Portal venous thrombosis (PVT) is an uncommon complication in post-liver transplant recipients. The reported incidence is 1-4%. It may occur within a month, called early or after one month of transplantation, known as late PVT. Early PVT has a poor prognosis, leading to graft failure in most cases. Treatment of such cases is quite challenging because of difficult alternative portal inflow establishment. We performed successful thrombolysis of acute major PVT with a unique technique using ultrasound-guided percutaneous trans-splenic vein access in a post-liver transplant recipient. The per-cuta

Splenic vein thrombosis is a relatively common finding in pancreatitis. Gastric variceal bleeding is a life-threatening complication of splenic vein thrombosis, resulting from increased blood flow to short gastric vein. Traditionally, splenectomy is considered the treatment of choice. However, surgery in necrotizing pancreatitis is dangerous, because of severe inflammation, adhesion, and bleeding tendency. In the Warshaw operation, gastric variceal bleeding is rare, even though splenic vein is resected. Because the splenic artery is also resected, blood flow to short gastric vein is not increa

Following the sudden onset of a clinical syndrome of acute portal hypertension in a young woman in the course of childbirth, angiography revealed a voluminous aneurysm of the splenic artery rupture into the splenic vein. Surgical treatment was successful. The authors review the main characteristics of splenic arterio-venous fistulae and discuss the physiopathological problems of portal hypertension associated with such lesions.

BACKGROUND: Splenic vein aneurysms (SVAs) are a rare vascular anomaly of the portal system that are often asymptomatic and found incidentally on imaging. Intervention is reserved for aneurysms that are enlarging, thrombosed, symptomatic, or ruptured to avoid catastrophic complications. This can range from endovascular to open, complex procedures. Here, we present the case of a massive, symptomatic, partially thrombosed SVA that required a hybrid approach involving both minimally invasive and open surgery.
CASE PRESENTATION: A 56-year-old woman presented to the emergency department for an infe

UNLABELLED: Traumatic superior mesenteric artery (SMA) and vein (SMV) injuries are rare but often lethal. The ideal management options of traumatic SMV injury are still controversial. Management options include venous repair and ligation. Splenic vein turndown procedure (SVTP) is a rare procedure that has been described in only 6 cases in the literature. Here, we reviewed the literature on the usage of the splenic vein turndown procedure (SVTP) as an alternative option in patients with traumatic SMV injury.
METHODS: We performed a narrative review for the available literature on the usage of

Relapsing splenic vein thrombosis, a very rare complication of Wegener granulomatosis (WG), is described in a female patient. Positive antiphospholipid antibodies found in this case are a rare occurrence in primary vasculitis, especially in WG. This probably caused or accentuated an effect of the WG on the splenic vein. Treatment of such patients with antiphospholipid syndrome and WG must include anticoagulation and immunosuppression and, as noted in this patient, the splenic vein thrombosis and other antiphospholipid syndrome and WG can resolve quickly.

We present a case of TIPS occlusion in which a small caliber transhepatic safety or anchoring wire in combination with a standard transjugular approach were utilized to recanalize the TIPS, portal and splenic veins. This technique may be a useful adjunct to the typical methods used for recanalization of thrombosed TIPS, portal and splenic veins without the need of large caliber transhepatic sheath access.

Celiac arterial aneurysms are very unusual and often lack clinical manifestations. According to our review of the literature, this is the first report of a patient with a large celiac arterial aneurysm who exhibited hematemesis from gastric varices. Arteriography, as well as color Doppler ultrasonography and enhanced computed tomography, contributed to the diagnosis of this aneurysm, which was best exposed by a left thoracoabdominal approach. In this patient the lesion was a false aneurysm with perforation of the celiac artery, so simple closure of the orifice was carried out and the revascula

Diagnosing leiomyosarcomas of the splenic vein is challenging, and a treatment strategy has not yet been established for this condition. We herein report the case of a 45-year-old female patient with a history of retinoblastoma who underwent pancreatosplenectomy for a primary leiomyosarcoma originating from the splenic vein and transcatheter arterial chemoembolization for metastatic hepatic lesions observed 5 months postoperatively. An initial medical check-up using abdominal ultrasound revealed a 40-mm mass behind the pancreatic tail. Imaging tests revealed a well-circumscribed mass compressi

AIM: Recently, the accessory middle colic artery (AMCA) has been recognized as the vessel that supplies blood to the splenic flexure. However, the positional relationship between the AMCA and inferior mesenteric vein (IMV) has not been evaluated. Herein, we aimed to evaluate the anatomy of the AMCA and the splenic flexure vein (SFV).
METHOD: Two hundred and five patients with colorectal cancer who underwent enhanced CT preoperatively were enrolled in the present study. The locations of the AMCA and IMV were evaluated, focusing on the positional relationship between the vessels and pancreas -

CONTEXT: Knowledge of variations of the vessels closely related to the pancreas adds greatly to the success of pancreatic surgery. Splenic vein is one of the vessels that are closely related to the pancreas. Its variations might not only change the haemodynamics in the vein but also cause unexpected bleeding during pancreatic surgery.
CASE REPORT: During our dissection classes, we noted the variations in the course and termination of the splenic vein. After emerging from the spleen, it coursed upward and to the right, embedded in a groove on the anterior surface of body of pancreas. After rea

OBJECTIVES: The purpose of this study was to determine whether a low splenic artery pulsatility index (PI) and reduced flow through the left portal vein are involved in redistribution of fetal growth restriction.
METHODS: Forty-two women with prenatally diagnosed intrauterine growth restriction of singleton fetuses were included. The next pregnant woman with an appropriately growing fetus, matched for gestational age, was selected as a control. Blood flow velocities were measured in the splenic artery and left portal vein. Obstetric and perinatal information was obtained from standardized med

The authors report their experience with CO(2) splenoportography in a retrospective review of 15 studies performed in 13 patients before or after liver transplantation. The studies were performed by injecting CO(2) through a small caliber needle introduced into the splenic parenchyma with ultrasound guidance and imaging with conventional digital subtraction techniques. The findings were compared with clinical follow-up and other imaging studies when available. Eight of the 15 studies were normal, of which two were confirmed by additional studies. Six of the 15 studies were abnormal, all of whi

Herein, we report the successful performance of a laparoscopy-assisted spleen-preserving distal pancreatectomy with conservation of the splenic artery and vein for a patient with pancreatic cystadenoma, as a minimally invasive procedure with the preservation of function. The laparoscopy-assisted distal pancreatectomy procedure involved detaching the spleen and the distal pancreas from the retroperitoneum by a hand-assisted procedure, removing them from the peritoneal cavity through a small incision, and detaching the distal pancreas by ligating and transecting the short gastric artery and vein
