[Report of two rare cases of fungal sinusitis].

Two cases of aspergillosis of the paranasal sinuses are reported. The first case was a 30-year-old man who had a 5-month history of bilateral proptosis. Physical examination revealed nasal polyps in both middle meatus. A skin test for Aspergillus was positive. Laboratory study showed levels of serum IgE and IgE specific for Aspergillus level to be elevated significantly. Computed tomography (CT) and magnetic resonance imaging (MRI) showed pansinusitis with some bone erosion. The patient underwent bilateral Caldwell-Luc procedures and external sinus surgery (frontal, ethmoid and sphenoid sinuse

Rhinocerebral Mucormycosis with Top of Basilar Artery Syndrome.

AIM: Our objective is to highlight the varied forms of vascular dissemination of rhinocerebral mucormycosis resulting in ischemic arterial syndromes, especially rare posterior ischemic circulatory strokes, through dissemination of mucormycosis from sphenoid bone to clivus by bony erosion and to the basilar artery, thus causing the top of basilar artery syndrome. MATERIALS AND METHODS: We have reported a case of rhinocerebral mucormycosis in an uncontrolled diabetic patient presented to the neurointensive care unit, department of neurology. FINDINGS: A 56-year-old male diabetic patient with p

An Unusual Case of Non-traumatic Tension Pneumocephalus.

Tension pneumocephalus is a life-threatening situation in neurosurgery. It can occur due to various causes, such as trauma, complications of neurosurgery or sinus surgery, and also in conditions where there is a CSF leak. We present an unusual case of a 53-year-old woman who came with complaints of severe headache and CSF rhinorrhea. On radiologic evaluation, tension pneumocephalus was noted as a result of sphenoid encephalocele. Sphenoidal encephaloceles are herniations of brain parenchyma, meninges, and CSF through a defect in the sphenoid bone. It typically presents as CSF rhinorrhea. Rarel

Craniofacial bone alterations in patients with neurofibromatosis type 1.

Osseous manifestations of neurofibromatosis 1 (NF-1) occur in a minority of the affected subjects but may be because of significant clinical impairment. Typically, they involve the long bones, commonly the tibia and the fibula, the vertebrae, and the sphenoid wing. The pathogenesis of NF-1 focal osseous lesions and its possible relationships with other osseous NF-1 anomalies leading to short stature are still unknown, though it is likely that they depend on a common mechanism acting in a specific subgroup of NF-1 patients. Indeed, NF-1 gene product, neurofibromin, is expressed in all the cells

Primary sphenoid and petrous apex esthesioneuroblastoma: case report.

A 62-year-old woman presented with raised intracranial pressure and features of a right cerebellopontine angle tumour with extension into the right middle cranial fossa. The patient died before a surgical excision could be performed. The autopsy revealed a primary esthesioneuroblastoma of the sphenoid sinus eroding the petrous bone and extending into the middle cranial fossa with metastatic tumour in the liver, and paratracheal and hilar lymph nodes. Although rare, esthesioneuroblastoma must be considered in the differential diagnosis of petrous-sphenoid lesions.

Cystic Degeneration of Craniofacial Fibrous Dysplasia.

BACKGROUND: Fibrous dysplasia (FD) is most often a slowly progressive benign disease in which the normal bone structure is replaced by fibrous and osteoid tissue. CASE DESCRIPTION: A 16-year-old adolescent, known with FD in the sphenoid bone, suffered an acute decreased visual acuity with papilledema on the left eye. The radiologic images were best compatible with cystic degeneration of the known FD with optic nerve compression in the optic canal. Decompression of the optic nerve was performed through an endoscopic exploration of the left sphenoid sinus. The visual acuity recovered completely

Giant epidermoid involving 3 cranial bones.

We describe a patient with an incidental finding of an expansile osteolytic lesion centered in the sphenoid involving 3 cranial bones-frontal, temporal, and sphenoid-and invading the brain parenchyma and orbit. Biopsy demonstrated an epidermoid. The lesion was excised through frontotemporal craniotomy. Despite the benign nature of the tumor, the epidermoid had a destructive pattern of growth. Complete resection was needed to adequately address the inflammatory mass effect of the large lesion.

Modified orbitozygomatic craniotomy with a single burr hole in the alternative sphenoid ridge keyhole.

BACKGROUND: One-piece modified orbitozygomatic approach (OZA) is an extended version of the pterional approach that also includes orbital walls and frontal process of the zygomatic bone. For this craniotomy one burr hole must be placed in MacCarty keyhole and another - in the temporal region. OBJECTIVE: To develop a technique of the one-piece modified OZA with single a burr hole in the alternative sphenoid ridge keyhole that allows access to orbit, anterior cranial fossa and middle cranial fossa and apply it intraoperatively. METHODS: A single human head specimen was used. The dissection was

An anatomical study of the pterygospinous bar and foramen of Civinini.

