[Spinal localization of Paget disease--case report].

INTRODUCTION: Paget disease, localized on thoracal vertebrae, presents a significant challenge in diagnostics and treatment. Presenting with progressive neurological symptoms, it has a broad differential diagnostic spectrum and requires additio-nal radiological, neurological and endocrinological evaluation. Besides drug therapy, an operative decompression of nerve elements is performed when indicated. CASE OUTLINE: We present a case of an older male patient complaining of chronic progressive pain localized at fourth and fifth thoracal vertebrae, with slight but progressive hypertonus and hype

Acquired spinal cord and peripheral nerve disease.

Acquired spinal cord diseases in ruminants result most commonly from infectious, traumatic, metabolic/nutritional, or toxic causes and rarely from neoplasia. Clinical signs of spinal cord disease depend on the neuroanatomic location of the lesion. Acquired spinal cord diseases including vertebral osteomyelitis/spinal abscess, cauda-equina disease, enzootic ataxia, lymphosarcoma, polyradiculoneuritis, and degenerative myeloencephalopathy are discussed. Acquired peripheral nerve disease in cattle most often is a result of injury, and most commonly only one limb is involved. Peripheral nerve injur

Spinal hydatid disease and its neurological complications.

Bone infection is estimated to represent 0.5-2.5% of cases of hydatid disease. Of these cases, the spine is affected in about 50%. 11 cases of histologically confirmed spinal hydatid disease treated surgically in 2 institutions in Spain and Lebanon were followed up clinically and radiologically for an average of 20 y (1-34 y). On presentation, all patients had severe neurological deficits. All patients underwent posterior spinal decompression, curettage, drainage and washout of paravertebral cyst cavities. Over the period of follow-up, patients underwent an average 4.8 spinal surgical procedur

Segmental hyperhidrosis as a manifestation of spinal and paraspinal disease.

BACKGROUND: Segmental hyperhidrosis is an uncommon finding which is usually associated with irritation or infiltration of pre-ganglionic sympathetic fibres or the sympathetic chain. METHODS: We report two cases of segmental hyperhidrosis with striking clinical features. RESULTS: In one case, a mesothelioma produced ipsilateral simultaneous underactivity and overactivity of sympathetic outflow and in the other case a thoracic central disc herniation was probably responsible for a band of sweating which clearly extended beyond the segmental level of injury. CONCLUSION: Segmental hyperhidrosis

Spinal involvement in children and adolescents with Gaucher disease.

Nineteen patients with type I Gaucher disease with spinal involvement first diagnosed during childhood, were followed for 2 to 24 years (average 9 years), and their clinical and radiologic history reviewed. Patients presented with three types of pain: mild pain that was defined as nonspecific, severe pain due to bone crisis, and pain associated with vertebral collapse. Collapse of vertebra occurred gradually anywhere along the thoraco-lumbar spine, and usually more than one vertebra was involved. In three patients, rectangular collapse was noted. Six patients suffered from central vertebra col

Spinal instrumentation for degenerative disease of the lumbar spine.

Instrumentation for spinal implant is currently changing more rapidly than at any previous period in its 100-year history. At present, the most common indication for spinal instrumentation is degenerative disease of the lumbar spine, which can be roughly categorized as of three kinds: herniated lumbar disks, spondylolisthesis, and spinal stenosis. The literature on indications for arthrodesis is reviewed. A brief summary of the approach and type of instrumentation available for the lumbar spine is presented.

Spinal cord compression secondary to Gaucher's disease.

A case of compression of the spinal cord and cauda equina secondary to collapse of the T-12 and L-3 vertebral bodies in a patient with type 1 Gaucher's disease is presented. Decompressive laminectomies were done at both the T-12 and L-3 levels with moderate improvement of the patient's neurologic findings. Histologic examinations confirmed the presence of Gaucher's cells in the bone. Review of the literature revealed only one previous report.

Multiple spinal hemangioblastomas in a case of Lindau's disease.

A 32-year-old man with Lindau's syndrome is presented. After an initial operation for cerebellar hemangioblastoma and laser coagulation of retinal tumors, other retinal tumors, a second cerebellar tumor, and four spinal hemangioblastomas occurred 10 years later; cystic lesions of both kidneys and of the pancreas could also be shown. There is no family history. Consequences for further diagnostic procedures in patients with hemangioblastomas are discussed.

Stenosis of the lumbar spinal canal in Forestier's disease.

Hyperostosis of the spine, Forestier's disease, is a rare, but well defined entity which results from silent, extensive paraspinal calcification. It has been regarded as an asymptomatic ankylosis requiring no medical or surgical treatment. Two patients with Forestier's disease are presented who show severe stenosis of the lumbar canal with early symptoms of neurological compression.

Acute spinal cord compression in Scheuermann's disease.

Acute myelopathy is a rare complication of Scheuermann's disease. Three patients are reported where spinal cord compression occurred at the apex of a kyphos. All were male, aged 14, 18 and 20 years, and each had a profound neurological defect associated with a short, sharp kyphos in the low thoracic region. Each patient underwent anterior decompression and all made an almost full recovery. It is deduced that factors which may influence the onset of cord compression include the angle of kyphosis, the number of segments involved, the rate of change of the angle, local anatomical variations, trau

[Boeck's disease with spinal involvement and retroperitoneal fibrosis].

Sarcoidosis (Morbus Boeck) is a frequently found disease of the reticulohistiocytic system. Etiology and pathogenesis were mostly unknown until today. Histologic study of sarcoidosis shown granuloma with epitheloid cells similar to tuberculosis, often with hyalinization but without cheesy necrosis. The disease mostly starts with a mediastinal and cervical lymphadenopathy. Radiographically, the disease shows three typical stages when spreading to the lungs. Bone involvement in lymphomas, especially vertebral involvement, is found to be rare. A case with an additional retroperitoneal fibrosis is

The Role of Adipokines in Spinal Disease: A Narrative Review.

BACKGROUND: Adipokines are bioactive molecules secreted by adipose tissue that influence both local and systemic physiological processes. Although adipokine dysregulation has been widely implicated in the pathogenesis of cardiovascular, gastrointestinal, nervous, immune, and musculoskeletal disorders- especially osteoarthritis, rheumatoid arthritis, and osteoporosis- their involvement in spinal pathology remains comparatively underinvestigated. Recent evidence indicates that certain adipokines may influence spinal disease progression by affecting bone, intervertebral discs, musculature, and ne

In Search of Spinal Muscular Atrophy Disease Modifiers.

The 5q Spinal Muscular Atrophy (SMA) is a hereditary autosomal recessive disease caused by defects in the survival motor neuron (SMN1) gene encoding survival motor neuron (SMN) protein. Currently, it is the leading cause of infantile mortality worldwide. SMA is a progressive neurodegenerative disease with "continuum of clinical severity", which can be modulated by genetic and epigenetic factors known as disease modifiers (DMs). Individuals (even siblings) with the same defects in SMN1 gene might have strikingly different types of SMA, supposedly due to the impact of DMs. There are several ther
