year follow-up were included. 32 patients r

Complications and results of long adult deformity fusions down to l4, l5, and the sacrum.

STUDY DESIGN: This is a consecutive study of patients having undergone surgical treatment of adult lumbar scoliosis. Follow-up ranged from 2 to 13 years (average 5 years). OBJECTIVES: To assess the complications and outcomes of patients with long fusions to L4 (n=23), L5 (n=21), or the sacrum (n=15) and determine if a "deeply seated" L5 segment is protective. SUMMARY OF BACKGROUND DATA: Few studies assess outcomes and complications in adults fused from the thoracic spine to L4, L5, or the sacrum with minimum 2-year follow-up. METHODS: Fifty-eight patients (59 cases; average age 43 years; ra

Stress fractures of the sacrum. An atypical cause of low back pain in the female athlete.

Low back pain is a common finding in an athletically active premenopausal female population. We describe an unusual cause of persistent low back/sacroiliac pain: a fatigue-type sacral stress fracture. Plain radiographs, bone scans, computed tomography, and magnetic resonance imaging studies were obtained in the female athletes to determine the nature of the pathologic abnormality. The most significant risk factor for fatigue-type sacral stress fractures was an increase in impact activity due to a more vigorous exercise program. Potential risk factors such as abnormal menstrual history, dietary

Insufficiency fractures of the sacrum: a cause of low back pain after lung transplantation.

Insufficiency fractures of the sacrum were diagnosed during the first year after successful transplantation in four (5.6%) of 71 lung and heart-lung transplant recipients. Each patient had development of low back pain after minor or no trauma; all had osteoporosis. In each instance, plain radiographs failed to demonstrate the fracture, and the diagnosis was established by radionuclide bone scanning that demonstrated the characteristic "butterfly" (bilateral sacral fracture) or "half-butterfly" appearance (unilateral sacral fracture). Sacral insufficiency fractures, a significant cause of low b

Transcutaneous gas tensions in the sacrum during the acute phase of spinal cord injury.

The first few months following injury to the spinal cord requires constant care of the subject if tissue breakdown is to be avoided. The management of acute traumatic cases involves complete bedrest in a supine position with appropriately positioned pillows to minimize trauma to the bone prominences. This study assesses the effectiveness of the management procedure in terms of the tissue response at the sacrum of 15 acute spinal cord injured subjects. The measurement of mean interface pressures during a representative period of recumbency was performed and these were related to changes in tran

Treatment of painful long-standing displaced fracture-dislocations of the sacrum. A case report.

A 19-year-old white man sustained multiple injuries, including a severely displaced Malgaigne fracture. Secondary uncontrollable hemorrhage was treated successfully by arterial embolization. The patient retained a painfully displaced right sacroiliac joint. His multiple injuries resolved symptom free with the exception of the right sacroiliac region. An attempt to reduce the dislocation by an extensive soft tissue release and skeletal traction was unsuccessful. Solid fusion of the L4 and L5 vertebrae to the ilium significantly reduced the patient's pain. Painful, long-standing severely displac

Sacrum Metastasis in Stage IA Endometrial Carcinoma 13 Years After Surgery: A Case Report.

Recurrence of early-stage endometrial cancer (EC) most commonly occurs within 5 years after initial surgery, and ultra-late recurrence more than 10 years after treatment is extremely rare. Bone metastasis from EC is also relatively uncommon. We report a case of Stage IA EC with sacral metastasis occurring 13 years after curative surgery. The patient is a 48-year-old woman who underwent radical hysterectomy for EC 13 years ago. She completed 5 years of follow-up with no recurrence. She was referred to our hospital because of a sacral tumor accompanied by pain and bladder and bowel dysfunction.

Blood flow responses over sacrum in nursing home residents during one hour bed rest.

OBJECTIVES: To describe individual BF responses in a nursing home resident population for one-hour periods of bed rest. METHODS: BF was measured for one hour over the sacrum in 0° supine position and 30° supine tilt position in 25 individuals aged 65 y or older while lying on a pressure-redistributing mattress. Measurements were made at three tissue depths (1, 2, and 10 mm) using the noninvasive optical techniques, LDF and PPG. RESULTS: Eleven participants had a PIV response at 1 mm depth in both positions and seven participants had a lack of this response at this depth and positions. The BF

Investigating sacroplasty: technical considerations and finite element analysis of polymethylmethacrylate infusion into cadaveric sacrum.

BACKGROUND AND PURPOSE: Sacroplasty is not as routinely performed as vertebroplasty, possibly due to technical challenges and the paucity of data regarding subsequent outcomes. The first goal of the present investigation was to describe a technique for sacroplasty that facilitates safe needle placement and polymethylmethacrylate (PMMA) extrusion. The second goal was to perform finite element analysis (FEA) by using a geometric model of sacral fracture to identify mechanical outcomes of sacroplasty. MATERIALS AND METHODS: Sacroplasty was performed on fresh pelvis specimens (n=4) under biplane
