Ineraid cochlear implant in the ossified cochlea: surgical techniques and results.

Extensive ossification of the cochlea is a common finding in patients with total deafness caused by meningitis, labyrinthitis, or otosclerosis. When the cochlea is totally ossified, the prognosis for achieving free-running speech without lip reading is poor. However, subtotal ossification with residual cochlear patency in the upper basal turn, the middle turn, or the apical turn can be maximally exploited by selective insertion of electrodes. Results can be surprisingly adequate in spite of the poor anatomic conditions. Computed tomography and exploratory cochleotomy, in which electrically evo

Emotional prosody perception and production in dementia of the Alzheimer's type.

PURPOSE: In this study, the authors investigated emotional prosody in patients with moderate Dementia of the Alzheimer's type (DAT) With Late Onset. It was expected that both expression and reception of prosody would be impaired relative to age-matched controls. METHOD: Twenty DAT and 20 control participants engaged in 2 expressive and 2 receptive tasks with randomly presented exemplars of sentences targeting the emotions of happiness, anger, sadness, and surprise. RESULTS: In the expressive tasks, objective acoustic measurements revealed significantly less pitch modulation by the patient gr

Subclinical Auditory Neural Deficits in Patients With Type 1 Diabetes Mellitus.

OBJECTIVES: Diabetes mellitus (DM) is associated with a variety of sensory complications. Very little attention has been given to auditory neuropathic complications in DM. The aim of this study was to determine whether type 1 DM (T1DM) affects neural coding of the rapid temporal fluctuations of sounds, and how any deficits may impact on behavioral performance. DESIGN: Participants were 30 young normal-hearing T1DM patients, and 30 age-, sex-, and audiogram-matched healthy controls. Measurements included electrophysiological measures of auditory nerve and brainstem function using the click-evo

Significance of hearing preservation in acoustic neuroma surgery.

In the past 10 years, 43 patients with acoustic neuroma have been operated on by the middle cranial fossa approach. In all cases, meticulous care was taken to preserve the cochlear function regardless of the degree of preoperative hearing disorder. Thirty-nine of 43 patients had various degrees of residual hearing, hearing was preserved in 16 of these (41%). The best result was obtained in patients with a tumor located less than 1 cm from the porus. The hearing preservation rate was 64% (11/17). The patients recovered hearing acuity quite well, especially at low frequencies in 2 cases who had

[Hearing impairment in the so-called normal ear during Menière's disease].

A detailed audiological examination of 84 specially selected patients was performed. They had Meniere's disease without any audiometric changes in the so-called normal ear. The hearing function was characterized on the basis of threshold tonic audiometry, ultrasound sensitivity, lower limit of perceived sound frequencies, hearing discomfort level, dynamic range of the hearing field, and intelligibility of low voice speech masked by noise. It was demonstrated that all patients with Meniere's disease on the side of the so-called normal ear, where no symptoms of hearing insufficiency can be detec

Small fenestra stapedectomy. A preliminary report.

The small fenestra stapedectomy is based on a rationale of creating a more effective acoustical mechanical transmission system, while reducing potential labyrinthine disturbance. Surgical technique for the small fenestra stapedectomy is described, including the creation of the fenestra and the use of McGee stainless steel piston prosthesis with loose areolar tissue around the piston. Postoperative results are compared in a series of 100 cases, 50 having the small fenestra technique, SFT, and 50 having partial or total footplate removal procedure. Vestibular results demonstrate a noticeable red

Anatomical and behavioral correlates of auditory perception in developmental dyslexia.

Developmental dyslexia is typically associated with difficulties in basic auditory processing and in manipulating speech sounds. However, the neuroanatomical correlates of auditory difficulties in developmental dyslexia (DD) and their contribution to individual clinical phenotypes are still unknown. Recent intracranial electrocorticography findings associated processing of sound amplitude rises and speech sounds with posterior and middle superior temporal gyrus (STG), respectively. We hypothesize that regional STG anatomy will relate to specific auditory abilities in DD, and that auditory proc

The fragile nature of the speech-perception deficit in dyslexia: natural vs synthetic speech.

