Splenic lymphoma arising in a patient with Gaucher disease. A case report and review of the literature.

Patients with Gaucher disease have an increased risk of malignancies, especially the lymphoreticular type. To our knowledge, this is the first reported case of Gaucher disease diagnosed during the workup and diagnosis of a splenic marginal-zone lymphoma with progression to diffuse large B-cell lymphoma. There are several theories as to how Gaucher disease leads to malignancies. The accumulated glucocerebroside in the reticuloendothelial organs, histologically visible as Gaucher cells, is thought to provide chronic antigenic stimulus to the immune system. Polyclonal hypergammaglobulinemia devel

Impact of long-term erythrocytapheresis on splenic function in patients with sickle cell disease.

Fifteen children with sickle cell disease undergoing long-term erythrocytapheresis were assessed for splenic regeneration using abdominal ultrasound and radionuclide spleen scans. Despite a significant level of reduction of hemoglobin S (HbS), none of the 15 patients showed evidence of splenic regeneration. Comparing our results with other published reports on hypertransfusion therapy in patients with sickle cell disease, it appears that the process of splenic regeneration in these patients is minimal and may depend partly on the intensity of transfusion therapy and the length of time that HbS

Acute splenic sequestration crisis in sickle cell disease: early detection and treatment.

Acute splenic sequestration crisis (ASSC) in children with various forms of sickle cell disease can result in life-threatening circulatory collapse due to the loss of circulating blood volume. Over a 6-year period we have treated 12 patients ranging in age from 5 1/2 months to 7 years presenting with acute sequestration crisis. Eleven had homozygous sickle cell disease and the other had sickle-thalassemia. One patient died of acute circulatory collapse. Eight patients underwent splenectomy after a major episode of sequestration with no serious infectious complications up to 5 years following s

Splenic visualization using 99mTc-methylene diphosphonate in a patient with sickle cell disease.

Splenic visualization with a bone-seeking agent 99mTc-methylene diphosphonate (99mTc-MDP) with absence of splenic uptake using sulfur colloid was seen in a 15-year-old boy in sickle cell crisis. Uptake of 99mTc-MDP is probably due to splenic infarction and calcification. This represents a potentially sensitive method for following the functional auto-splenectomy process in sickle cell disease and may also detect the presence of splenic tissue when it is not visualized in a sulfur colloid liver/spleen scan. Splenic visualization by bone-seeking agents should not be confused with skeletal lesion

Atypical Cat Scratch Disease With Splenic Lesion Mimicking Kawasaki Disease in a Healthy 5-Year-old Girl.

BACKGROUND: Atypical cat scratch disease (CSD) and Kawasaki disease (KD) are differential diagnoses of pediatric febrile illnesses. Diagnosing atypical CSD can be challenging because of its wide range of symptoms. However, its similarity to KD has rarely been addressed. METHODS: We present the case of a 5-year-old girl with atypical CSD and splenic lesions who fulfilled the diagnostic criteria for KD. We also conducted a literature review of previous cases in which CSD was suspected alongside KD and detailed the diagnosis and treatment processes. RESULTS: A previously healthy 5-year-old girl

Case Report of Primary Splenic Angiosarcoma in a Young Patient: A Very Rare and Challenging Disease.

INTRODUCTION: Primary splenic angiosarcoma (PSA) is a rare type of angiosarcoma often associated with an unfavorable prognosis. The presenting symptoms are usually nonspecific, adding to the diagnostic difficulty of this disease. Limited cases have been described in younger adults, as most cases have been reported in individuals between 50 and 70 years old. PRESENTATION OF CASE: We report the case of a 25-year-old patient with cerebral palsy who initially presented for high-grade fever and left upper quadrant (LUQ) pain. On computed tomography (CT) scan, a large heterogenous mass was discover

Endovascular Fiducial Placement in Splenic Metastatic Disease as a Novel Option for Radiotherapy: A Case Report.

Splenic metastases are rare but difficult-to-treat entities, especially if they recur after initial surgery or ablation. They are particularly difficult to treat with radiation therapy due to their subdiaphragmatic location and the presence of respiratory excursions. This report describes a case in which an endovascular fiducial placement approach was used to mark the splenic metastasis prior to radiation therapy. We present a 77-year-old male with an extensive history of colorectal metastatic disease. After hemicolectomy, the patient showed metastases - first in the spleen and then in the liv

A life-threatening discovery: How hematemesis unveiled a splenic artery aneurysm in Behçet's disease.

Behçet's disease BD is a rare, chronic, systemic vasculitis of unknown origin characterized by recurrences of inflammatory episodes that may affect various organs, including the skin, eyes, joints, nervous system, and blood vessels. The vascular involvement in BD, generally referred to as Angio-Behçet, predominantly involves the venous system, although arterial involvement is less common. The arterial complications include stenosis, occlusion, or aneurysm. Peripheral and pulmonary arteries may be involved, while involvement of the splenic artery is exceptionally rare. This report describes an

Splenic and portal venous flow associated with frailty and sarcopenia in older outpatients with cardiovascular disease.

