OBJECTIVE: Degenerative lumbar spondylolisthesis (DLS) is a common degenerative disease that causes low back pain and lower extremity pain. Transforaminal Lumbar Interbody Fusion (TLIF) is an effective surgical method for treating this condition. However, postoperative complications exist, such as cage subsidence, whose causes are complex. This study investigated the characteristics and risk factors of cage subsidence after TLIF in DLS patients.
METHODS: A total of 131 TLIF patients were divided into the subsidence group (subsidence group, n=39) and the non-subsidence group (non-subsidence gr

PURPOSE: To evaluate the effect, in long-term postoperative follow-up (more than 15 years), of combined spinal fusion (anterior and posterior) and only posterior on the pulmonary function of patients with congenital scoliosis.
METHODS: Case series with five patients, operated on from 03/1997 to 12/2009, groups: dual approach with anterior arthrodesis through thoracotomy versus only posterior arthrodesis. Data processed in SPSS 20.0. Comparison of means (Student's t-test and Anova, or Mann-Whitney and Kruskal-Wallis/Dunn) with p = 0.05.
RESULTS: There was no difference in the absolute and pre

BACKGROUND CONTEXT: It is a common practice to obtain immediate postoperative radiographs prior to hospital discharge after instrumented spinal fusion. There is limited evidence to support the clinical benefits of such a practice, which may result in unnecessary radiation exposure and added healthcare costs.
PURPOSE: To determine differences in 90-day and 1-year reoperation between patients who underwent instrumented cervical, thoracic, or lumbar fusion and received immediate postoperative radiographs prior to hospital discharge compared to those who did not.
STUDY DESIGN: Retrospective coho

BACKGROUND: Little data exist on pregnancy and childbirth for adolescent idiopathic scoliosis (AIS) patients treated with a spinal fusion. The current literature relies on data from patients treated with spinal fusion techniques and instrumentation, such as Harrington rods, that are no longer in use. The objective of our study is to understand the effects of spinal fusion in adolescence on pregnancy and childbirth.
METHODS: Prospectively collected data of AIS patients undergoing posterior spinal fusion that were enrolled in a multicenter study who have had a pregnancy and childbirth were revi

BACKGROUND: Previous studies identified a correlation between preoperative resilience scores and patient reported outcome measures in several surgical subspecialities. No previous studies, to our knowledge, have analyzed preoperative resilience and patient reported outcomes in lumbar spinal fusion.
METHODS: Patients undergoing lumbar spinal fusion completed the Brief Resilience Scale (BRS) preoperatively, in addition to measures of disability (Oswestry Disability index [ODI]), quality of life (PROMIS global physical and mental health scales and EuroQol5), and leg and back pain (VAS) at pre- a

BACKGROUND: Obtaining successful arthrodesis at the craniocervical junction and atlantoaxial joint can be more challenging than in other segments of the cervical spine. This challenge stems from the relatively hypermobile joints between the occipital condyles, the motion that occurs at C1 and C2, as well as the paucity of dorsal bony surfaces for posterior arthrodesis. While multiple different techniques for spinal fixation in this region have been well described, there has been little investigation into auxiliary methods to improve fusion rates.
OBJECTIVE: To describe the use of an occipital

BACKGROUND: Surgeons have hesitated to use steroids in patients undergoing posterior spinal fusion because of the risk of wound complications. The literature has supported the use of postoperative steroids in other areas of orthopaedics on the basis of more rapid recovery and improved postoperative pain control. We hypothesized that a short course of postoperative dexamethasone following posterior spinal fusion for the treatment of adolescent idiopathic scoliosis (AIS) would decrease opioid usage without increasing wound-healing problems.
METHODS: Consecutive patients undergoing posterior spi

Neurosurgical patients with titanium spinal implant hypersensitivity can be difficult to diagnosis due to its rarity. Suspicion for titanium allergy is generally localized to the hardware site and may initially be thought to be an infectious process. Patients who report anorexia and fatigue over a long duration after the initial post-operative period may be diagnosed with depression rather than a systemic response to spinal metallic instrumentation. To our knowledge, a systemic titanium hypersensitivity reaction to spinal fixation devices has not been reported in the literature. We offer this

