Gamma knife radiosurgery of skull base meningiomas.

Meningiomas are the most frequent benign tumors treated by gamma knife radiosurgery and the majority of them are located on the skull base. Between 1992 and 1999, 197 skull base-located meningiomas in 192 patients were treated by gamma knife in Prague. Contact with the chiasma or optic tract was not regarded as a contraindication for gamma knife radiosurgery and such contact was observed in 32% of the skull base meningiomas treated. 176 patients were monitored during a median of 36 months, of whom 73% showed a decrease in tumor volume; no change was observed in 25% and continued growth was obs

Endoscopic management of anterior skull base encephaloceles.

Encephaloceles are relatively rare phenomena produced by the protrusion of brain and dura through an anterior skull base defect. Although they can occur as congenital defects, encephaloceles can also present after trauma. The diagnosis is usually made with nasal endoscopy and imaging studies. This report reviews our recent experience repairing 5 encephaloceles in 4 patients. The diagnostic approach and the technical aspects of surgical management are discussed. Although encephaloceles are a rarity, this diagnosis should be considered as part of the differential diagnosis in evaluating a patien

Dural arteriovenous malformations of the skull base.

Dural arteriovenous malformations (AVMs) are interesting lesions, which can present as diagnostic and therapeutic challenges. Such AVMs were initially described as located solely in the dural venous sinuses, but during the last decade researchers have discovered them in other locations such as the tentorium and foramen magnum. It is understandable that they can develop in other locations because numerous arteriovenous shunts normally occur all over the dura. Early reports emphasize that they are congenital; however, some investigators have proposed that they are acquired lesions arising after

Giant prolactinomas presenting as skull base tumors.

BACKGROUND: Prolactinomas invading the skull base are rare, and could easily be confused with skull base tumors of nonpituitary origin. CASE DESCRIPTION: We report a series of 4 cases of giant prolactinomas invading the skull base and presenting with atypical symptoms. Case 1 presented with a short history of headache and nasal obstruction. Case 2 presented with progressive hypoacusia, dizziness, and ophthalmoplegia. In Case 3, the patient developed rapid progressive visual failure and psychiatric symptoms. Case 4 presented with a 1-year history of headache and retrorbital pain. The diagnosis

Gamma knife radiosurgery for skull base meningiomas.

Radiosurgery has been proven to be a safe and effective management strategy for skull base meningiomas either primarily or for tumor recurrence or progression after prior microsurgical resection. With its steep radiation falloff, radiosurgery provides long-term tumor growth control without the complications associated with conventional fractionated radiation therapy. Stereotactic MR imaging has allowed better definition of the tumor margin for precise multiisocenter conformal dose planning, and our current radiation dose prescription has decreased the incidence of new cranial nerve deficits af

Neurofibromatosis of the orbit and skull base.

Von Recklinghausen's disease is characterized by multiple neurofibromas, pigmentations, and pachydermatoceles of the skin depending on the disorder of the neural crest derivative. We report on a 25-year-old patient suffering from neurofibromatosis localized in the cranio-orbital region, whose main problem was a pulsating right exophthalmos caused by herniation of the frontotemporal lobe through a defect of the greater wing of the sphenoid. The lesion was approached through a combined route (i.e., transfrontally and transfacially [upper eyelid incision]). Surgical strategy and associated proble

Congenital encephalocele of the medial skull base.

Meningoencephaloceles of the temporal bone are rare. Although most often seen following otologic surgery or trauma, congenital meningoencephaloceles can exist. The clinical presentation, diagnostic evaluation, and surgical management of three patients with congenital meningoencephalocele are presented. Two of the three patients presented to our institution with recurrent episodes of meningitis; one presented with partial complex seizures. Diagnostic evaluation included temporal bone computed tomography with magnetic resonance imaging. In two patients, defects were imaged following high-pressur

Alteration of rCBF in skull base lesions.

Cerebral functional imaging methods provide complementary information on brain function and large vessels regulatory controls which are compromised in cranial base lesions. The presence of a skull base tumor can alter regional cerebral blood flow (rCBF) in adjacent and remote brain tissue. This report presents the results of 104 CBF studies in 66 patients with skull base lesions (aged 17-75 years). The lesions included 36 meningiomas, 21 neurinomas and 9 other tumors. Regional CBF in tumor and brain tissue was measured prior to treatment using stable xenon enhanced computed tomography (Xenon(/

Stereotactic radiosurgery of anterior skull base tumors.

