The allocation of patients with personality disorders to a suitable treatment approach: The development of a checklist based on patients' life stories.

Treatment assignment for patients with personality disorders (PDs) involves a complex process consisting of diagnostic assessment and deciding on the most appropriate psychotherapeutic treatment. This article describes the development of a checklist for systematic analysis of life stories to support reflective and transparent assignment of patients to either dialectical behavioral therapy (DBT) or schema-focused therapy (SFT). In a first study, an email survey, focus group, and member check were conducted among eight clinical experts to identify relevant dimensions in life stories in patients

The intertwined path of perfectionism and self-criticism in a client with obsessive-compulsive personality disorder.

Psychotherapy for obsessive-compulsive personality disorder (OCPD), where perfectionism is a defining criterion, is understudied. Despite a high prevalence few evidence-based treatments are available for the presentation. Here we describe the course of a 6-month program of metacognitive interpersonal therapy with an OCPD patient with prominent perfectionism and self-criticism, which were considered primary outcomes of the case study. Therapy aimed initially at increasing awareness of maladaptive interpersonal schemas and promoting a healthy self. First, behavioral experiments were used to try

Change process in psychotherapy for criminal offenders: a comprehensive review with content analysis.

The aim of this study is to systematically identify and synthesise the therapeutic components that facilitate recovery in criminal offenders undergoing psychotherapy or psychosocial interventions. A systematic review was conducted covering 163 studies published between 2000 and 2024, focusing on interventions such as cognitive-behavioral therapy, schema therapy, mentalisation-based treatment, and attachment-based approaches. A content analysis of these studies resulted in 7 core themes and 32 sub-components representing therapeutic components frequently emphasised in the literature. Structured

The relationship between therapist-client interpersonal compatibility, sex of therapist, and therapeutic outcome.

Explored the relationship between therapist-client interpersonal compatibility, sex of therapist, and psychotherapeutic outcome. The research sample consisted of 48 therapist-client dyads. The therapists were all graduate students enrolled in an individual therapy practicum, while the client sample was composed of 48 undergraduate female (mean age 19.3 years). The interpersonal compatibility of the dyads was determined from the FIRO-B results for each of the members' using a modification of Schutz's (1966) original mathematical schema. Therapy outcome was determined from pretherapy and postthe

Neonatal transient pseudohypoparathyroidism: could it be included among inactivating parathyroid hormone (PTH)/PTH-related protein signalling disorders?

We report a case of transient pseudohypoparathyroidism in a full-term newborn that presented at 20 hours of life with hypocalcemic seizures, hyperphosphatemia and raised parathormone levels. The diagnosis of pseudohypoparathyroidism was made according to biochemical investigations. The infant was treated with calcium supplementation and vitamin D analog therapy, and he remained stable and symptom-free with normal serum biochemistries during follow-up. We suggest that transient pseudohypoparathyroidism of the newborn (ntPHP) might be included among inactivating parathyroid hormone (PTH)/PTH-rel

Adrenal masses in the newborn.

The incidence of neonatal adrenal tumors is increasing due to the expanded use and accuracy of prenatal ultrasonography in routine obstetric care. Although adrenal and juxtarenal masses may represent benign lesions (adrenal hemorrhage, subdiapragmatic extralobar pulmonary sequestration), the majority of masses either are premalignant or malignant. Previous algorithms for the diagnosis and management of these lesions have been guided primarily by the high incidence of neuroblastomas within this group. Improved insight into the relatively benign behavior of many neonatal neuroblastomas has stimu

Postoperative metabolic alkalosis and acute renal failure: rationale for the use of hydrochloric acid.

Metabolic alkalosis secondary to chloride depletion, especially following gastrointestinal surgery and associated with acute renal failure, is a frequent clinical occurrence. Management of the resultant acid-base disturbance mandates chloride replacement. The presence of oliguria limits the choice of accompanying cation. The use of intravenous hydrochloric acid to correct and maintain proper chloride balance, secondary to external gastric fluid losses, is recommended as a straightforward approach. Two brief case synopses are presented. Both patients, florid examples of profound chloride deplet

The alliance-outcome association in borderline and obsessive-compulsive personality disorder.

Personality disorders are considered a possible factor affecting the relationship between therapeutic alliance and therapy outcome. The present study investigated the alliance-outcome effect in patient groups with borderline personality disorder (BPD) and obsessive-compulsive personality disorder (OCPD). Data derived from a sample of n = 66 patients, treated in a day care hospital setting with a dialectical-behavioral and schema therapeutic treatment concept. Patients rated their symptom severity at admission, early alliance after 4-6 therapy sessions and symptom severity as well as alliance a

Randomized trials of radiation therapy in adult low-grade gliomas.