Pyoderma gangrenosum (PG) is a rare inflammatory dermatosis that is most often associated with inflammatory bowel disease, but which can occur as a pathergic reaction around surgical incisions. The authors report the case of a patient who developed postoperative PG over the course of several months after undergoing extensive spinal instrumentation between the T4 and iliac levels. This is only the second such case occurring after spine surgery to be reported. The authors additionally review the literature to characterize treatment approaches and outcomes for this condition. The case highlights

OBJECTIVE: Adjacent segment disease (ASD) is a long-term complication of lumbar spinal fusion. This study aims to evaluate demographic and operative factors that influence development of ASD after fusion for lumbar degenerative pathologies.
METHODS: A retrospective cohort study was performed on patients undergoing instrumented lumbar fusion for degenerative disorders (spondylolisthesis, stenosis, or intervertebral disk degeneration) with a minimum follow-up of 6 months.
RESULTS: Our inclusion criteria were met by 568 patients; 29.4% of patients had developed surgical ASD. Median follow-up wa

The aim of this study is to assess the impact of preoperative comanagement with complex care pediatricians (CCP) on children with neuromuscular scoliosis undergoing spinal fusion. We performed chart review of 79 children aged 5-21 years undergoing spinal fusion 1/2014-6/2016 at a children's hospital, with abstraction of clinical documentation from preoperative health evaluations performed regularly by anesthesiologists and irregularly by a CCP. Preoperative referrals to specialists, labs, tests, and care plans needed last minute for surgical clearance were measured. The mean age at surgery was

PURPOSE: Adjacent segment degeneration (ASDeg) and disease (ASDis) have become major concerns after fusion surgery. However, there is no definitive data or knowledge about the incidence or risk factors. The review discusses the incidence and risk factors and prevention of ASDeg and ASDis in the relevant literature.
METHODS: We performed a systematic review of meta-analyses, randomized control trials, and cohort studies published in English to provide evidence-based information about ASDeg and ASDis.
RESULTS: According to a meta-analysis, the pooled incidence of ASDeg after lumbar and cervica

BACKGROUND: Somatosensory evoked potentials (SSEPs) and motor evoked potentials (MEPs) are frequently used to monitor neurologic function during spinal deformity surgery. The sensitivity and specificity of intraoperative neurophysiologic monitoring (IONM) in patients undergoing posterior spinal fusion (PSF) is debatable.
METHODS: A retrospective review of all patients undergoing PSF with IONM from October 2008 to December 2012 was performed. Factors including sex, operative time, and spinal levels of posterior fusion were analyzed as risk factors for intraoperative alerts.
RESULTS: A total o

The first case of a spinal epidural abscess caused by Roseomonas mucosa following instrumented posterior lumbar fusion is presented. Although rare, because of its highly resistant profile, Roseomonas species should be included in the differential diagnosis of epidural abscesses in both immunocompromised and immunocompetent hosts.

BACKGROUND: To examine the survival function and prognostic factors of the adjacent segments based on a second operation after thoracolumbar spinal fusion.
METHODS: This retrospective study reviewed 3,188 patients (3,193 cases) who underwent a thoracolumbar spinal fusion at the author's hospital. Survival analysis was performed on the event of a second operation due to adjacent segment degeneration. The prognostic factors, such as the cause of the disease, surgical procedure, age, gender and number of fusion segments, were examined. Sagittal alignment and the location of the adjacent segment

BACKGROUND: Posterolateral fusion with cages and posterior instrumentation is an accepted method in the treatment of lumbar instability associated with spinal stenosis or scoliotic deformity, but with modest results. We propose hereby an alternative, simple method to treat kyphosis due to the vertebral fracture which has brought about comparable outcomes.
METHODS: Three patients with documented adjacent segment compression fractures were treated. Vertebroplasty was performed with polymethylmethacrylate (PMMA), either using the transpedicular route at the adjacent level or via the route of the

Posterior lumbar spinal fusion is a common surgery of spine. The parapsinal muscular dysfunction was postulated to be due to injured paraspinal muscle inflicted by the surgery. To better understand the muscle dysfunction after spinal fusion, this descriptive study evaluated electrophysiology and histology changes in paraspinal muscle of rabbits with and without fusion. Three New Zealand white rabbits underwent 2-level posterior spinal fusion with instrumentation. Anothor 3 rabbits underwent surgical exposure alone and served as sham control. Pre-operative and follow-up electromyography (EMG) a
