Cauda equina syndrome.

Single or double-level compression of the lumbosacral nerve roots located in the dural sac results in a polyradicular symptomatology clinically diagnosed as cauda equina syndrome. The cauda equina nerve roots provide the sensory and motor innervation of most of the lower extremities, the pelvic floor and the sphincters. Therefore, in a fully developed cauda equina syndrome, multiple signs of sensory disorders may appear. These disorders include low-back pain, saddle anesthesia, bilateral sciatica, then motor weakness of the lower extremities or chronic paraplegia and, bladder dysfunction. Mult

Evaluation of Conditions for the Development of Cryogenic Spinal Cord Injury Using a Canine Model: An Experimental Study on the Safety of Cryoablation for Metastatic Spinal Tumors.

BACKGROUND AND PURPOSE: Although the application of cryoablation to metastatic spinal tumors has been attempted, spinal cryoablation has the unique complication of cryogenic spinal cord injury. This study aimed to elucidate the conditions for the development of cryogenic spinal cord injury. MATERIALS AND METHODS: Fifteen canines were used in this study. A metal probe was inserted into the 13th thoracic vertebral body. Cryoablation was performed for 10 minutes by freezing the probe in liquid nitrogen. The control canine underwent probe insertion only. Spinal cord monitoring, epidural temperatu

Stem cells--a clinical update.

BACKGROUND: Stem cells of adult origin have been used clinically for 40 years in the treatment of haematological neoplasms such as leukaemia. These cells were originally obtained from bone marrow, but are now also being derived from umbilical cord blood. OBJECTIVE: With the increasing public awareness of stem cell use, general practitioners need to be aware for which disorders these cells can, and are, being used. DISCUSSION: Recent clinical trials with stem cells have been for ischaemic heart disease and to assist nonunion of bone. Other adult stem cells used in clinical trials include olfa

Neoplastic meningitis as the presentation of occult primitive neuroectodermal tumors.

Seven children and young adults initially presented with subacute meningitis and/or increased intracranial pressure. The diagnosis of neoplastic meningitis secondary to a primitive neuroectodermal neoplasm was delayed by the absence of an obvious primary tumor. The neuroradiologic appearance was that of a basimeningeal infiltrative process, complicated by communicating hydrocephalus or "pseudotumor cerebri." Myelography was important in the diagnosis of disseminated meningeal malignancy in four cases. Cerebrospinal fluid cytologic diagnosis was insensitive but ultimately confirmed in five case

Atypical teratoid/rhabdoid tumor with 26-year overall survival: case report.

Atypical teratoid/rhabdoid tumor (AT/RT) is an aggressive embryonic brain tumor predominantly seen in young children. The authors report an unusual case of a fourth ventricle AT/RT in an infant who survived for 26 years. The tumor was resected when the patient was 6 months of age, and radiation therapy (40-Gy total dose) was performed thereafter. The patient was free from the disease for 26 years until a recurrent tumor was found in the spinal cord. The spinal cord neoplasm was a "collision tumor" with 2 components: benign schwannoma and recurrent AT/RT. The patient died of dissemination of th

Granulocytic sarcoma, an undiagnosed leukemia, initially manifested as paralysis.

Granulocytic sarcoma (GS) may be a presenting sign of myelogenous leukemia. Occasionally, an extramedullary neoplasm composed from myelocytic precursor cells occurs in patients without evidence of leukemia. Rarely, undiagnosed leukemia occurs initially manifesting with paralysis to spinal cord GS. We present a case report of 20-year-old girl with an undiagnosed leukemia, initially manifesting as paralysis. En bloc spondylectomy with chemotherapy postoperatively constituted the treatment of choice for this tumor. After two courses of chemotherapy, the patient made a good postoperative recovery

Local and diffuse spread of juvenile cerebellar astrocytomas in subarachnoid space. Light and electron microscopic study.

Based on 5 cases of juvenile astrocytomas the unusual behaviour of this type of tumor is described. It is expressed by local and/or diffuse spread of the neoplasms in the subarachnoid space of the brain hemispheres, cerebellum and spinal cord. The light and electron microscopic examinations have revealed the morphological features of well differentiated tumors of astrocytic origin and of fibrillary type which in a great number of cases have a favourable prognosis. The cause of break through into the subarachnoid space, despite its morphology which indicates on a high degree of differentiation

Cryosurgery re-visited for the removal and destruction of brain, spinal and orbital tumours.

Advances in neuroimaging and cryosurgical techniques have prompted us to re-evaluate the potential of cryosurgical techniques for the removal and the destruction of various neoplasms. We have used cryosurgical instrumentation to remove tumours in the brain, spine and orbit in 71 patients without complications. Cryosurgery was used to facilitate removal and extraction in 64 and to destroy residual neoplasms when removal was incomplete in 7. Intraoperative real time ultrasonic imaging permitted precise delimitation of tumours from surrounding tissues and allowed monitoring during the production

A 10-year old girl with neck pain.