A number of studies reported that developmental dyslexics are impaired in speech perception, especially for speech signals consisting of rapid auditory transitions. These studies mostly made use of a categorical-perception task with synthetic-speech samples. In this study, we show that deficits in the perception of synthetic speech do not generalise to the perception of more naturally sounding speech, even if the same experimental paradigm is used. This contrasts with the assumption that dyslexics are impaired in the perception of rapid auditory transitions.

The Finnish speech-in-noise test in MELAS mutation and other sensorineural hearing impairments.

A computerized adaptive Finnish speech-in-noise test has recently been developed. The purpose of the present study was to measure speech recognition thresholds in noise (SRTN) in patients with sensorineural hearing impairment (SNHI) caused by mitochondrial mutation [mitochondrial myopathy, encephalopathy, lactic acidosis and stroke-like episodes (MELAS)] and in patients with SNHI from other causes and to compare them with those in normal hearing controls. SRTN differed significantly between controls and the patients with SNHI from other causes and between controls and the MELAS group, but not

Factors predicting postoperative sentence scores in postlinguistically deaf adult cochlear implant patients.

A sample of 64 postlinguistically profoundly to totally deaf adult cochlear implant patients were tested without lipreading by means of the Central Institute for the Deaf (CID) sentence test 3 months postoperatively. Preoperative promontory stimulation results (thresholds, gap detection, and frequency discrimination), age, duration of profound deafness, cause of deafness, lipreading ability, postoperative intracochlear thresholds and dynamic ranges for electrical stimulation, depth of insertion of the electrode array into the scala tympani, and number of electrodes in use were considered as po

Clinical results with a percutaneous bone-anchored hearing aid.

During a 2-year period ending in July 1987, nine patients were implanted with a percutaneous bone-anchored hearing aid developed at the University of Gothenburg and Chalmers University of Technology, Sweden. Patients selection was based on the presence of conductive or mixed hearing loss in patients who are unable to wear a conventional hearing aid because of infection or ear canal problems. Patients had speech discrimination scores of at least 60% for phonetically balanced monosyllables (CID lists) at 40 dB above threshold, and a pure tone bone conduction average of 45 dB hearing loss or bett

Knockdown of Dyslexia-Gene Dcdc2 Interferes with Speech Sound Discrimination in Continuous Streams.

UNLABELLED: Dyslexia is the most common developmental language disorder and is marked by deficits in reading and phonological awareness. One theory of dyslexia suggests that the phonological awareness deficit is due to abnormal auditory processing of speech sounds. Variants in DCDC2 and several other neural migration genes are associated with dyslexia and may contribute to auditory processing deficits. In the current study, we tested the hypothesis that RNAi suppression of Dcdc2 in rats causes abnormal cortical responses to sound and impaired speech sound discrimination. In the current study,

Analysis of word comprehension in a case of pure word deafness.

A case of pure word deafness due to a left temporal infarct is reported. The results of dichotic tests suggest that auditory verbal material may be processed in the right hemisphere. The inability to repeat nonsense words, the frequent semantic paraphasias in real-word repetition tasks, and the capacity to give a partial account of the meaning of a word that the patient cannot repeat show that despite the impairment of the phonological analysis, lexical semantic processing is possible. An attempt is made to demonstrate that the patient resorts to this semantic processing and that this reflects

Consequences of conductive auditory impairment for binaural hearing.

Studies of persons with conductive hearing loss have revealed substantial degradations in binaural hearing abilities. Described here is a model based on the hypothesis that this degradation is due to high levels of bone-conducted signals relative to air-conducted signals. The model makes quantitative predictions for the effects of conductive impairment on measurements of interaural discrimination. Qualitative predictions for binaural advantages in detection and speech intelligibility are also made by employing auxiliary models. Generally, available data are consistent with the models, although

Hearing preservation in acoustic neurinoma surgery.

In a series of 120 patients with acoustic neurinoma, hearing preservation at removal of the tumour via the suboccipital approach was attempted in 30 ears. Hearing was preserved in 13 ears (43%), but in two, hearing was lost entirely in 3 to 4 years; thus the success rate was 36%. In 9 of the remaining 11 ears useful speech discrimination was present. Audiological tests showed increased retrocochlear loss after surgery. A surgical team should master the methods of both translabyrinthine and suboccipital surgery in order to choose the best approach for each patient.