Stereotactic radiosurgery is an increasingly safe and usually effective method of preventing growth of small to moderate-sized primary tumors of the anterior skull base. Tumor growth control is obtained in more than 90% of patients with skull base tumors having benign histology. Neurologic function is maintained in most patients. The risk of temporary or permanent injury to critical neural and vascular structures is significantly lower than the risk associated with microsurgery. The optic nerves, chiasm, and tracts are structures that appear most sensitive to the radiation doses used during ra

Neuromonitoring in operations in the skull base.

Methods to monitor the integrity of cranial motor nerves during operations on skull base tumors have been developed over the past decade. These methods can help the surgeon to identify cranial motor nerves that are located in the surgical field but which may not be visible directly. Methods have also been developed that allow monitoring of the function of sensory systems such as the auditory system and it has been shown that brainstem auditory evoked potentials can provide important information about the integrity of the auditory nervous system. It has been shown in several studies that such n

Approach to diagnosis of skull base lesions.

The ability to operate safely on the skull base and yet preserve cranial nerves and leave the patient with minimum neurologic deficit is a fairly new development. The field is expanding rapidly, and lesions that were once deemed unresectable can now be resected. With this in mind, one must maintain an extremely high index of suspicion for lesions in this area. Early diagnosis of skull base lesions is enhanced by a careful neuro-otologic history and physical examination on all patients with complaints of pain, headache, or cranial nerve dysfunction, either motor or sensory. Based on a thorough

The transcochlear approach to the skull base.

A new approach to tumors of the skull base is described. This approach is accomplished by forward extension of the translabyrinthine opening into the cerebellopontine angle. The facial nerve is mobilized in the temporal bone from the stylomastoid foramen to its entrance into the internal auditory canal. Having removed the barrier of the facial nerve, additional bone removal can be carried forward to the internal carotid artery, which now becomes the forward limit for temporal bone resection. The access attained through this exposure allows removal of tumors arising from the petrous tip, as wel

Impact of skull base cisterns invasion and extent of resection on the management of skull base intracranial epidermoid tumors.

BACKGROUND: This study reports the authors' experience with microsurgical resection of skull base epidermoid tumors, examining the relationship between the number of invaded intracranial subarachnoid cisterns (SC) and the extent of resection, recurrence and outcomes. METHODS: Data from 84 patients who underwent skull base epidermoid tumor resection between 2000 and 2022 were prospectively collected and retrospectively reviewed. Clinical presentations, imaging findings (including the number of invaded cisterns), and extent of resection were analyzed for their influence on postoperative complic

Neck swelling due to skull base (pseudo)meningocele protruding through a congenital skull base bone defect: a case report.

BACKGROUND: Meningocele is defined as a protrusion of the meninges through an opening in the skull or spinal column, forming a bulge or sac filled with cerebrospinal fluid. A pseudomeningocele is defined as a cerebrospinal fluid (CSF) collection formed due to escape of CSF through a dural defect with trapping of CSF into the surrounding soft tissues. We herby report rare occurrence of a large (pseudo)meningocele in a young patient with congenital skull base defect presenting as upper lateral neck swelling. CASE PRESENTATION: We present the case of a 17-year-old boy who had painless progressiv

Skull Base Erosion Resulting From Primary Tumors of the Temporomandibular Joint and Skull Base Region: Our Classification and Reconstruction Experience.

PURPOSE: We sought to introduce our classification and reconstruction protocol for skull base erosions in the temporomandibular joint and skull base region. PATIENTS AND METHODS: Patients with neoplasms in the temporomandibular joint and skull base region treated from January 2006 to March 2017 were reviewed. Skull base erosion was classified into 3 types according to the size of the defect. RESULTS: We included 33 patients, of whom 5 (15.2%) had type I defects (including 3 in whom free fat grafts were placed and 2 in whom deep temporal fascial fat flaps were placed). There were 8 patients (

[Deep Dural Suturing and Multilayer Skull Base Reconstruction].

This review described the principles and techniques of dural suturing and multilayer skull base reconstruction in extended endoscopic endonasal surgery, with an emphasis on the Japanese tradition of deep suturing. Secure watertight reconstruction is a fundamental requirement in extended transsphenoidal and endonasal skull base surgery because persistent cerebrospinal fluid (CSF) leakage can result in life-threatening meningitis. Since its introduction in 2007, multilayer reconstruction using various materials combined with a pedicled nasoseptal flap has become the standard approach. In contras