Neuromyelitis optica (NMO), (Devic's disease), is a relatively uncommon autoimmune disease predominantly involving the spinal cord and optic nerves. We present a 10 year-old female with intermittent neck pain, progressive right upper and bilateral lower extremity weakness. MR imaging and intraoperative findings were strongly suggestive of a neoplastic process. However, pathologic examination showed an inflammatory demyelinating lesion and serological studies were positive for NMO-IgG. The patient improved dramatically following resection of the compressive "pseudotumor", with resolution of her

Dumbbell-shaped paraganglioma of the cervical spine in a child.

Paragangliomas are tumors arising from extra-adrenal paraganglia and account for 0.3% of all neoplasms. The carotid body and temporal bone are the most frequent sites of paraganglioma, though it has been reported to occur in almost every part of the body. Spinal paragangliomas are uncommon and mainly take the form of intradural compression of the cauda equina or thoracic cord. Extradural paragangliomas are still rare; only few cases involving the dorsolumbar spine have been reported. We are reporting an interesting case of a dumbbell-shaped primary extradural paraganglioma of the cervical spin

Magnetic resonance imaging of the pediatric spine.

Magnetic resonance is an excellent modality for imaging the pediatric spine. Its successful use requires understanding both the basic physics and the sedation protocols necessary for acquiring high-resolution images. Interpreting the images accurately depends on appreciating the differences between the normal anatomy of the pediatric and the adult spine. Evaluating the images requires familiarity with the differential diagnosis of pediatric spine disease, including the most common processes (infections, neoplasms, and trauma) as well as spinal dysraphism. Despite the acknowledged usefulness of

Magnetic resonance imaging of the bone marrow in hematological malignancies.

Despite its lack of specificity, magnetic resonance imaging (MRI) of the bone marrow has the potential to play a role in the management of patients with primary neoplastic disorders of the hematopoietic system, including lymphomas, leukemias and multiple myeloma. In addition to its use in the assessment of suspected spinal cord compression, bone marrow MRI could be used as a prognostic method or as a technique to assess the response to treatment. The current review addresses the common patterns of bone marrow involvement observed in primary neoplasms of the bone marrow, basic technical princip

Microsurgical resection of a giant cervico-medullary ependymoma: 2D-dimensional video.

BACKGROUND: Ependymoma is a slowly growing benign neoplasm that constitutes 3-9% of all neuroepithelial spinal cord tumors.[3,4] They rarely involve the cervicomedullary junction where they both compress the distal brainstem and upper cervical cord. Due to the critical contiguous structures, gross total resection of these lesions may result in significant morbidity/mortality.[1,2] Utilizing intraoperative neuromonitoring can help limit the risks of removing these lesions. Not when considering the risk/complications of partial versus total resection, the surgeon should keep in mind that they ar

A rare case of atypical intradural extramedullary glioblastoma diagnosed utilizing next-generation sequencing and methylation profiling: illustrative case.

BACKGROUND: Primary spinal cord tumors, especially primary spinal cord glioblastoma multiforme (PSC-GBM), are exceptionally rare, accounting for less than 1.5% of all spinal tumors. Their infrequency and aggressive yet atypical presentation make diagnosis challenging. In uncertain cases, a surgical approach for tissue diagnosis is often optimal. OBSERVATIONS: A 76-year-old male presented with a rapidly progressing clinical history marked by worsening extremity weakness, urinary retention, and periodic fecal incontinence alongside diffuse changes on neuraxis imaging. The patient, in whom subac

Diffuse Meningiomatosis without Neurofibromatosis: A Rare Diagnosis with Atypical Presentation.

Meningiomas are amongst the most common neoplasms of the central nervous system; however, "multiple meningiomas" or "meningiomatosis" account for < 10% of cases. The association with neurofibromatosis 2 is seen in ∼50% of cases. We report a case of 35-year-old female patient who presented with left eye proptosis, left forehead swelling, and multiple cranial nerve palsies predominantly on the left side. Imaging evaluation revealed innumerable en plaque meningiomas forming a sheet-like nodular thickening along the dura, causing marked hyperostosis and extending into the orbits, cavernous sinuses

Intraspinal lipomas without associated spinal dysraphism.

INTRODUCTION: The aim of this study was to report surgical strategies and clinical outcomes for thoraco-lumbar intradural lipomas. Intraspinal lipomas are rare congenital histologically benign neoplasms, which account for less than 1% of all spinal cord tumors. These tumors are most frequently found in the lumbosacral area as components of a dysraphic state, however, intramedullary lipomas are not associated with spina bifida or cutaneous malformations and have only been described as isolated cases among spinal lipomas, where the thoracolumbar region is rarely affected. CASE PRESENTATION: Thr

Embryonal neuroepithelial tumors induced by human adenovirus type 12 in rodents. 2. Tumor induction in the central nervous system.

Intracranial inoculation with human adenovirus type 12 (Ad12) induced tumors multicentrically in the brain and spinal cord of 37.2% of hamsters, 30.2% of mice, and in the brains of 91.0% of rats. Brain tumors developed preferentially at the olfactory bulb, lateral ventricular horns, tapetum region, and ventral and dorso-caudal aspects of the fourth ventricle. In the spinal cord, tumor developed on the dorsal aspect and, in hamsters, at the root of the cauda equina. Microtumors were found almost invariably in the subependymal areas and occasionally in the leptomeninges. The histological and ult