BACKGROUND: Older patients with cardiovascular disease often experience frailty and sarcopenia. We evaluated whether a reduced blood flow in the splenic and portal vein is associated with frailty and sarcopenia in older patients with cardiovascular disease. METHODS: Blood flow in the splenic and portal vein was evaluated using EPIQ7 (Philips) in older patients (aged ≥ 65 years, 123 patients) with cardiovascular disease, who visited the frailty outpatient clinic. Frailty was assessed using the Japanese version of Cardiovascular Health Study (J-CHS) criteria and the Kihon Checklist (KCL), while

Splenic sequestration crisis in children with sickle cell disease in the Eastern region of Saudi Arabia.

BACKGROUND: Acute splenic sequestration crisis (ASSC) is a potentially life-threatening complication of sickle cell disease (SCD). Previous studies have shown that patients who carry the African sickle gene haplotypes have more severe SCD than those with the Asian haplotype. In Saudi Arabia (SA), people living in the eastern region are known to carry the Asian haplotype, whereas patients in the southwestern (SW) region are more likely to carry the African haplotype. The aim of this study was to determine the prevalence and clinical features of ASSC among children with SCD who originated from b

Yellow Fever Vaccine-Associated Viscerotropic Disease: A Unique Case of Atraumatic Splenic Rupture.

Atraumatic splenic rupture is an uncommon, life-threatening abdominal emergency involving the rupture of the spleen in the absence of obvious trauma. This case report provides a summary of a 48-year-old male who presented to the Emergency Department of the University of Ghana Medical Centre with syncope and hemodynamic instability one week after receiving a yellow fever vaccine. A diagnosis of an atraumatic splenic rupture was eventually confirmed by imaging, and a splenectomy was done. This report aims to provide valuable insights into the recognition and management of atraumatic splenic rupt

The impact of hepatic and splenic volumetric assessment in imaging for chronic liver disease: a narrative review.

Chronic liver disease is responsible for significant morbidity and mortality worldwide. Abdominal computed tomography (CT) and magnetic resonance imaging (MRI) can fully visualise the liver and adjacent structures in the upper abdomen providing a reproducible assessment of the liver and biliary system and can detect features of portal hypertension. Subjective interpretation of CT and MRI in the assessment of liver parenchyma for early and advanced stages of fibrosis (pre-cirrhosis), as well as severity of portal hypertension, is limited. Quantitative and reproducible measurements of hepatic an

Spontaneous Resolution and Recurrence of Aseptic Splenic Abscesses in a Patient With Newly Diagnosed Crohn's Disease.

Aseptic splenic abscesses are rare in the early phases of Crohn's disease and are typically reported in patients with longstanding illness or uncontrolled symptoms despite medical treatment. We present a case of recurrent aseptic splenic abscesses in a young man newly diagnosed with Crohn's disease, whose illness remained well-controlled. This unique case raises questions regarding the spontaneous resolution of aseptic splenic abscesses without steroid therapy and their recurrence without acute Crohn's disease flare-ups.

First Report of an Association Between Crohn's Disease and Isolated Splenic Tuberculosis in Pediatric Population.

Isolated splenic tuberculosis (TB) in children is extremely rare, and congenital or acquired immunodeficiency is usually a predisposing factor for this disease. Herein, we report a case of isolated splenic TB in a 5.5-year-old child associated with Crohn's disease. As far as we are aware, this association is reported for the first time in children. Clinicians should be aware and consider extra pulmonary TB, especially in endemic regions. In addition, evaluation of an underlying disorder in unusual presentations of TB is advisable.

Undetectable minimal residual disease is an independent prognostic factor in splenic marginal zone lymphoma.

The role of minimal residual disease (MRD) in splenic marginal zone lymphoma (SMZL) has not been well studied. We prospectively designed a study to evaluate undetectable MRD (uMRD) by multiparameter flow cytometry as a prognostic factor. Residual disease level of <0·01% was defined as uMRD. A total of 71 newly diagnosed patients with bone marrow involvement were enrolled and all received rituximab-based therapy. The overall response rate (ORR) was 98·5% (70/71), with a complete remission (CR) rate of 54·9% (39/71). There were a total of 295 MRD detections in bone marrow and 77·4% patients (55/

Traumatic splenic laceration with delayed rupture secondary to coughing in a patient with Von Willebrand disease.

We describe the case of a 54-year-old male with Von Willebrand Disease who presented to the Emergency Department (ED) with 2 weeks of worsening abdominal pain after falling on his left flank while boating. On his initial presentation, he was found to have a Grade II splenic injury that was managed non operatively by the trauma service. Four days later, he returned to the ED when he developed severe abdominal pain after coughing and was found to have active extravasation from the splenic parenchyma with hemoperitoneum on CT angiography and a grossly positive FAST exam. Intraoperatively, he was

[Hepato-splenic involvement, an unusual finding secondary to cat-scratch disease: a clinical case].

The infection by Bartonela henselae (BH), the cause of cat scratch disease, it could be asymptomatic or produce local and multisystem illness. The objective of this case report is to document that the hepato-splenic involvement is unusual in BH infection, and the treatment is discussed and individualized in each patient. This case is about an eleven-year girl who presented with findings in abdominal tomography and ultrasound of hepato-splenic abscesses, with later positive serology for BH. In this way, a bibliographic review is carried out to show the low prevalence and incidence of hepato-spl