There are two central questions in the radiotherapeutic management of the adult patient with a supratentorial low-grade (WHO grade II) astrocytoma, oligo-astrocytoma, or oligodendroglioma. The first question is one of timing. Following tissue diagnosis with or without maximum surgical resection, should immediate postoperative radiation therapy (RT) be given, or should RT be deferred to the time of local recurrence? The second question is one of dose. Assuming RT is given, should lower doses (ie, approximately 45-50 Gray (Gy)) or higher doses (ie, approximately 60-65 Gy) be administered? One Ph

Therapeutic strategies to manage climate distress among young people.

The impact of climate change on mental health is increasingly evident, particularly among young people (16-25 years old), ranging from acute distress after extreme weather events to more persistent feelings of anxiety, grief, and uncertainty regarding the future. This Viewpoint explores how mental health care for young people can be improved to more effectively respond to the psychological impacts of climate change. Rather than proposing a new treatment model, this Viewpoint provides a starting point for clinicians by examining how existing therapeutic approaches could be applied, adapted, or

Psychotherapies for the treatment of personality disorders: the state of the art.

PURPOSE OF REVIEW: To provide an update of systematic reviews, meta-analyses and recent clinical outcome studies for personality disorder (PD) in order to investigate the state of the art of the evidence of psychotherapy for personality disorders. RECENT FINDINGS: Few outcome studies in patients with Cluster A and Cluster C PD have been conducted, which limits the conclusions which can be drawn. Most recently published research has been conducted with borderline PD. There is limited evidence that dialectical behavior therapy (DBT), mentalization based therapy and schema therapy are more effec

New approaches to the management of Philadelphia-chromosome-positive acute lymphocytic leukemia.

The discovery of targeted ABL tyrosine kinase inhibitors has allowed significant advances in the treatment of de novo Philadelphia chromosome (Ph)-positive ALL. Whereas the outcome with standard chemotherapy was previously dismal, the use of imatinib in front-line therapy has improved relapse-free survival and overall survival, even in the absence of allogeneic stem cell transplantation in first complete remission (particularly for those with comorbidities or lack of a suitable donor). However, the emergence of resistance to imatinib presents a new therapeutic challenge. Novel tyrosine kinase

A case study on a severe paranoid personality disorder client treated with metacognitive interpersonal therapy.

Paranoid personality disorder (PPD) is a severe condition, lacking specialized and empirically supported treatment. To provide the clinician with insights into how to treat this condition, we present a case study of a 61-year-old man with severe PPD who presented with ideas of persecution, emotionally charged hostility, and comorbid antisocial personality disorder. The client was treated with 6 months of Metacognitive Interpersonal Therapy, which included: creating a shared formulation of his paranoid attitudes; trying to change his inner self-image of self-as-inadequate and his interpersonal

Therapists' problematic experiences when working with obsessive-compulsive disorder: a qualitative investigation of schema modes, mode cycles, and strategies to return to healthy adult mode.

INTRODUCTION: Obsessive-compulsive disorder (OCD) is one of the most challenging pathologies for therapists, being chronic and often characterized by frequent relapses. The therapeutic relationship plays a critical role in the outcome of therapy. The dynamic interaction between therapist and client schema modes determines the quality of the relationship. THE OBJECTIVE: of the present qualitative research is to investigate and conceptualize the triggers for therapists when working with OCD clients, the therapists' schema modes that are activated, and the strategies they use to get back into th

Failures in cognitive behavior therapy: The state of the art.

The aim of this review is to evaluate the state of the art of studies investigating failures in cognitive behavior therapy (CBT). The last couple of years relatively few studies have specifically focused on failures in CBT. Generally, clinicians underestimate the number of therapies that fail and therapists are no better than chance at predicting effectiveness classification. In patients treated in outpatient clinics, effects of therapy were lower than effects of therapy in disorder-specific CBT efficacy randomized controlled trials. In eating disorders, results of CBT are moderate with high r

The empirical status of the third-wave behaviour therapies for the treatment of eating disorders: A systematic review.

Although third-wave behaviour therapies are being increasingly used for the treatment of eating disorders, their efficacy is largely unknown. This systematic review and meta-analysis aimed to examine the empirical status of these therapies. Twenty-seven studies met full inclusion criteria. Only 13 randomized controlled trials (RCT) were identified, most on binge eating disorder (BED). Pooled within- (pre-post change) and between-groups effect sizes were calculated for the meta-analysis. Large pre-post symptom improvements were observed for all third-wave treatments, including dialectical behav