PURPOSE: The pterygospinous ligament extends from the posterior free margin of the lateral pterygoid plate till the spine of the sphenoid. The ligament may ossify partly or completely leading to the formation of the pterygospinous bar. A complete ossification of the ligament results in the formation of the foramen of Civinini. Presence of the complete or incomplete pterygospinous bar may lead to a difficulty in passing the needle during anaesthesia for the trigeminal neuralgia or the bar may also compress the mandibular nerve and its branches to cause lingual numbness, pain and speech impairme

Associations of osseous abnormalities in Neurofibromatosis 1.

The characteristic sites of Neurofibromatosis 1-associated osseous manifestations are the long bones (usually the tibia and fibula), vertebrae and sphenoid wing. Although these focal bony lesions may cause profound clinical consequences, a minority of people with NF1 are affected. However, most people with NF1 are shorter than expected for their age, gender and family. The pathogenesis of NF1 focal osteopathy and its relationship, if any, to short stature are unknown. We examined associations between the occurrence of various osseous lesions in 3377 NF1 probands from the Children's Tumor Found

Spontaneous obliteration of a greater sphenoid wing dural arteriovenous fistula involving the diploic venous system.

BACKGROUND: Sphenoid wing dural arteriovenous fistulas (DAVFs) are rare vascular anomalies caused by abnormal arteriovenous shunting near the sphenoid bone. They are associated with significant risks, including cortical venous reflux and intracranial hemorrhage, especially when involving high-risk drainage pathways. Spontaneous regression of high-flow sphenoid wing DAVFs is exceedingly rare and has not been previously documented. CASE DESCRIPTION: We report a 59-year-old woman with a history of head trauma who presented with progressive right eye proptosis, redness, blurred vision, and tinnit

Transcranial extradural subtemporal repair for sphenoid sinus lateral recess meningoencephalocele: technical note.

Cranial meningoencephalocele is a rare condition consisting of the herniation of meninges, CSF, and brain tissue through a cranial or skull base defect. Sphenoid sinus lateral recess meningoencephalocele is of particular interest due to the complex anatomy surrounding the bone defect and their demanding surgical management. In this technical note, we reported a step-by-step description of a rare case of sphenoid sinus lateral recess meningoencephalocele causing headache due to recurrent cerebrospinal fluid leak treated with a subtemporal craniotomy with extradural middle cranial fossa drilling

Intrasellar Cephalocele.

Intrasellar cephalocele is very rare and is often overlooked because of its atypical clinical demonstration. A 2-year, 10-month-old girl was referred to our hospital with short stature. Magnetic resonance imaging revealed an extension of the anterior third ventricle, which protruded into the pituitary fossa, but not into the craniopharyngeal canal, sphenoid bone, or sphenoid sinus. In addition, there was no bony defect of the floor of the sellar turcica or the sphenoid sinus, and the mass was not protruding into the nasal cavity or nasopharynx. Thus the patient was clinically diagnosed with in

A Report of Two Cases of Planum Sphenoidale Injuries With Cerebrospinal Rhinorrhea Following Office Balloon Dilation of the Sphenoid Ostium: Lessons Learned.

Balloon-assisted dilation (BAD) of paranasal sinus ostia is a Food and Drug Administration (FDA)-approved minimally-invasive procedure used to treat medically refractory chronic rhinosinusitis. Several large cohort studies have reported relatively-low complication rates with BAD. Thus, users of this technology may perceive this to be a safer alternative to formal dissection of the frontal and sphenoid sinus and perform these as office-based procedures. Here, we present 2 patients who underwent BAD of the sphenoid sinus and developed cerebrospinal fluid leak (CSF) leaks with pneumocephalus. Bot

Pneumosinus dilatans of the sphenoid and visual loss: when should the optic nerve be decompressed?

Pneumosinus dilatans of the sphenoid sinus is a rare disorder which can be responsible for visual impairment and blindness. We present the case of an adolescent female who experienced progressive decrease in right-eye vision over 2 years. CT scan of the head showed an extensive pneumatization of the sphenoid bone extending to the lesser wing of the sphenoid and to the anterior clinoid process on the right side. MRI revealed right nerve atrophy in the optic canal and in the posterior part of the orbit. A surgical decompression of the right optic canal was performed via an intradural fronto-pter

Endonasal Exposure of Lateral Recess of the Sphenoid Sinus: Significance of Pterygoid Process Pneumatization.

BACKGROUND: Caudal pneumatization of the pterygoid process may impact endonasal exposure of the lateral recess of sphenoid sinus (LRSS). OBJECTIVES: This study aims to explore the implications of a pneumatized pterygoid process for an endonasal transpterygoid approach to the LRSS and to define strategies regarding the preservation or sacrifice of the vidian nerve. METHODS: Dissection of the LRSS (11 sides) was performed on 6 cadaveric specimens, preselected for the radiographic presence of an LRSS. In addition, the dimensions of the LRSS were measured on the deidentified CT images of 120 pat
